ILLINOIS 4-H Extension Colleagues: A few notes about the attached SAMPLE Illinois 4-H Experience Award Application.. The intention of this application is to provide you with a sampling of IDEAS as to the type of item/event/activity that can be included in each one of the boxes AND the format in which we wish information to be included. For the actual application, 4-H members should NOT be expected to complete ALL boxes!! They more than likely will not be completing even half of the boxes. Please share that with your 4-H volunteers and members. The object is not to fill in everything; it is to keep track of what the member did experience this past year.
