Risk & Consequence Assessment Table 1: Risk Rating Score Will not result in injury or illness First aid treatment only (no lost time) Medical treatment required (casualty treatment) Serious or multiple injury / illness (permanent disability) Death or multiple lifethreatening injuries CONSEQUENCE Insignificant Minor Moderate Major Catastrophic Expected to occur in most circumstances Almost Certain 6 7 8 9 10 Will probably occur Might occur at some time LIKELIHOOD Likely 5 6 7 8 9 Possible 5 6 7 8 Not likely to occur in normal circumstances Unlikely 5 6 7 May occur but only in exceptional circumstances Rare 2 5 6 Table 2: Risk Score and Action RISK RATING SCORE SCORE Risk Likely Outcome There are two stages in defining Risk on a project site: INHERENT RISK: is the risk with no control measures in place The hazard has the potential to kill or cause a serious injury and has an > 7 High unacceptably high probability ( Might Occur ) of occurrence. TARGET RISK: is the assessed risk after the control measure/s have been implemented. 6 and 7 Medium 5 Low The hazard presents an unacceptable combination of the Likelihood of occurrence versus the seriousness of the resulting injury. The hazard is unlikely to cause significant injury or the probability of a serious injury is Rare or has Never occurred. A risk category of LOW is required before work proceeds. Appropriate control measure/s MUST be implemented until a LOW risk category (risk rating score of 5 or less) is achieved. To derive the Risk Rating Score, the following steps are followed: 1) Assess the Likelihood (ie Risk) 2) Establish the Consequences The intersection of the two provides the risk rating score.
