Affiliation Agreement Pt 1 Due no later than the 31 st October 2018 Administration/Operations/Engagement In affiliating with the Adelaide Footy League (SA Amateur Football League), the member club agrees to the following: SECTION - 1 The Football Club applies for Affiliation. SECTION - 2 TEAM NOMINATION Team Ranking in Club Division (eg D1, D1R, 1 Women, U18) New team in 2018 YES/NO? First Second Third Fourth Fifth Sixth Seventh Eighth Ninth Tenth SECTION 3 CLUB ACCEPTANCE In affiliating with the Adelaide Footy League (SA Amateur Football League), the Member Club agrees to: 1. Abide by The League s Rules and Regulations and Constitution of The League 2. Acknowledge the contents as described in the agreement. For the purpose of considering any application for renewal of membership to The League, The Executive Committee retains the right to examine and inspect the Member Club s records, oval(s), change rooms, clubrooms and facilities.
SECTION 4 CLUB DELEGATE In order to vote at the Annual General Meeting each Member Club must notify The League in writing the names of their Delegate and Proxy Delegate. A Member Club that does not submit names shall be ineligible to vote until such names are submitted. Please complete these details below: Name of Delegate:. Name of Proxy Delegate:. SECTION 5 MAJOR ROUND Any Member Club applying to host a Major Round Match final shall fulfil The League s minimum requirements including: 1. Shall provide signed confirmation of acceptance of these conditions by the required dates. 2. Shall provide a ground surface in a reasonable condition. 3. Shall provide reasonable change rooms facilities for players, umpires and trainers. 4. Shall provide coaching facilities of a sound structure with an ability to shield from inclement weather 5. Shall provide a minimum standard of facilities for spectators being (a) Provision for food and drink (b) Male and Female Toilets (c) Accessible Defibrillator (d) Compliant Stretcher 6. Shall fulfil The League s obligations to its major sponsors. (a) The Member Club shall be sponsored by Lion Nathan Brewery and shall be ratified by a representative of the Lion Nathan Brewery no later than the end of May of the given season (b) The venue shall exclusively supply Lion Nathan Brewery products for this event 7. Shall not have had three or more players found guilty of an Intentional act of striking and or kicking and or head butting and or abuse of an umpire in the current year. 8. Shall have paid all charges, fines or club fees by the due dates in the current or year before unless otherwise agreed in writing by The League. 9. Have participated in an approved Drug and Alcohol Awareness Programme at a minimum of once per three years prior to the year in which the club is applying to host a Home Final. 10. Will ensure all appointed coaches are accredited as per The League s requirements by the first occasion in which the given coach will officiate. 11. Shall have supplied the names and contact details for each of the elected and or appointed Club Officials as set out below by the due date as described in this Agreement.
SECTION 6 COMMUNICATIONS 1. Give permission for photographs to be used by The League. Photographs will be displayed on The Leagues website, used in newsletters/publications and filed in The League Office. 2. Allow video footage to be used in any form The League sees fit. SECTION 7 LEAGUE CALLS/SUBSCRIPTIONS The..Football Club wishes to be invoiced the prescribed amounts at the following frequencies Please tick As per the standard periods of payment I.e. 30 th April, 31 st May, 30 th June and 31 st July The total amount for payment at the first due date of 30 th of April Half 30 th April, half 31 st May SECTION 7 DECLARATION The club agrees to abide by the above conditions and applied to Affiliate with The League for the 2019 season. Resources Coaching Accreditation: Any coach intending to coach at your football club MUST have obtained a minimum accreditation standard of Foundation Level (previous Level 1) to coach within the Adelaide Footy League.
