Fall 2016/17 Registration Packet We are pleased to announce registration for the Fall 2016/17 Season is now open. This packet will provide you with all the information you will need to register. Early Registration Discount (Squirt, Peewee, and Bantam levels only) Take advantage of our Early Registration and save $200! Early registration not only helps reduce your fees and gives you the opportunity to spread the cost across 7 payments, it also allows us to more effectively plan and secure ice for the season. The discount will automatically be applied at registration. Mark your calendars for these important dates! Travel Prep, Mite and Midget levels have the discount built into their pricing and can register at any time for the same price. Key Dates Early Registration ends - Friday 7/15 Warm-Up Skates - Thursday, 8/24 - Monday, 8/28 * Tryouts - Thursday, 9/1 - Monday 9/5 * Bantam Checking Clinic - Friday, 9/9 (tentative) As we head into the 2016/17 hockey season we began a new chapter after officially transitioning from Leafs Hockey Club to West Dundee Hockey Club. The board is excited to focus our energy on making Leafs the best place for your child to play Tier II hockey in Illinois. We are excited to be placing teams at almost every level of the CSDHL, NIHL, and NWHL which will allow all our skaters to be placed in a situation that will be both challenging and fun. You might notice a few changes from past years. There will be no Member Appreciation this year. Many members expressed the desire to have the weekend before tryouts off. This is especially important this year as tryouts will run from September 1-4 with many levels on the ice all 4 days. We want all our skaters to perform at their best during the tryout process. This is a new approach for Leafs, but we are trying to keep our families from having to come to the rink on Labor Day. Early registration is even more important this year as we have less flexibility with ice scheduling than we had in previous years. The more members register early, the clearer picture we ll have of what the needs of the club will be in terms of coaching, skills and ice for the upcoming season. Please follow the following steps: 1. Go to www.usahockeyregistration.com 2. Select ICE PLAYERS & COACHES and follow the instructions. 3. The fee is $53 per player. USAH fees are waived for players age 6 (2010) and younger. Note: Any member who registered with USAH on or after April 1, 2016 is eligible for participation through August 31, 2017. It is important that the information provided in this registration is accurate. Be sure to include contact information for both parents. Which program is right for my player? Kids develop and mature at different rates. We offer these guidelines to help assist you in making your Fall program decision. All players are welcome to try out for any program but when you register, please consider carefully what tryout session you attend. Please see the Mite Program page for details about our mite offerings. Elite AA-A Level Travel (Midget Squirts) - CSDHL and NIHL Players will participate in a competitive closed tryout process (no parent or other spectator viewing). Player cuts can occur at this time. Hockey directors and head coaches evaluate players based on the following criteria: skating, puck control, physical ability, shot strength, play making ability, sense of the game, work ethic and coachability. Each coach may weigh these differently based on coaching philosophy and/or team needs. Players are given a rating by all evaluators. Drafts are held and teams may be selected as early as the first day of tryouts. The highest rated players are selected for the first team, then the second team, etc. In the CSDHL and NIHL leagues, player positions and ice times will vary and are at the discretion of the coaching staff. When registering, players can select CSDHL or NIHL, but team placement will be determined during the tryouts. Fees will be adjusted accordingly after tryouts. B-Level (Bantam-Squirt) - NWHL Players will participate in an evaluation during which the hockey directors and head coaches will evaluate players based on the following criteria: skating, puck control, physical ability, shot strength, play making ability, work ethic and coachability. Coaches may weigh these differently based on their coaching philosophy. After the last session of evaluations, a player draft is held and players are placed on a team. Players are assigned a ranking, and balanced teams are formed by placing the highest ranking players on team one (for example), the next highest ranking player in team two, the next highest ranking player on team one (if there are only two teams). In NWHL league, players are given balanced and equal ice time. Players registered for B- level travel do not need to attend to A-level tryouts unless they are interested in playing on a CSDHL/NIHL Team. West Dundee Hockey Club LEAFS Fall 2016/17 3

Mite Programs We are excited to announce a new Mite Program for the 2016/2017 season. In light of NIHL's decision not to continue offering games at the mite level this fall, West Dundee Hockey Club LEAFS and West Dundee Junior Leafs will be offering a hybrid program that will insure that our players are getting the maximum benefit out of their LEAFS Hockey experience. This program will offer a combination of team based skills practices, USA Hockey/ADM style skill sessions and cross ice AND full ice mite nights designed for the ultimate age specific skill development. Players will be participating in practices and skills sessions approximately 2-3 times each week. All tournaments and full Ice games will be offered at an additional cost through West Dundee Junior Leafs (formerly West Dundee Dynamites). Junior Leafs is part of the Chicago United Hockey League (CUHL), an AAU league specifically for the mite level. We will start the season with evaluation skates where coaches will rate all skaters on their skating ability and overall hockey skills. These player ratings will be used for coaches to select three to four teams. The teams will be selected top down based on like-skilled players. The top players will be selected first for our Mite Team 1, followed by Mite Team 2, and Mite Team 3, etc. We are committed to developing young skaters through exposure to the quality and years of experience that the LEAFS Coaches offer while also providing them with the full ice hockey experience we know our members want. All game uniforms are to be purchased through LEAFS, mite players will be provided an additional patch to be added to their jersey as part of the Junior Leafs registration. This registration for mites covers all practices and skills sessions. There will be an additional registration that covers all game and league fees that will open in July. To participate in mite games this fall players will be required to register for both LEAFS and Junior Leafs. The LEAFS Travel Prep U6 program is specifically designed for young players who love to skate and want to play competitive hockey but aren t quite ready for travel hockey yet. It is a comprehensive skill development program combining the best traditional principles with new, innovative training techniques. There are no tryouts for LEAFS Travel Prep. The program is tailored to new players who are ready to learn hockey concepts and begin preparing for full-ice, competitive travel hockey. It provides intensive and repetitive skating and stick handling drills focused on the fundamental skills that all coaches look for. The program helps refine and strengthen skills and decision making in a small play area and offers competitive 3 on 3 games against other cross ice teams. LEAFS Travel Prep is fun and fast paced and intended to build a strong foundation of skills and discipline necessary for competitive travel hockey! LEAFS Travel Prep players will receive jerseys after the start of the season. Girls ONLY Teams Girls hockey rules! West Dundee Hockey Club LEAFS is dedicated to the advancement of our Girls Only hockey program. This year we are offering a U14 Girls Only program. This program is designed for female hockey players looking for a higher level of competition and is available to players from birth years 2002, 2003, 2004 and 2005. The team will play in NIHL Girls Only U14 division. There will be two payment options: Pay in Full: $300 due at registration. Remaining balance deducted on 9/15. (There is no additional fees for this option.) Payment Plan. $300 due at registration. Remaining balance will be taken out in 4 payments (Sept, Oct, Nov and Dec). A $125 fee will be applied to this plan. If we are unable to field a Girls Only team (minimum 13 skaters and a goalie), a full refund will be issued. Financial Overview West Dundee Hockey Club LEAFS recognizes that the registration and payment process can be confusing. We will do our best in this packet to clearly outline the financial details of registering your player for Fall Hockey. The financial obligation includes hockey fees, your Parent Participation contribution and the purchase of raffle tickets. Parent Participation and Raffle fees are refundable. Payment plans are available. DON T FORGET! Register before July 15 to SAVE $200! (excludes Midgets, Mites and Junior Leafs) Details on fee reduction opportunities on Page 8! All Squirt, Peewee and Bantam players will register and pay the NIHL rate and will attend all tryout sessions. Players that make a CSDHL team will be charged an additional fee based on the position/age level. This fee will be due before the first team practice and can be paid by credit card or check. Players returning for NW evaluations will receive a refund for the difference between NIHL and NWHL on or around October 1, 2016. All Mite players will register and pay the Mite rate and will attend all tryout sessions. All Travel Prep players will register and pay the Travel Prep rate. There are no tryouts for Travel Prep. All Midget players will register and pay for the CSDHL rate and will attend all tryout sessions. PLEASE NOTE: These amounts include the $300 non-refundable registration fee. They DO NOT reflect the Raffle or PPH amount. The non-refundable registration fee, Raffle and PPH fees are collected at registration for all levels except Midget ($300), Mite ($499) and Travel Prep ($300). Travel Prep players are exempt from raffle and PPH fees. Mite players are exempt from raffle fees. West Dundee Hockey Club LEAFS Fall 2016/17 6

Cash Prize Raffle Fee Reduction Opportunities Each player will receive 30 raffle tickets and have the option of recouping $300 of their fees by: (1) selling their raffle tickets at $10 each and keeping the proceeds; or (2) entering tickets into the raffle on their own behalf, and effectively buying the tickets themselves. Families with multiple Club players will be given 20 tickets ($200) for the second player; and 10 tickets ($100) for the third player. The forth and subsequent players will not receive raffle tickets. These raffle tickets are purchased by player at time of registration and NOT included in the program fee. Raffle winners are pulled at the November Public Board Meeting. Note: Mites and Travel Prep do not participate in Raffle. 1st Prize: $2,750 2nd Prize: $2,500 3rd Prize: $2,000 4th Prize: $1,000 Multiple Player Discounts A multiple player discount is available for those families that have three or more players in the Club. The oldest two players will pay the standard amount. The third and subsequent players will receive 10% off their fees. 5th Prize: $500 (5 winners) Contact registrar@leafshockeyclub.com for a special discount code to redeem your discount. Goalie Discounts The Club continues to encourage skaters to try their hand at becoming a goalie. To offset the additional equipment expense goalies incur, goalies are offered a fee reduction of approximately 25% percent. Goalies are entitled to the reduced payments ONLY IF they play goalie full-time AND own their own equipment. Parent Participation Program The Parent Participation Program is an essential component in the successful operation of the Club. We are a volunteer-run organization that relies heavily on the support of it s members! Each family will a pay a $300 refundable deposit towards the Parent Participation Program during Registration. They will then be asked to donate a combined total of 15 volunteer hours in one or more of the following capacities fundraising, coaching/team management, assistance with tryouts and registration, tournament activities, and/or various other committees. The 15 hours may be split up among different individuals representing one family. The $300 deposit is fully refundable provided the entire 15 hour time commitment is fulfilled. There will be no refunds given for partial hours served. Families may opt-out of the program but must acknowledge they understand they are forfeiting their $300 deposit. Volunteer opportunities will extend from June 1, 2016 - Spring 2017 tryouts. Refund checks will be cut December 15, 2016 and April 15, 2017. Travel Prep does not participate in Parent Participation Program. West Dundee Hockey Club LEAFS Fall 2016/17 7

Payment Details PAY IN FULL ONLINE There is no charge for this option. This simplest payment option and reduces the payment transaction fees the member and club pays to process the registration. Travel Prep, Mite, Squirt, Peewee, Bantam, Midget: Submit total player fees online today upon conclusion of registration session via Visa, MasterCard, Discover account or checking account via ACH. Girls Only: Submit $300 fee online upon conclusion of registration session via Visa, MasterCard, Discover account or checking account via ACH. Remaining payment in full will be automatically deducted using the same payment method from registration. There is no finance fee for this option. PAYMENT PLAN ONLINE This is the preferred option for members that would like to distribute their fees across the season. You will be charged an additional $125 per player ($75 for Travel Prep) to pay using a payment plan. Travel Prep, Mite-Bantam: Fees are divided into 7 installments with the first installment of $1000 ($499 MITES, $300 Travel Prep) to be collected online upon conclusion of registration via Visa, MasterCard, Discover account or checking account via ACH. Remaining balance will be automatically debited from the SAME ACCOUNT on 7/15, 8/15, 9/15, 10/15, 11/15, and 12/15. You will be charged an additional $125 to pay using a payment plan ($75 for Travel Prep). Midget: Fees are divided into 7 installments with the first installment of $300 to be collected online upon conclusion of registration via Visa, MasterCard, Discover account or checking account via ACH. Remaining balance will be automatically debited from the SAME ACCOUNT on 7/15, 8/15, 9/15, 10/15, 11/15, and 12/15. You will be charged an additional $125 to pay using a payment plan. Midget (Deferred) and Girls Only: Fees are divided into 5 installments with the first installment of $300 to be collected online upon conclusion of registration via Visa, MasterCard, Discover account or checking account via ACH. Remaining balance will be deferred until after tryouts are complete and will automatically start being debited from the SAME ACCOUNT on 9/15, 10/15, 11/15, and 12/15. You will be charged an additional $125 to pay using a payment plan. If a player does not make a team, payments will be suspended after tryouts. If you wish to use a different account to pay the balance, please log in to your Sport Ngin account BEFORE the first day of payment to update your account information. Financial Hardship funds are available to certain members whose financial situation may prevent them from participating in LEAFS Hockey Club programs. The aim of the Financial Hardship Program is to provide financial assistance to families who are undergoing a hardship and/or to ease the financial burden for deserving families. Financial Hardship funds are awarded on a case-by-case basis and members who would like to be considered must submit the necessary supporting documents and a completed Financial Hardship Application. Not everyone who applies will be granted hardship funds. Hardship assistance is not based on talent. Players receiving assistance must also demonstrate dedication through consistent attendance at team practices, games and positive sportsmanship. Financial Hardship assistance is typically granted prior to Fall tryouts. Applications are available online and should be submitted by July 15th, 2016. Late Payment/NSF Fees: Payments that are seven (7) days late may result in player being removed from ice (team practice, skills session, game or tournament). A $25 Reinstatement Fee will be assessed to restore player s ice privileges. All fees must be current (including late fees) in order for player to return to practices, skills, games or tournament. It is the responsibility of the person registering to ensure credit card information is valid there is no administrative fee to change credit card information and can be done by the member online. If a credit card is declined or stopped by the member for any reason, a $25 DCC Fee will be assessed. Returned Checks will result in a $25 NSF Fee. West Dundee Hockey Club LEAFS Fall 2016/17 8

Financial Commitment All refund requests must be submitted in writing to the Registrar at registrar@leafshockeyclub.com. Requests received prior to 9/1 will receive a refund of amounts paid, less a $50 cancellation fee. Requests received after tryouts have begun on 9/1, and prior to the team draft, will receive a refund amounts paid less the $300 non-refundable registration fee and a $50 cancellation fee. Teams may be drafted as early as the first night of tryouts. No refunds will be given after the draft unless there is a special circumstance as determined by the Executive Committee of the Board of Directors. If The Club is unable to field a team at your level we will process a complete refund of payments received. Members requesting a refund after teams have been posted are responsible for the full fees per the Financial Obligation Agreement signed during registration. The season balance is due immediately upon notification that the player would like to cancel their registration. Players are not permitted to change his/her team or association until they have satisfied the outstanding balance of their registration. Failure to pay will result in submission of the player to AHAI Rules and Ethics. AHAI can impose suspensions until the financial obligation is satisfied. Partial refunds for situations involving a season ending injury can be granted by the board after considering such factors as the date of injury and the costs incurred by the club on behalf of the player. To be eligible for an injury credit, the player must not be unable to participate in team activities. Injury refund requests should include documentation to support the injury claim that states that the player is unable to participate in hockey. The team manager is responsible for signing the form confirming the amount of time the player was injured. Injury refunds will be reviewed by the Hockey Director and the Executive Committee. All Squirts, Peewees and Bantams will pay the NIHL rate at registration. Players who make a CSDHL team will be required to pay an additional registration fee, as outlined below, before their first practice. Players that are selected for a NWHL team will receive a refund of the NIHL fee difference prior to October 1, 2016 based on the fee schedule found on page 6. All Midgets will pay the CSDHL travel hockey rate. Mites will pay the listed rate without any up-charges or refunds. Uniform Standards and Ordering Information In an effort to present a standard, consistent appearance across the club, all West Dundee Hockey Club LEAFS, CSDHL, CUHL, NIHL and NWHL players are required to wear all components of the official LEAFS uniform. This uniform consists of the LEAFS jersey, hockey socks, pant shell and royal blue helmets and gloves. Royal blue gloves are not typically available in sizes under 10. Players who need gloves smaller than 10 will be allowed to wear black. Helmet stickers (both the logo and player number) are a component of the official uniform. Helmets should be clear of all other stickers. Helmet stickers will be sold during Sprit Wear sales throughout the season. Please place your uniform orders asap to ensure timely delivery for the Fall season. Jerseys are custom made in the USA and take months from start to finish. Please have all orders placed by 7/1 for September delivery. All jersey orders placed after 7/1 are subject to being placed in the second round of ordering with expected deliveries in late November. Loaner jerseys can be issued for early fall games. If you are new to the LEAFS you do not need to wait to order jerseys. Numbers will be assigned and placed on the items you have ordered. This includes game jerseys and warm ups. It is the policy of the club that we are unable to consider specific number requests or requests to change numbers. Numbers will be assigned based on birth year and availability. The number assigned will be the player s permanent number.
