73rd Annual Journal Star Summer Championship Swim Meet July 16th, 2016 LOCATION: SCHEDULE: Central Park Pool 415 W. Richmond Peoria, IL 61604 Morning Session 10 & under: Warm-up: 7:00-7:45 a.m. Meet starts: 8 a.m. Afternoon Session 11 graduating senior in high school: Warm-up: 1:00-1: 45 p.m. Meet starts: 2:00 p.m. INSURANCE: Certificate of Insurance is required for each team attending the meet. The attached form can be used for individuals not attending with an insured club, or for clubs not able to produce a Certificate of Insurance. Every swimmer participating in the meet must be covered by their teams Certificate of Insurance, or by an Individual Liability Release form. RULES: USA Swimming and Illinois Swimming 2016 rules will be followed. All Illinois Swimming warm-up procedures will be strictly enforced. Electronic timing will be used as the official time. The back-up buttons and hand-held watches are used only in the case of a system failure. Swimmers must hit the touch pad to properly finish the race. 8 and under swimmers will finish in the shallow end. Forward starts will be from starting blocks placed at the deep end of the competition pool. WARM-UPS: Lanes will be assigned to teams and unattached individual swimmers. No diving is allowed during warm-ups until announced. COACHES MEETING: There will be a coaches meeting 15 minutes prior to the start of each session if needed. Each team should be represented. ENTRY LIMITS: A swimmer may enter three (3) individual events and two (2) relays per day. A swimmer must be entered in one (1) individual event to be eligible for relay events. A swimmers age is determined by the age the swimmer is on July 16, 2016 ENTRY FEES: $15.00 per swimmer per day. There are no fees for relays. All entry fees must accompany the official entry form. No substitutions or changes of events may be made after that date. Coaches are requested to turn in relay swimmer names to the clerk of course 1/2 hour before the start of the meet. SCORING: Individual events: 9-7-6-5-4-3-2-1 Relay events: 18-14-12-10-8-6-4-2 AWARDS: Individual events: Award Medals will be given to 1st -8th place swimmers and Award ribbons will be given to 9th -16th place swimmers in the following age categories: 6& Under, 7 year olds, 8 year olds, 9 year olds, 10 year olds, 11 year olds, 12 year olds, 13-14 year olds, and 15-HS age swimmers. Relay events: Award Medals will be given to 1 st -3 rd place. Team Awards: The team awards are designed for summer swim clubs in Central Illinois. USA Swimming Teams will be exempt from team awards. Team trophies will be given to 1 st -3 rd place teams. CONSESSIONS: Full concessions will be available throughout the meet, as well as the Maui Wowi vendor for smoothies... bring your cup for a refill from last year! I consent to my minor s participation in the activity and acknowledge that the minor and I fully understand my/minor s participation may involve risk of serious injury or death, including losses which may result not only from my/minor s own actions, inactions or negligence, but also from the actions, inactions, or negligence of others, the condition of the facilities, equipment, or areas where the event or activity is being conducted, and/or the rules of play of this type of event or activity. I understand that if I have any risk concerns, I should discuss the risks associated with my participation with the activity coordinators and event staff, before I sign this document and before the activity begins. Release Minor s Rights: In consideration of allowing Minor Participant to participate in this event, I hereby release and hold harmless The Peoria Area Water Wizards Swim Club, USA Swimming, its local swimming committees and their members of its board of directors, officers, employees, volunteers, other participants, and agents (collectively, the Released Parties ), of and from, and do discharge and waive, any and all claims, demands, losses, damages, and liabilities that Minor Participant may have or sustain with respect to any and all damage and/or injury, of any type, arising out of his or her participating in this USA Swimming event. I also agree that if any portion of this agreement is held to be invalid the balance, notwithstanding, shall continue in full force and effect. The undersigned also agrees that this Release and Waiver of Liability, Assumption of Risk and Indemnity Agreement extends to all acts of negligence by the Releasee and is intended to be as broad and inclusive as is permitted by the laws of the State in which the Event(s) is/are conducted and that if any portion thereof is held invalid, it is agreed that the balance shall, notwithstanding, continue in full legal force and effect.
