SAFETY INSTRUCTIONS 1. This fall protection apparatus compliant with Z359.14 Class B: 1996 is tested and approved for the securing of maximum 2 persons against falling. In conjunction with additionally required DIN EN 361 compliant safety harnesses, DIN EN 360 compliant ULTRA-SAFE fall arresters and Z359.14 Class B compliant Ultra-Safe fall arresters of type HRA with rescue lifting gear, it serves for the safety of persons performing work that involves a risk of falling. The fall protection apparatus and fall arresters must only be used as intended. Notice: The ULTRA-SAFE HRA fall arrester with rescue lifting gear does not come included with the hoist! 2. The accompanying instructions for use and installation of this apparatus must be strictly followed. 3. Failure to observe the instructions for use can present a danger to life. In the event of a fall, hanging in the harness for any longer than 20 minutes must be avoided (risk of suspension trauma orthostatic shock). A rescue plan must exist that accounts for all possible emergencies that can arise during work. 4. The apparatus may only be operated by appropriately trained and skilled persons. No users of this equipment may suffer from any kind of health impairment! (e.g. alcohol, drug, medication or circulation problems) 5. An optional ULTRA-SAFE HRA fall arrester with rescue lifting gear can only protect one person while in use, but can be used alternately by several persons in succession. 6. The hoist is fastened by inserting it into the floor flange. 7. Before each use, the legibility of all product labels must be verified. When using the optional ULTRA-SAFE HRA fall arrester, a functional check must be performed either by tugging sharply on the cable or by applying a test weight of at least 15 kg. The safety catches must fall into place when doing this. The karabiner hook must be checked for proper function. 8. The safety protection of the worker is established once the hoist has been correctly installed without error, the optional ULTRA-SAFE HRA fall arrester has been fastened into the mount on the hoist, and the end of the extensible lanyard (with karabiner hook) has been attached to the safety harness worn by the worker. 9. The apparatus should be arranged as close to perpendicular over the head of the person to be secured in order to prevent swinging in the event of a fall. 10. ULTRA-SAFE fall arresters may not be used to secure persons to rest on dry bulk goods or similar materials that a person can sink into. It must be ensured that the type of work limits free falling to a minimum. Slackening of the cable must be avoided. In the event of a fall, impact with the ground or any other obstacles must be impossible. 11. Never use an apparatus that is damaged or has been strained by a fall, or if there is any doubt as to the safe condition of the apparatus. Only after inspection by a competent person and with written clearance may such an apparatus be used again. 12 The fall protection apparatus must be inspected by the manufacturer or by a person trained and authorized by the manufacturer depending on the operational demands, or at least every 12 months. This must be documented in the accompanying inspection logbooks.the effectiveness and service life of the fall protection apparatus depends on these regular inspections. 13. If the cable breaks or becomes kinked or roughened, the optional ULTRA-SAFE fall arrester must be sent in to the revision workshop for the cable to be replaced 14. The optional ULTRA-SAFE fall arrester is intended for use at temperatures between - 30 and +50 C, in accordance with DIN EN 360. 15. The permissible load on the fall protection apparatus corresponds to DIN EN 795 for securing 2 persons. 16. The hoist and the optional ULTRA-SAFE fall arrester must be protected against exposure to welding flames and sparks, fire, acids, bases, extreme temperatures and similar environmental influences. After successful use and any dismantling of the fall protection apparatus and optional equipment, these must be stored in a clean, dry and well ventilated place until next use. The equipment must be protected against impact, shocks and blows during transport. 17. No modifications or additions to the complete system may be made. 2

POSSIBLE VARIATIONS IN USE FOR SECURING 2 PERSONS AGAINST FALLING AND FOR RESCUE IN CONJUNCTION WITH THE ADDITIONAL ATTACHMENT POINT (3) Figure 16 Figure 17 Fig. 16 & 17: Say a person climbs into a manhole, secured to the HRA fall arrester, attached to the eyelet on his safety harness (instructions for use of equipment and gear must be observed). The backup person has the possibility to secure himself onto the additional attachment point, for example using a fall arrester in conjunction with a safety harness. Even during a rescue operation in an emergency, the backup person must be secured against falling at all times. It is not possible to rule out injuries to the backup person or damage to the PPE in the event of a fall, in a manhole for example, due to impact against edges or objects. THE HPMH MANUAL WINCH IS MOUNTED AS FOLLOWS: Pull out the lock pin (2) and push the winch with its mount preinstalled onto the rear part of the upper arm (see Fig. 18). Then secure the winch (Fig. 19) with the lock pin (2). Roll the cable off the drum by turning the handle. Open the top cable cover (1) as described above, draw the cable with karabiner through and close again. See Fig. 11 and Fig. 12. OVERVIEW OF WALL AND FLOOR SLEEVE Plastic Socket THE US-HMH100 MANUAL WINCH IS MOUNTED AS FOLLOWS: Wall Flange Identification Plate Base Plate Fastening Holes Floor Flange When mounted onto concrete surfaces, floor/wall sleeve must be fastened using the supplied M16 chemical anchors. Any other type of screw or fastening mechanism may not be used and is not permitted. It must be proven that the floor onto which the hoist will be mounted can withstand the anticipated stress from the hoist in the event of a fall (2 x dynamic 1,350 lbs in unfavourable load position with consequent static load of the weight of the 2 persons). The chemical anchor must be able to withstand a tensile force of 7,425 lbs or 2,000 psi. 4.3 110 0.79 20 5.6 142 The mounting instructions given below and the instructions for the chemical anchor test must be strictly followed! 0.4 10 11.8 300 11.8 300 9.5 240 7.8 200 10.6 270 9.5 240 11.8 300 0.79 20 0.4 10 MOUNTING EQUIPMENT / CHEMICAL ANCHORS 2.9 75 The mounting equipment comprises: 0.16 4 5.75 146 9.5 240 11.8 300 4 anchor bolts: 5/8 x 5.3 length 4 washers 4 nuts 4 cement cartridges 16.1 410 d 0 M 5.0 129 h ef = h 1 h t fix 6

Otherwise, store the cartridges in a cool place and protect them against prolonged exposure to sunlight. FASTENING INTO CONCRETE The anchor manufacturer's specifications of the edge distance, drilled hole diameter, drilled hole depth and curing times must be observed. Procedure: 1. Drill four 11/16 right-angled (Fig. 27) holes to a depth of 4 7/8 using a hammer drill. 2. Brush out all drilled holes to free them from dust (Fig. 28) 3. Into each drilled hole (Fig. 29), insert 1 hand-hot cement cartridge. The resin inside the cartridge must have a honey-like fluidity. 4. Fasten the anchor shaft (Fig. 30) into the drill chuck and drill it into the hole at a slow speed. Continue inserting the anchor until cement can be seen escaping. The anchor shafts have a knurled insertion marking (Fig. 31) that should remain level with the concrete surface. 5. Do not move the anchor shaft any more; see the curing times given in Fig. 34. 6. Tighten the screw connection to max. 59 ft. lbs. (Fig. 33) NOTICE: The proper installation must be inspected by an accredited person before it is put into use, in accordance with Betriebssicherheitsverordnung 10 [German Ordinance on Industrial Safety and Health].
