West Irondequoit Central School District 2018-2019 Swain Ski & Snowboard Club SUNDAYs AT SWAIN Dates: January 6 th, 13 st, 27 th February 3 th We are offering four Sunday ski trips in January and February. We will leave Irondequoit High School at 9:00 am and return to school at 5:15 pm on the abovementioned dates. Your trip includes a motor-coach bus ride to Swain Mountain and back to school, an all-day lift ticket and lesson. Rental of skis and snowboards are available. We will take the first 50 students to sign up and pay. Lift And Bus Ride Only $ 215 Lift, Rental and Bus Ride- $264 THINGS YOU NEED TO DO; 1. FILL OUT CO-CURICULAR/REGISTRATION FORM 2. FILL OUT SKI PATROL MEDICAL FORM 3. FILL OUT RENTAL FORM IF YOU NEED RENTALS 4. WRITE A CHECK TO IHS SKI CLUB 5. TURN IN FORM AND CHECK ASAP 6. WEST IRONDEQUOIT SCHOOL SKI AND SNOWBOARD CLUB RULES ON BUS AND AT THE MOUNTAIN: 1. All Rules, including the Co-Curricular Policy and Student code of conduct, apply from the time that students load the bus until they are picked up at the end of the day. 2. No one can transfer their membership to another student without the permission of their advisor. 3. In order to arrive back at Irondequoit on time, ski club members must be on the bus and in their seats no later than 4:00 pm. If a student arrives later than 4:00 twice they will not be allowed on the following trip. 4. STUDENTS BOARDING THE BUS AFTER 4:00 PM TO RETURN TO SCHOOL WILL BE PENALIZED WITH A SUSPENSION FROM THE NEXT TRIP. 5. Ski boots and bags MUST go underneath the bus. 6. NO LOUD SINGING OR SHOUTING IS ALLOWED ON THE BUS. 7. Students who ride down on the bus must return on the bus. 8. The use of tobacco products or any drugs, including alcohol, is forbidden. As a school function, we reserve the right to inspect student bags. *Parents will be called* 9. No Foul Language 10. No flash photography, lasers, or flashlights are permitted on the bus. 11. All trash must be picked up from the seats and floors at the end of the evening. Any cases in which the bus is left a mess, regardless of whether it is your seat or not, will result in a suspension of food/beverage privileges. NOTE: IN CASES WHERE BUS RIGHTS ARE SUSPENDED OR STUDENTS ARE EXPELLED FROM THE CLUB FOR EGREGIOUS CODE OF CONDUCT VIOLATIONS NO REFUNDS WILL BE GIVEN

Yes No Date of last tetanus shot Blood Type Additional emergency contact: Name/Number Terms & Conditions WARNING TO SKIERS: Downhill skiing and snowsports, like many other sports, contains inherent risks including, but not limited to the risk of personal injury, including catastrophic injury, or death, or property damage, which may be caused by variations in terrain or weather conditions; or, surface or subsurface snow, ice, bare spots or areas of thin cover, moguls, ruts, bumps, or other persons using the facilities; or rocks, forest growth, debris, branches, trees, roots, stumps; or other natural objects or manmade objects that are incidental to the provision or maintenance of a ski facility in New York State. New York law imposes a duty on you to become apprised of and understand the risks inherent in the sport of skiing, which are set forth above, so that you make an informed decision of whether to participate in skiing notwithstanding the risks. New York also imposes additional duties upon you, to which you must adhere, for the purpose of avoiding injury caused by any of the risks inherent in skiing. If you are not willing to assume all of these risks and abide by these duties, you must not participate in skiing at this area. Swain has the right under Article 18 of the New York State Obligations Law to remove from the ski area and/or deny skiing privileges to anyone whose conduct, excesses or attitudes, as determined by Management or Patrol create a nuisance for themselves or others. In such cases, no refunds shall be issued. YOUR RESPONSIBILITY CODE: Always stay in control, and be able to stop or avoid other people or objects. People ahead of you have the right of way. It s your responsibility to avoid them. You must not stop where you obstruct a trail, or are not visible from above. Whenever starting downhill or merging into a trail, look uphill and yield to others. Always use devices to prevent runaway equipment. Observe all posted signs and warnings. Keep off closed trail and out of closed area. LOOK BEFORE YOU LEAP. EASY STYLE IT. RESPECT GETS RESPECT. BE AWARE...SKI WITH CARE. REFUND POLICY: Student coupon booklets are not refundable if lost or stolen. In the case of injury that occurs on or before January 31, 2019, the injured student s leftover lift ticket coupons may be forwarded to the 2019-20 ski season. A signed letter from the acting physician stating the student s inability to participate in snowsports must be presented to Swain in order for the coupon transfer to occur. For injuries that occur on or after February 1, 2019, that cause the student the inability to use the lift ticket coupons, no coupon transfers will be issued. Rental Form 2018-19 Helmet Policy: Anyone under the age of 18 who is renting a snowboard is REQUIRED to wear a helmet. Type II: Ski or Snowboard Moderately, prefer a variety of speeds, prefer to ski on varied terrain, not classified as either Type I or Type III, favor average release/retention settings appropriate for recreational skiing. Type III: Ski or Board Aggressively, normally ski at faster speeds, prefer fast and aggressive skiing on slopes of moderate to steep pitch, prefer higher than average release/retention settings, prefer decreased releasability in a fall in order to gain a decreased risk of inadvertent binding release. RENTAL & LIABILITY RELEASE AGREEMENT: PLEASE READ CAREFULLY BEFORE SIGNING I accept for use as is the equipment listed on this form, and accept full responsibility for the care of the equipment while it is in my possession. I will be responsible for the replacement at full retail value of any equipment rented under this form, but not returned to Denton Hill Family and Ski Resort, Inc, Swain Recreation LLC. I agree to return all rental equipment by the agreed date in clean condition to avoid any additional charges. I have received instruction on the use of my equipment and fully understand its use and function. I have made no misrepresentations to (DHFASRINC, SRLLC) concerning my height, weight, age or skier type, I verify that the visual indicator settings recorded on this form agree with the numbers appearing in the visual indicator windows of the equipment listed on this form. I agree to hold harmless and indemnify (DHFASRINC.SRLLC) and its owners, agents and employees, as well as the equipment manufacturers and distributors for any loss or damage, including any that result from claims for personal injury, death or properly damage related to the use of this equipment. I understand that there are inherent and other risks involved in the sport of snow skiing, for which this equipment is to be used, that injuries are a common and ordinary occurrence of the sport, and I freely and voluntarily assume those risks. I understand that a ski-binding-boot system will not release at all times or under all circumstances where release may prevent injury or death, nor is it possible to predict every situation in which it will release, and it is, therefore, no guarantee of my safety, I hereby release from any legal liability (DHFASRINC, SRLLC) and its owners, agents and employees, as well as the manufacturers and distributors of this equipment from any and all liability for damage and injury or death to myself or to any person or property resulting from the selection, installation, maintenance, adjustment or use of this equipment and for any claim based upon negligence, breach of warranty, contract or other legal theory, accepting myself the full responsibility for any and all such damage, injury or death which may result. This agreement is governed by the applicable law of this state or province.
