Release of Liability READ CAREFULLY THIS AFFECTS YOUR LEGAL RIGHTS I on behalf of myself and my child, have voluntarily requested to participate in the Youth Sailing Camp offered by Half Moon Bay Yacht Club (HMBYC) at 214 Princeton Ave Half Moon Bay, California, 94019. In exchange for participation in the Youth Sailing Camp, I agree for myself and for the members of my family, to the following: 1. To observe and obey all posted rules and warnings and oral instructions given by HMBYC, or the employees, representatives or agents of HMBYC. 2. To recognise that there are certain inherent risks associated with the above described activity and I assume full responsibility for personal injury to myself and my family members, and further release and discharge HMBYC for injury, loss or damage arising out of me or my family s use of or presence upon the facilities of HMBYC, whether caused by the fault of myself, my family, HMBYC or other third parties. 3. To indemnify and defend HMBYC against all claims, causes of action, damages, judgements, costs or expenses, including attorney fees and other litigation costs, which may in any way arise from me or my family s use of, or presence upon the facilities of HMBYC. 4. To pay for all damages to the facilities of HMBYC caused by me or my family s negligent, reckless, or willful actions. 5. Any legal or equitable claim that may arise from participation in the above shall be resolved under California Law. 6. I also understand that there is a minimum class size of 5. In the event that this number is not reached two weeks prior to the start of camp, there is a possibility that this camp date advertised will be cancelled. I will be offered an alternative camp week or a full refund. ( Note: the spring break camp is fully enrolled and this paragraph does not apply.) 7. I understand that Half Moon Bay Yacht Club may take photographs and videos of the children at camp for the purposes of promoting sailing at the club. (Note: campers will never be identified by full name in any such photography/videography.) I HAVE READ THIS DOCUMENT AND UNDERSTAND IT. I FURTHER UNDERSTAND THAT BY SIGNING THIS RELEASE, I VOLUNTARILY SURRENDER CERTAIN LEGAL RIGHTS. Dated: Parent Signature: Camper Signature: Page 3/4

Youth Sailing Rules 1. Sailors must wear their PFD s at all times while on the water, dock and boats. 2. Willingness to learn and participate is mandatory. Anyone who refuses to participate will be asked to leave the class and their parents will be called to take them home for the remainder of the day. In the case of a repeated offense or at the discretion of the Head Instructor on the first offense, the sailor will be asked not to return and no refund will be given. 3. No swimming off the dock during program hours without permission and/or supervision. 4. The use of skateboards, bikes, roller blades, scooters and other recreational modes of transportation are not allowed on HMBYC grounds. 5. Sailors are not allowed on the ferry or on the docks without PFD and permission of the instructor. 6. Sportsman like behaviour is to be demonstrated by all sailors at all times in keeping with the Corinthian spirit. 7. Do not use, borrow or take equipment that is not yours unless you have received specific permission from the owner. 8. Water guns, water balloons, and other non sailing toys are not permitted during program hours unless by permission of the Head Instructor. 9. Appropriate sailing attire must be worn during program hours. 10. Sailors are responsible for putting away all gear at the end of each day. 11. Sailors are responsible for providing their own lunches during camp. 12. Sailors are not permitted to leave class until a parent or guardian has checked them out by speaking with an instructor. 13. Hitting, fighting and/or bad language wil not be tolerated. Anyone who exhibits this behaviour will be asked to leave the class and their parents will be called to take them home for the remainder of the day. In the case of a repeated offense or at the discretion of the Head Instructor on the first offense, the sailor will be asked not to return and no refund will be given. 14. Abuse of boats or failure to follow safety instructions will lead to loss of sailing privileges, expulsion from the class and/or potential financial responsibility. 15. Alcohol, tobacco and drugs are prohibited on club grounds. Violation of this rule is grounds for immediate expulsion from the program without refund. I HAVE READ AND UNDERSTAND THE SAILING CAMP RULES. Camper Signature: Parent Signature: Date: Page 4/4

Parent & Sailor Responsibilities Take the swim test All campers will be expected to take a swim test on the first day of camp. This will involved a 20 yard swim with and without a life vest and treading water. Prepare your sailor Ensure that your sailor has the appropriate attire for sailing. Think about layers like fleece and wool for cold days and quick drying fabrics or rash guards when the sun is out. Please no cotton for on the water activities! Your child s enjoyment will primarily depend on their comfort. Ensure that they will be warm, protected from the sun and well fed! BIG bag to put everything in Sack lunch and Snack Snugly fitting wetsuit Sailing gloves (Highly recommended! Thermal rubberized gardening gloves work great and are a less expensive solution.) Neoprene booties or comfortable closed toed shoes Sun hat and warm hat Sunglasses Sun screen Weather proof jacket Change of clothes and towel Water bottle Recommended Sailing Suppliers: Wetsuit Sales and Rentals: Sailing Gloves and Apparel: Cowboy Surf Shop West Marine 2830 Cabrillo Highway 1 1119 Industrial Road Half Moon Bay San Carlos (650) 726 6968 (650) 593 2070 Please arrive on time: To maximize instructional time, please enable your child to arrive with enough time to be ready to sail by the beginning of the class period. We suggest arriving at the club no later than 15 minutes prior to the beginning of class. Rigging and chalk talk will begin promptly at 8:30 AM. Especially the youngest in our classes need assistance and time to get dressed properly. Please arrive early if this is the case. Parent Involvement: If you would like to help volunteer with the smooth running of the program please let us know beforehand so that we can plan how you can help us best! The programs, not to mention the club, could not run without the dedication of volunteers. We are always looking for a Dock Parent who can remain on shore and be available should a child become cold, wet, tired and wish to return to shore. This will enable the instructor to continue with the class. No prior sailing experience is required just a love of working with kids and a positive attitude! Maybe you will learn something about sailing along the way and want to become a member at HMBYC! Parents and Grandparents who would just wish to observe the camp are asked to do so from the club deck. We ask that you do not interrupt the class once it has begun as this will impact the instructor s flow of thought and could compromise the learning and safety of all students. If there is anything that you are worried about and would like to share with the instructor, regarding instructional time or techniques please inform the instructor before the class begins. After class: It is imperative that you pick your child up, or make arrangements to do so immediately after class is over at 3:30 PM. If you are not personally able to pick your child up and wish to carpool with another student or send someone else, we will require written authorization to release your child to anyone other than the parent or guardian. The instructor will not be able to supervise your children more than ½ hour after class unless advance arrangements have been made, by paying the instructor s salary for the time. Questions?
