Annual Registration & Waiver Form In an effort to streamline the registration process, an annual registration and waiver form has been developed. This is a separate form from the seasonal League Registration and Payment forms. This new form has been included as an insert in this brochure. If necessary, additional forms can be obtained three ways: 1. Download the form online at. 2. Email Danielle Marino, recreational coordinator, at dmarino@udsakron.org. 3. Call Danielle Marino at 330-762-9755, ext. 233 to request a form via mail. IMPORTANT POLICY CHANGE: To ensure the health and safety of all participants, this form must be completed and returned to UDS before participating in any UDS All-Star Training Club program. The form will be kept on file and will be valid for one year following the date of your signature. If at any time you need to update your information or if you have any questions regarding this new policy, please contact Danielle Marino, UDS recreational coordinator, by phone at 330-762-9755, ext. 233, or by email at dmarino@udsakron.org. Social Etiquette Appropriate social behavior is stressed during all programs. Our staff members and volunteers will do their best to ensure each participant s success in our programs. If a participant s behavior is detrimental to the group or self (kicking, biting, hitting, self-abusing, refusal to stay with the group, etc.), a parent or guardian will be called to pick up the participant immediately. Note: If a participant requires visual supervision or is not independent in his/her personal care, a caregiver MUST accompany the participant to each activity, as UDS All-Star Training Club staff members cannot guarantee constant visual supervision.

Payment Options For the best possible experience, please turn in all registration forms prior to the first day. Payment may be made by cash, money order, check or credit card. Checks should be made out to United Disability Services. There will be a $25 fee for any returned checks. If you are eligible for services through your local county board, your funding source (Level 1 Waiver, IO Waiver, SELF Waiver and Family Support Services) may be used to cover the cost of your participation in UDS All-Star Training Club activities. Athletes are responsible for contacting their service & support administer (SSA) to initiate the process. Please have your SSA contact Danielle Marino, recreational coordinator, by phone at 330-762-9755, or by email at dmarino@udsakron.org to verify the process has begun. Initial contact must be made prior to the beginning of the activity. If you are deemed ineligible to use your waiver funding, you must pay by cash, check, money order or credit card. Financial Assistance For any athlete, with or without a disability, who is in need of financial assistance to cover a portion or all of the cost of ATC activities, please call 330-762-9755, ext. 233, for more information about the many options available. Family Rate Policy *Change All participants who compete must register. The first participant pays the standard registration fee. Each additional athlete/family member will be charged $20. Parents, family members or staff who are there to assist only do not need to register. Note: The Family Rate does not apply to the winter Bowling League. All individuals who participate will need to pay the price indicated on the registration form. Refunds/Cancellations Refunds will be issued on a case-bycase basis and if granted, will be given as a voucher for a future activity. In the event of a cancellation, all athletes will be notified prior to the start date. We will also post updates on the UDS All-Star Training Club website. Your registration/participation indicates that you have accepted all UDS All-Star Training Club policies.

Come out and join our team! This league is for athletes who want to learn to play for the first time or athletes who are looking to improve their game. Each athlete will bowl two games per week. The complete season is fifteen weeks. There is no session December 24 due to the holiday. The last session will be our championship tournament where individual trophies will be awarded. IMPORTANT POLICY CHANGES 1. All participants must have a completed registration and waiver form on file BEFORE participating in any UDS All-Star Training Club program. See page 3 for more info. 2. To compete in the championship tournament, participants must have attended seven sessions during the regular season to bowl in the last session on February 25. Regular Season: Saturdays, November 12 - February 18 *No Session December 24 Championship Tournament & Party: Saturday, February 25** **IMPORTANT! Participants must attend seven sessions to participate in the championship tournament. Time: Check-in begins at 12:30 p.m. Lanes open at 1 p.m.** ** Please remember to arrive no later than 1 p.m. if you would like to bowl on your regularly assigned lane. If you arrive after 1:30 p.m., you will only be allowed to bowl one game. If you arrive after 2 p.m., you will not be able to bowl that session. Where: Bill White s Akron Lanes ~ 2911 E. Pay-As-You-Go Parent/Volunteer: $5 per participant each session * Please note the Family Rate Policy does not apply to Bowling. Everyone that bowls must pay the assigned fee. All athletes will learn the basics of this great game including free throws, jump shots, ball handling, and game situations. All athletes will be major contributors to their respective teams. The season will open with all athletes attending the 5:30 session of their chosen night. The first session will consist of a skills challenge. Based on the results, athletes will be assigned either the 5:30-6:30 p.m. session or 6:30-7:30 p.m. session. The season will conclude for all athletes with an Invitational and awards ceremony at St. Vincent - St. Mary High School on March 12, 2017. The complete season is nine weeks. Please indicate on your registration form if you would like to play on Sundays or Mondays. Please note there is no guarantee we will be able to accommodate everyone s first choice, however we will try to accommodate your request if possible.
