AN-SNAP Class allocation There are two steps to allocating a SNAP class for each patient

SNAP class allocation Step 1 1. The impairment code must be allocated. There are two over-riding rules that need to be considered when allocating an impairment code: a. The episode should be classified according to the primary reason for the current episode of rehabilitation care b. Rehabilitation program names related to funding are not necessarily the same as the impairment group names

Impairment Code Guidelines The impairment code should be allocated by an experienced clinician AROC recommends use of the Impairment Code Guidelines which include examples of aetiologic diagnoses that could underpin each impairment. The list is not exhaustive.

Case study for impairment code eg. a patient in a debility/reconditioning funding program may be having rehabilitation due to deconditioning related to a cardiac disorder this episode should be classified to 9.2 - Chronic cardiac insufficiency not to 16 - Reconditioning/restorative

AN-SNAP class allocation - Step 2 2. Once the impairment code has been allocated the AN-SNAP class is determined in conjunction with: FIM motor score OR FIM motor and cognition scores OR FIM motor score and age The additional information required is determined by individual impairment group NOTE: Assessment only as a treatment goal (AN-SNAP class 201 for all impairments)

AN-SNAP class allocation For Example: A patient that has had a stroke with right middle cerebral artery involvement and ischemia on the right side of the brain and has left side involvement. The patient will be allocated impairment code 1.1. The patients admission FIM motor score is 51 and FIM cognitive score is 22. The AN- SNAP class is therefore 206.)
