Please read this carefully, for some rules and fees may have changed. To ensure you receive your pool ID s before the opening day, Saturday May 26 th, the enclosed registration form should be returned for processing no later than: MONDAY MAY 21, 2018 Your pass(s) will be returned to you by the doorman in your building within a few days of submission. Please detach and review the pool rules and procedures carefully. Keep it available for reference, do not return it with the application. Only page 5 should be submitted with pictures and payment (if necessary). Please provide 1 COLOR PASSPORT SIZE PHOTO (2x2) for each applicant 5 years old and older. Write first name(s) on the back of the photo(s). VILLAGE MALL AT HILLCREST CONDOMINIUM POOL RULES AND REGULATIONS 2018 SEASON The pool rules and regulations contained in this document have been thoughtfully developed to accommodate the needs of all Village Mall residents, their guests and employees. This document is a user guide and may not contain all of the rules, regulations and protocols relating to pool operations and administration. Please read the entire document, as there have been changes made since last season. Pool hours of operation will be posted, as needed, at lobby or other locations and may be modified as operating conditions require. Residents will be notified of any changes to regularly scheduled pool operating hours. ADMISSION RULES AND PROCEDURES: All residents and their guests are expected to follow the rules and procedures indicated below a. You must reside at Village Mall to use the pool facilities. An owner, who is a non-resident, is not permitted to use the pool. b. Village Mall resident and attendant employees are required to present a current, management authorized, photo ID pool pass in order to enter the pool facility. c. Any family member of a Renter is considered to be a Guest, unless they are listed on the lease agreement, notwithstanding the length of time they may have remained in the apartment. d. Every patron must be logged in on the pool attendance sheet provided at the pool deck entrance. All requested information will be entered by the staff member in attendance, after passes are verified and guest payments, if any, are collected. e. All children five years or older must have a Village Mall photo ID. f. Residents may bring to the pool, no more than six (6) guests on Monday through Friday and no more than four (4) guests on Saturday, Sunday and Holidays. g. An adult from the sponsoring apartment must register all guests and remain with them while the guest(s) are at the pool facility. h. Pool fee s for non-owners (renters) is $125.00 per adult and $250.00 for family. (Must reside at building). i. Pool guest fees: $10.00 for each person 6 years of age and older; $5.00 for children (ages 1 to 5). No cash accepted under any circumstances. Only a Residents check or money order, with their address and apartment number indicated on the check, will be accepted for payment. Checks or money orders must be made payable to Village Mall Condominium at the time of their visit. j. Attendants: Residents who engage a non-resident in the capacity of mothers helper, baby sitter, health aid or nanny etc., may obtain a season Attendant Pass for them (from the management office for $150.00 per season) or a daily pass (at the pool for $10.00 per day). These individuals are authorized to use their pass only when engaged in the process of performing their supervisory duties. Attendants must be at least 16 years of age. Only one season pass of this type, per apartment, is permitted. A season Attendant pass, does not count as one of the regular season passes. Residents, who want to purchase a season pass for an Attendant, must fill in the appropriate information on the pool pass registration form. pg. 2

k. Each apartment is entitled to purchase a maximum of two season guest passes ($150.00 per pass). These will be issued by the management office. They are valid only for the current season. Each season pass, will allow a guest to be admitted to the pool each day, provided they are accompanied by an adult resident from the apartment they are visiting. l. Guests admitted on a single day pass basis, will have a plastic ID band placed on their wrist, designating their guest status for that day. The band must be worn for the entire day. m. Lifeguards have discretion to assure that the legal pool area occupancy limits established by the Health Department, are being enforced. They can limit the number of persons entering or remaining at the pool facility. n. No admission is permitted without payment (No cash accepted). No admission is permitted without a valid 2018 Pool ID, no exceptions. o. Any bank fees associated with a bounced check will be charged to the apartment Owner. Failure to pay bounced check fees will result in suspension of all pool passes associated with the apartment until the fees are paid. p. There is a $25.00 administrative processing fee for replacement of lost pool passes. q. Admission to the pool facility is a privilege afforded to Owners who are in good financial standing with the Condominium. Owners, who are three months in arrears or more, will not be issued passes or if issued, access to the pool facility will be suspended, until their account is made current. Arrears include accumulated late charges or unpaid items of any kind. All guest, season or attendant passes associated with an Owners apartment, will also be suspended if an arrears issue exists. r. Renters, their guests or Attendants associated with apartments where the Owner is in arrears, will not be issued passes or be permitted to use the pool facility until and unless the Owners account is in good standing. OTHER IMPORTANT INFORMATION AND RULES: It is required that all children under the age of 16, must be accompanied by someone 16 years of age or older, while at the pool facility. Children must be properly supervised at all times. Children under 6 years of age must wear an age appropriate, Coast Guard approved flotation vest when on the pool deck if they are within 3 feet of the pool edge. Children wearing water wings or any other non-swimmer must be accompanied by a responsible attendant (swimmer) at least 16 years of age, in the water with them, at a distance of not greater than arm s length, in the more shallow end of the pool. a. Swim diapers must be worn by any child or any adult with incontinence difficulties. b. Cover-up type garments must be worn when pool patrons are off of the pool deck area. c. All pool patrons must shower before entering the water. d. Only one chair or lounge is permitted per person; Reservation of pool chairs or lounges is prohibited, except when leaving the pool area for intervals of less than 30 minutes. e. If there is a shortage of seating or pool occupancy capacity, admission and/or seating will be allocated on a first come first served basis. f. Health aids or other responsible individuals accompanying patrons with mobility or other special needs, must remain with their clients, at all times, when in the pool or on the pool deck area. Pool pass registrations, will be processed as quickly as possible and may be picked up from the doorman station in your building. There may be processing delays up to a week, so please submit your form as early as possible before the season begins. Temporary passes may be issued at management s discretion, for extenuating circumstances only (out of town during registration period). pg. 3 The Board of Managers, through their authorized agents, reserves the right to refuse anyone admission to the pool facility.

SAFETY: The following are prohibited in the pool area: Glass containers or breakable plastic of any kind. Entering the pool without showering. Pets Ball playing, velocipedes (Only strollers are permitted, if space is available and proper use / care is observed). All injuries or unusual events must be reported to the life guards. Rowdy, loud, abusive or inappropriate behavior. Food (except for infants). Drinks (unless in paper or non-breakable plastic containers). Alcoholic beverages. Smoking. Defacing or destruction of pool equipment or pool area surfaces. Use of illegal substances. Solicitation or campaigning of any kind. Diving from the sides of the pool. Running on the pool deck. Floats, rafts, snorkels, masks (covering the nose area), fins (except foot or hand fins specifically designed for lap swimming use), inflatable items or other bouncy devices / toys, (except as may be required for flotation, by adults, having a medical necessity for a device). Removal of chairs, lounges or tables from the pool deck. Playing of radios or other sound emitting devices (you must use headphones). Use of the pool when lifeguards are not on duty. Quality of life at the pool: o Residents and Guests must wear cover-up type garments when going to the pool or leaving the pool area. Cover- ups must be worn in the hallways, elevators, lobbies, and promenade deck. o Please avoid dripping water in the elevators by toweling off completely. o Hosts must meet guests in the lobby of their building and accompany them to deck or pool and remain with their guests. The host must provide the Doorman with a list of guests they expect. o Parents of younger children, who will swim, are requested to have them use the bathroom prior to entering the pool. o Fitness club bathrooms are not for use by pool patrons. There is a bathroom inside the building 1 promenade level, for their use. o Failure to comply with the rules and procedures may result in suspension of privileges. Additional important information: The Board of Managers through their authorized agent(s), reserves the right to suspend or revoke pool privileges for residents or guests who violate these rules and anyone who compromises the safety and comfort of others using the pool facility. Management and Lifeguards have the authority to request removal of any resident or guest from the pool facility; anyone who is abusive, harassing or behave in an objectionable manner. Management reserves the right to close the pool due to inclement weather, mechanical failure or other types of unexpected situations. Closings may also be executed by the lifeguard staff on duty, as needed. Pool patrons must respond in a quick and orderly manner to requests by staff to evacuate the pool deck.
