WINDY CITY JUNIORS PLAYER/PARENT CONTRACT Notice to Parents and Legal Guardians: Please read this agreement (the Agreement ) very carefully. It describes the responsibilities and obligations that come with membership in WINDY CITY JUNIORS Volleyball Club. By completing the application for membership, you agree to the terms and conditions as described herein. The Parties: 1. WINDY CITY JUNIORS Volleyball Club, hereafter referred to as WCJ, 2. Player (Print full name):, and 3. Parent or legal guardian of Player if a minor or otherwise financially responsible for Player: (Print full name and relationship) (circle one) Mother, Father, Guardian. 4. Team Assignment: This contract is for a postilion on the team. Membership Includes: 1. WCJ shall provide volleyball instruction and supervision during USA Volleyball (USAV) sanctioned volleyball tournaments, in accordance with the governing policies and guidelines established by the USAV and the United States Olympic Committee (USOC), for the 2017-18 Club Volleyball season. 2. WCJ shall provide, at a minimum, weekly instructional practices held at a facility designated by the Club and at times and for durations specified by WCJ. 3. WCJ shall provide qualified coaches. 4. WCJ shall provide game uniforms. 5. WCJ shall pay all fees and registration expenses associated with team tournament play. (travel not included) 6. WCJ shall provide training equipment, including but not limited to, volleyball nets, balls, and training aids. Membership Fee and Payment Schedule: 1. The 2017-18 membership base fees are as follows: 15s and Older: $1,800 Individual Player Fee 14s and Younger: $1,500 Individual Player Fee Developmental Team/Practice Players: $600 Player Fee 2. The following payment schedules are offered as a minimum payment plan: 14s and Younger: Payment Plan #1 The second installment payment of $300 or remainder is due on or before December 1, 2016.

The third installment payment of $300 or remainder is due on or before January 1, 2017. The fourth installment payment of $300 or remainder is due on or before February 1, 2017. The balance and final installment payment of $300 is due on or before March 1, 2017. 14s and Younger: Payment Plan #2 The first installment payment of $200 or remainder is due on or before October 1, 2016. The second installment payment of $200 or remainder is due on or before November 1, 2016. The third installment payment of $200 or remainder is due on or before December 1, 2016. The fourth installment payment of $200 or remainder is due on or before January 1, 2017. The fifth installment payment of $200 or remainder is due on or before February 1, 2017. The balance and final installment payment of $200 is due on or before March 1, 2017. (Players who have not paid by the deadline will not be permitted to compete in team activities until the payment has been received). 15s and Older: Payment Plan #1 The second installment payment of $375 or remainder is due on or before December 1, 2016. The second installment payment of $250 or remainder is due on or before November 1, 2016. The third installment payment of $250 or remainder is due on or before December 1, 2016. The fourth installment payment of $250 or remainder is due on or before January 1, 2017. The fifth installment payment of $250 or remainder is due on or before February 1, 2017. The balance and final installment payment of $250 is due on or before March 1, 2017. (Players who have not paid by the deadline will not be permitted to compete in team activities until the payment has been received). Developmental/Practice: Payment Plan The first and final installment payment of $300 is due on or before the first practice of the season. 3. Traveling Team Fees: Teams which elect to compete in additional tournaments in addition to those listed on the WCJ schedule will be required to cover the additional cost as well as provide additional payment to compensate for coach salaries, travel and expenses. 4. Practice Players: Each team may carry practice players. Practice players will be charged a reduced rate to be a part of the team. That rate will be determined by the Club Director. 5. Method of Payment: WCJ will accept checks and/or cash as payments. Checks should be made payable to: Windy City Juniors and mailed to: 1400 E College Drive Cheyenne, WY 82007 at the appropriate times or hand delivered to director at practice facility. 6. Incidentals: WCJ accepts NO liability for costs associated with travel, parking, meals, or incidental expenses incurred while participating in WCJ activities, including but not limited to, practice sessions and tournament play.

Terms and Conditions: WCJ reserves the right to change practice times, days, and durations without notice. WCJ solely determines content of instructional practices and the extent to which each player participates during practice and tournament events. WCJ reserves the right to control entry into the practice facility. Players and/or visitors may be asked to leave the practice facility or tournament venue at WCJs discretion. All Windy City Juniors practice sessions will be closed-off to all outside entities (including but not limited to: parents, peers, family, etc.) WCJ administration and coaches have the right to move players from one roster to another with that player s consent at any point during the competitive season WCJ reserves the right to suspend and/or terminate a player s membership for non-adherence to RMR policies (http://www.rm rvolleyball.org/), non-adherence to practice facility rules, or non-adherence to the terms and conditions of this Agreement, including without limitation failure to make on-time tuition payment. WCJ reserves the right to utilize player images (photos, video) for promotional tools and recruiting efforts without compensation (this includes use of player images on recruiting and social media outlets as well as on the team website, Windycityjuniors.com) The Player shall meet the below-listed minimum requirements: The player is in good, physical condition and does not have a health condition(s) that may be complicated by strenuous physical activities, such as training for and/or playing competitive volleyball. The player does NOT use illegal drugs, tobacco or alcohol. The player has a positive attitude and personally desires to play club volleyball. The player remains in good academic standing (GPA above 2.0 and/or must have a D- or better in all classes) throughout the duration of the season. Players who fall into poor academic standing may be held from practices or competition until scores improve at the discretion of WCJ. The player will remain in good standing with the law. Any player who is charged with a crime, misdemeanor or violation of state law is subject to suspension. Each case will be reviewed and dealt with on an individual basis. All violations must be reported to WCJ within 48 hours of occurring. The player agrees to obey the USAV Rules and Code of Conduct, RMR policies, WCJ Rules and Expectations, and to conduct herself as an honorable member of WCJ. The Player s Parent and/or Guardian: The Player s Parent and/or Guardian can demonstrate legal guardianship of the player if requested by the Club or governing organization. The Player s Parent and/or Guardian assume responsibility for payment of all costs, fees, and expenses associated with the Player s participation and membership in the Club. The Player s Parent and/or Guardian understand that payment in full is required and that the registration fees are nonrefundable without exception, regardless of the Player s ability to participate in Club activities, including early withdrawal or termination from the Club. If you become thirty (30) days in arrears on payments due on your account, you may, at the sole discretion of WCJ, be subject to immediate suspension from your team or termination from the club without further notice. The Player s Parent and/or Guardian acknowledges that playing volleyball can result in serious injury and the Player s Parent and/or Guardian assume all liability for Player injuries resulting from participating in Club activities. The Player s Parent and/or Guardian promise to obey the Sportsman s Code of Ethics, RMR policies, and club rules. The Player s Parent and/or Guardian assume all responsibility for associated visitors and guests adherence to the same. Voluntary termination of membership by you SHALL NOT reduce or suspend your obligation to pay all dues and other indebtedness incurred as the result of signing this Agreement, including without limitation prepayment for tournaments, gym rentals, and other reasonable expenses. If you are terminated, you will remain liable for all dues and other indebtedness incurred as the result of signing this Agreement. The Player s Parent and/or Guardian has read, understands, and freely concurs with the stipulations, fees, terms and conditions of this agreement. The Player s Parent and/or Guardian is capable of legally entering into a contract, in accordance with the laws of the State of Wyoming and freely does so by completing and signing this membership agreement, a legally binding contract.

WCJ Volleyball Club Rules and Regulations: The following rules and regulations are what are expected from all club members regardless of age level. These rules are in addition to any and all rules set forth by USA Volleyball and Rocky Mountain Region Volleyball. 1. Practice is mandatory. If you must miss a session you should notify your coach as soon as possible prior to that practice. 2. Tournaments are mandatory, if you must miss a tournament you should notify your coach as soon as possible prior to that tournament. 3. Club members should arrive 10 minutes prior to practice to ensure they are ready once practice begins. 4. Many of the WCJ coaches have strict rules regarding when you must arrive before a tournament. It is the players and parents responsibility to know this policy, and adhere to it. Arriving late to a tournament may affect playing time. 5. Rides should be at the gym no later than 5 minutes after the scheduled conclusion of practice. 6. Due to liability issues, parents and/or club member guests are not permitted to participate in practice, unless Director's approval has been given. 7. For tournaments, Club members should always have both color uniform jerseys available, have their hair pulled back, and wear knee pads and designated socks. 8. Hydration is extremely important to the health of our athletes. Club members should each bring a water bottle to each practice and must hydrate during water breaks. 9. Club members and their parents should conduct themselves in a manner that is not only becoming to themselves but as representatives of WCJ. The way we talk, the way we dress, the way we play all affects the way people view our club. Strive to present the most positive image possible when at tournaments and at practice. Any inappropriate behavior will be handled on a case by case basis, with suspension from the club as an option if actions warrant, which does not waive your financial obligation. 10. WCJ Volleyball Club has a no-tolerance policy in regards to illegal drugs, tobacco and alcohol for its members. 11. Players must remain at the tournament until dismissed by the coach. This especially includes when a team must ref the final match of the day. ALL players must remain at the gym until the assignment is complete. 12. All jewelry is to be removed prior to practice or tournaments. 13. In cases where a player is injured, she is still allowed to attend practice. However, the athlete will not be allowed to participate in the physical part of the training, but can participate in the "mental" side of the training. 14. Playing time will not be discussed with parents at a tournament or during practice. You should make an appointment where the two of you can talk at least 24 hours after the event s conclusion. Playing time is at the complete discretion of the coach, and is based on a number of factors including but not limited to performance in practice, games, etc. Opportunities are always given at practice, which is where the majority of the athletes learning takes place. Coaches will be required to make all efforts to distribute playing time equitably (this does not mean equally). Please talk over the playing time policy with your child and make sure to make the proper decision for them. Parent and Player Code of Conduct: Youth sports programs play an important role in promoting the physical, social and emotional development of children. It is therefore essential for parents to encourage youth athletes to embrace the values of good sportsmanship. Moreover, adults involved in youth sports events should be models of good sportsmanship and should lead by example by demonstrating respect and self-control. I therefore pledge to be responsible for my words and actions while attending any youth sports event with regards to WCJ Volleyball Club and shall conform my behavior to the following code of conduct: I will not engage in unsportsmanlike conduct with any coach, parent, player, participant, official or any other attendee. I will not encourage my child, or any other person, to engage in unsportsmanlike conduct with any coach, parent, player, participant, official or any other attendee. I will not engage in any behavior which would endanger the health, safety or well-being of any coach, parent, player, participant, official or any other attendee. I will not encourage my child, or any other person, to engage in any behavior which would endanger the health, safety or well-being of any coach, parent, player, participant, official or any other attendee. I will not use drugs or alcohol while at a youth sports event and will not attend a youth sports event while under the influence of drugs or alcohol. I will not permit my child, or encourage any other person, to use drugs or alcohol at a youth sports event and will not permit my child, or encourage any other person, to attend, coach, officiate or participate in a youth sports event while under the influence of drugs or alcohol. I will not engage in the use of profanity. I will not encourage my child, or any other person, to engage in the use of profanity.

I will treat any coach, parent, player, participant, official or any other attendee with respect regardless of race, creed, color, national origin, sex, sexual orientation or ability. I will encourage my child to treat any coach, parent, player, participant, official or any other attendee with respect regardless of race, creed, color, national origin, sex, sexual orientation or ability. I will not engage in verbal or physical threats or abuse aimed at any coach, parent, player, participant, official or any other attendee. I will not initiate a fight or scuffle with any coach, parent, player, participant, official or any other attendee. I will not engage in gossip about players, coaches or team families as this has no place in this Club. I will instruct my child not to gossip about players, coaches or team families as this has no place in this Club. I will not use social media outlets such as Facebook, Twitter, etc. to post comments that are negative in anyway of players, coaches, officials, or anyone who is associated with Club Volleyball. I, as a parent will not coach my child or any other player on my child s team during a Club scheduled practice or tournament event. I hereby agree that if I fail to conform my conduct to the foregoing while attending a WCJ Volleyball Club youth sports event (which includes practices) I will be subject to disciplinary action, including but not limited to the following in any order or combination: Verbal warning issued by WCJ Volleyball Club or representative of USAV/Rocky Mountain Region Volleyball. Written warning issued by WCJ Volleyball Club or representative of USAV/Rocky Mountain Region Volleyball. Suspension or immediate ejection from a youth sports event issued by WCJ Volleyball Club or representative of USAV/Rocky Mountain Region Volleyball. Suspension from multiple youth sports events issued WCJ Volleyball Club or representative of USAV/Rocky Mountain Region Volleyball. Season suspension issued by WCJ Volleyball Club or representative of USAV/Rocky Mountain Region Volleyball. I also understand and agree that I am equally responsible and I will be subject to the same disciplinary action for conduct from my attending family members and guest that have not signed this Code of Conduct. I understand and agree that if my Club is fined due to my behavior or behavior of my attending family members or guest at any RMR event I am financially responsible for any fines imposed due to my or their misconduct. Release and Waiver: I give my permission for the above named player to participate in WCJ Volleyball Club. I understand that volleyball is a limited contact sport that involves jumping, running, and ball handling in a confined playing area and injuries may occur. I acknowledge that even with the best coaching, the use of the most advanced protective equipment, and strict observance of the rules, injuries are still possible. I hereby authorize the WCJ staff to act for me according to their best judgment in any emergency situation requiring medical attention and I hereby waive and release WCJ Volleyball Club and director s, Board Members, and staff from any and all liability stemming from any injuries or illnesses incurred while participating in the club. I understand, agree, and acknowledge that some activities may be of hazardous nature and/or include physical and/or strenuous exercise or activity. If any injury is sustained and requires hospitalization, I understand that I or my medical insurance company is solely responsible for any and all bills and claims that may be filed as a result of the injury. With full understanding of these facts, I state that to the best of my knowledge, my daughter listed above has no medical, physical, mental, or emotional health conditions that would hinder or prevent her participation in WCJ Volleyball Club. Indemnification: To the fullest extent permitted by law, players, parents and/or guardians agrees to indemnify and hold harmless WJC, its officials, employees, officers, agents, successors, assignees, volunteers, and its designated practice or game facilities from any and all lawsuits, losses, liability for injuries, damages, claims, penalties, actions, demands or expenses arising from or in connection with this Contract. Miscellaneous The rules and regulations included in this Agreement are NOT inclusive. Amendments to WCJ rules and regulations may be made from time to time as necessary. On all questions regarding the interpretation of and application of the WCJ rules and regulations, the decision of the Directors of WCJs shall be final. I, the Player and/or the Player s parent or legal guardian, have read, understand, and freely accept this agreement as written and indicate so by my signature below.

By signing this document I acknowledge that I have read in full, understand and agree to abide by the statements in this membership agreement which include Membership Fee and Payment Schedule, Terms and Conditions, WCJ Volleyball Club Rules and Regulations, Parent and Player Code of Conduct and Release and Waiver.
