This information is in an abbreviated form. *** NO RECORDS FOUND *** PART TWO - MAJOR PENALTIES The following actions published herein and imposed pursuant to the rules and regulations of the Racing Commissions of the various states and provinces are given full faith and credit and strictly enforced by the United States Trotting Association consistent with the provisions of USTA Rule 22, Section 13 and 14 relative to RECIPROCITY OF PENALTIES. The fact that there is no separate action taken by the USTA in the form of a "ruling of reciprocity" shall not be construed as a failure to recognize and/or reciprocate with those published penalties. Please note that the following action is in an abbreviated form. Further information may be available by contacting the Racing Commissions or their representatives. Mica Beauchamp is now under Full Suspension for 30 days beginning 1-15-19 to and including 2-13-19 for failing a human drug test. Mica Beauchamp was positive for MARIJUANA (see DHRC rule 5.1.8.5.2). Mica Beauchamp is also responsible for the cost of the failed test ($30.00). The State Steward did not recommend him for licensure. The NJRC conducted an informal hearing with Raymond Burt on November 20, 2018 and the Commission determined that Raymond Burt had failed to meet his burden of establishing his qualification for licensure in

accordance with NJAC 13:71-7.7, 7.27 and 7.31. Therefore, for the reasons set forth in the Commission`s letter dated January 16, 2019, the application of Raymond Burt for licensure in New Jersey as a standardbred owner and trainer is hereby denied. Raymond Burt is denied access to all grounds that are under the jurisdiction of the NJRC for any and all purpose except for those areas of racetracks that are open to the public. DEPIETRO, ANN L CREAM RIDGE, NJ YOB 1966 Fhld on 11/24/2018 FINED: $500 FULL - 15 DAYS, 1/13/2019 THRU 1/27/2019 LOW OR EXCESS LEVEL OF AUTHORIZED MEDICATION (BUTE/LASIX) Ann Depietro was the programmed trainer of the horse "CAMS ORACLE" which raced in the 3rd race at Freehold Raceway on Saturday, November 24, 2018. "CAMS ORACLE" finished 2nd on November 24, 2018, and as a result, was required to submit blood samples for analysis. A report from Truesdail laboratories, which is the official chemist for the New Jersey Racing Commission, of the blood same collected from "CAMS ORACLE" identified the presence of Phenylbutazone above the allowable limits. This is in direct violation of N.J.A.C. 13:71-23.8(f). As a result, Ms. Depietro is hereby issued a full suspension of her New Jersey Racing Commission license for a period of fifteen (15) days. The suspension shall begin on January 13, 2019 and run through January 27, 2019, inclusive. In addition, Ms. Depietro is fined the sum of five-hundred ($500.00) dollars. During the period of suspension, Ms. Depietro is denied access to all grounds that are under the jurisdiction of the New Jersey Racing Commission GARRETT, NOAH TIPP CITY, OH YOB 1974 DTN on 12/10/2018 FINED: $1,000 LOW OR EXCESS LEVEL OF AUTHORIZED MEDICATION (BUTE/LASIX) Dayton Raceway O.S.R.C. 3769-18.01(B)(4)(d) Trainer NOAH GARRETT is hereby fined the sum of one thousand dollars ($1,000.00) for violation of Ohio State Racing Commission Rule 3769-18-.01(B)(4)(d). Second offense within 365 days. Blood Sample #849753 tested by the Analytical Toxicology Laboratory at the Ohio Department of Agriculture on January 4, 2019 was found to contain furosemide at a concentration of 142 + 17 nanograms per milliliter of plasma which exceeds the O.S.R.C. regulatory threshold of 100

nanograms. In addition, MIKES HOPE disqualified from first and placed last. Any purse money earned by MIKES HOPE in the race must be returned and redistributed. All fines due within 48 hours from receipt of this ruling. JOHNSON, JOHN M GLEN ELLYN, IL YOB 1969 Mea on 1/3/2019 FINED: $50 INDEFINITE SUSPENSION(IND) AS OF 1/8/2019 PA Code Title 58-183.266,183.267(8). Mr. Johnson failed to complete his licensing requirements in Pennsylvania in a timely manner. He is hereby issued a $50 fine, which is due by January 18, 2019. Mr. Johnson is hereby suspended indefinitely beginning January 8, 2019, until such time he completes his licensing requirements. During the term of suspension, Mr. Johnson is banned from all racing activities otherwise permitted if properly licensed by the Commission. Mr. Johnson has 10 days to appeal this decision to the Pennsylvania State Horse Racing Commission, Bureau of Standardbred Racing. KONRAD, EDWARD M WESTLAND, MI YOB 1964 Nfld on 12/15/2018 FINED: $500 Blood sample # 850747 collected at Northfield Park on 12/15/18 from the horse Im Shameless was found to contain flunixin at a concentration of 58+- 11 nanograms per milliliter of plasma. This exceeds the Regulatory Threshold of 20 nanograms per milliliter adopted by the OSRC. O.S.R.C.- 3769-18-01(B)(1)(a) 18-02 18-99 LOY, TYE R COLUMBIA STATION, OH YOB 1955 Nfld on 12/8/2018 FINED: $1,000 HORSE DISQUALIFIED, PURSE REDISTRIBUTION Urine sample #850675 collected at Northfield Park on 12/8/18 from the horse Dragon Lady Art was found to contain methocarbamol at a concentration of 3.2+-0.5 micrograms per milliliter of urine. This exceeds the Regulatory Threshold of 1.0 microgram per milliliter adopted by the OSRC. All purse money must be returned for redistribution ($5,250.00). Ms. MacKenzie failed to complete her licensing requirements in Pennsylvania in a timely manner. She is hereby fined $50, which is due by January 18, 2019. Ms. MacKenzie is hereby indefinitely suspended beginning January 8, 2019, until such time she completes her licensing requirements. During the term of suspension, Ms. MacKenzie is banned from all racing activities otherwise permitted if properly licensed by the Commission. Ms. MacKenzie has 10 days to appeal this decision to the Pennsylvania State Horse Racing Commission, Bureau of Standardbred Racing. MAIER, DANIEL R LEBANON, OH YOB 1984 DTN on 11/19/2018 FINED: $1,000 FULL - 90 DAYS, 3/3/2019 THRU 5/31/2019 Dayton Raceway O.S.R.C. 3769-18-01(B)(10&13) 18-02 18-99 Trainer DAN MAIER has been fined one thousand dollars ($1,000.00) and issued a 90- day full suspension from March 3, 2019 thru May 31, 2019. Blood sample #860185 on the horse DESIRE ONE collected at Dayton Raceway on November 19, 2018 was found to contain presence of Ambroxol, an expectorant. In addition, Mr. Maier is required to return purse money earned four thousand two hundred and fifty dollars ($4,250). During his suspension Mr. Maier is denied the use and privileges of all grounds sanctioned by the OSRC. All horses owned and/or trained by Mr. Maier are suspended and may only be transferred by the judges at Dayton Raceway and/or the OSRC in accordance to its policy. All fines due within 48 hours from receipt of this ruling, PLUTA, KIM S JACKSON, MI YOB 1970 Nfld on 12/1/2018 FINED: $500 FULL - 15 DAYS, 1/9/2019 THRU 1/23/2019 HORSE DISQUALIFIED, PURSE REDISTRIBUTION

Blood sample #850583 collected at Northfield Park on 12/1/18 from the horse Kick It In Kate was found to contain Meloxicam. All purse money must be returned for redistribution ($4,000.00). O.S.R.C.-3769-18-01(B)(10)&(13) 18-02 18-99 3769-17-30(B) 17-43 Mr.Pluta is barred from all grounds governed by the OSRC during his suspension. Any horses under his care that he wants to transfer to another owner or trainer must be approved by the judges or the OSRC. ROSSI, MICHAEL DOVER, DE YOB 1963 DD on 1/10/2019 FINED: $200 FAILURE TO PASS THE BREATH ANALYZER TEST Considering mitigating circumstances, driver Michael Rossi is issued a $200.00 fine for failing the Breathalyzer test (0.044) on 1-10-19 at Dover Downs(see DHRC rule 5.1.8.2.6).Breathalyzer Test showing a reading of more than 0.02 percent of alcohol is considered a positive test. All fines are due within 10 days of written rule. STAMATIEN, ARISTOTLE G WEST HAVEN, CT YOB 1930 MR on 10/18/2018 FINED: $500 FULL - 10 DAYS, 1/19/2019 THRU 1/28/2019 As trainer of record of #2, Eagle Pass," that finished 1st in Race 10 at Monticello Raceway on 10/18/18, while the drug testosterone was present in his system in violation of the above-stated rules, the purse was returned and redistributed and horse was disqualified with 30 day Full Suspension and $1,000.00 fine.. Reduced to 10 days and $500 fine. Starts 1/19/2019 through 1/28/2019. eligible 1/29/2019 and payment of fine. VAN OTTERLOO, HARVEY ROCK VALLEY, IA YOB 1937 Mea on 12/11/2018 RESTORED TO GOOD STANDING(RES) AS OF 1/16/2019 PA Code Title 58-183.266,183.267(8). Mr. Van Otterloo failed to complete his licensing requirements in Pennsylvania. He is hereby fined $50, and issued an indefinite suspension beginning December 11, 2018 until such time he completes the licensing requirements and pays the above fine. During the term of his suspension, Mr. Van Otterloo is banned from all racing activities otherwise permitted if properly licensed with the
