At intersection of Auburn St. & Meridian Rd. turn right. When AUBURN dead ends into Winnebago Rd., turn left. Go straight through light at intersection of Winnebago Rd. & Bypass 20. At first stoplight (Bell School Road), turn right (north). At next light, turn right into school parking lot. HARLEM 735 Windsor Rd. Loves Park, IL 61115 Take 251 South to Windsor Road. Turn left at the light and look for the Middle School on the right hand side of the road. CHRISTIAN LIFE 5950 Spring Creek Rd. Rockford, IL 61114 Take 251 South to 173. Turn Left on 173 and follow until you reach Perryville Road. Turn right onto Perryville Road and continue until you come to Mulford Road. Turn right onto Mulford Road and continue all the way past Spring Brook Rd. Stoplight for the school entrance will be at the top of the next hill. Turn right into the parking lot. For Soccer, be aware the fee for admission is $2 for adults and $1 for children. BYRON 850 N. Colfax Street Byron, IL 61010 Go to HWY 75. Take HWY 75 to Meridian Road. Turn left onto Meridian. Take Meridian Road to Route 2. Turn right onto Route 2. Take that into Byron. You will go to the 2nd stop light and turn right onto Tower Road. (I think one of the signs also says N. Colfax ) The Middle School is on the right about 1½ blocks down. SPENCER PARK Take I-90 South until you reach E. State Street exit in Rockford. Exit I-90 and at light turn left on E. State St. Follow into Belvidere (Business Route 20) and turn right N. Appleton Road. This road literally goes straight into the park. DALQUIST PARK Take 251 South until you reach E. State Street in Rockford. Turn left on E. State and continue until you reach 29 th Street. Turn right on 29 th Street and follow to the park. FULLER FOREST PRESERVE Take I-75 to Meridian Rd. Turn left (south) on Meridian Rd and continue all the way past Route 20 south of Rockford. At intersection of Fitzgerald Road & Meridian, turn left. Entrance to Forest Preserve should be the 1 st right on Fitzgerald Road.

DRUG & ALCOHOL POLICY It is the responsibility of all athletes to remain substance-free. As stated in the Student Handbook, any student who violates this policy is subject to suspension or removal from the team, in addition to the standard disciplinary measures. MEDICAL HISTORY & CONSENT A signed and dated Certificate of Physical Fitness and Medical Treatment form must be on file for each participating athlete in a given sport. These waivers are designed with the safety and well being of each student in mind. PARENT/GUARDIAN PERMISSION The Agreement To Participate forms must be signed, dated, and returned to the coach in order for any student to participate in a particular sport. These forms must be turned in before the date of the first scheduled tryout for that sport. Athletes will not be allowed to participate in tryouts until this form has been signed. ATHLETIC FEE The athletic fee for each sport is $50, with an individual maximum of $150 (3 sports) and a family maximum of $250. The athletic fee must be paid before the date of the first scheduled contest for that sport. Please make checks out to SMMS. Athletes will not be allowed to participate in competition(s) until this fee has been paid. It should be noted that the purpose of the Athletic Fee is to offset the costs involved in each sport for busing, officiating, etc. It is NOT intended to help in financing uniforms and equipment this is the sole purpose of the Booster Club. TEAM SELECTION Individual teams for each sport are selected by way of a try-out process at the beginning of the season. Two teams will be selected: 8 th grade team, and 7 th grade team. All 6 th and 7 th grade students may try out for the 7 th grade team. Eighth grade students may tryout for the 8 th grade team only. If a particular sport has lower numbers at tryouts, 7 th grade students are also given consideration for the making 8 th grade team roster; however this is purely up to the discretion of the coach and athletic director. Due to the large number of students trying out, it may be necessary for coaches to reduce the number of participants trying out by posting first cuts, in addition to final cuts. Final cuts will determine the players to be rostered for that particular team. All 6 th and 7 th grade students who do not make either cut are encouraged to try again the following year. PRACTICES Practice times and dates will be set by the coach of that particular sport. Due to the number of athletic teams and different coaching schedules, some practices may not start immediately after school. Also, the time and location of practices are subject to change pending availability of the SMMS, WPES, and RGS facilities. Saturday practices are occasionally scheduled, and are determined by the coach as he/she sees fit. Athletes are responsible to attend every scheduled practice. Please make every possible effort to inform the coach of dates in which this is not possible. SCHEDULES Schedules for each sport s contests will be distributed at the beginning of the season. Please keep in mind that some flexibility is necessary during the season for rescheduling contests that may pose a conflict to the schools or teams involved, or due to uncontrollable circumstances (i.e. weather, emergency, etc.) GAMES Our athletic teams are involved in a competitive interscholastic league, the Northern Conference. Coaches treat each contest with this idea in mind. Each coach has a different approach in determining the amount of time an individual player may play in a game. Players are not guaranteed a certain amount of playing time in games. GAME TIMES All conference games start as follows unless otherwise indicated: cross country and track at 4:00pm; soccer at 4:15pm; basketball and volleyball at 4:30pm. The next game starts ten minutes after the first contest. The order of teams performing depends on the individual sport. In soccer, 8 th grade team plays first, followed by the 7 th grade team. In all other sports, the 7 th grade contest is played first, followed by the 8 th grade contest. Track meets are one single event with one start time. GAME LOCATIONS The site for all SMMS athletic team home contests will be at the SMMS campus. A list of sites and directions for all away contests may be found in the handbook. EQUIPMENT All athletes are responsible to have proper equipment for that particular sport in accordance with IHSA rules and regulations. Soccer coaches, in addition, request that each athlete provide his/her own water and soccer ball (for training). All other equipment provided by SMMS is property of the school and district. Penalties will be assessed for vandalized or stolen property. UNIFORMS All uniforms are property of SMMS and must be returned clean at the conclusion of the season. Any athlete whose uniform is unaccounted for by the end of the season will be subject to a fine by the school. Any part of the uniform that is purchased prior to or during the season becomes the property of the athlete. TRANSPORTATION Parents are responsible for transporting athletes to and from practices when necessary. Parents are also responsible for transporting athletes to and from games when transportation is not provided. Bus transportation to all away games will be provided; however, transportation back will not, with the exception of schools outside the greater Rockton/Roscoe and Rockford area. We ask that parents please cooperate in helping us keep busing costs to a minimum. Athletes are allowed to travel home from away contests with parents/guardians only if (a) permission has been given by the coach, and (b) the parent/guardian has signed the bus release form. Athletes carpooling with another athlete/family will need the written consent of a parent/guardian one days prior to the scheduled contest in question. BUS TRAVEL Athletes are responsible to abide by all safety rules and regulations of the driver and coaches when traveling by bus to contests. Any athlete not in accordance with these said rules will be subject to disciplinary action by the school administration. BOOSTER CLUB This volunteer organization was formed by parents to help offset the expenses involved with extracurricular sports, and to help provide the necessary facilities and equipment for each activity. Membership fee is $25 per school year. Any volunteer help you can offer during the course of the season or school year would be greatly appreciated. Please see insert in middle of the handbook.

Concussion Acknowledgement Form A concussion is a brain injury and all brain injuries are serious. They are caused by a bump, blow or jolt to the head, or by a blow to another part of the body with the force transmitted to the head. They can range from mild to severe and can disrupt the way the brain normally works. Even though most concussions are mild, all concussions are potentially serious and may result in complications including prolonged brain damage and death if not recognized and managed properly. You cannot see a concussion and most sports concussions occur without loss of consciousness. Signs and symptoms of concussion may show up right after the injury or can take hours or days to fully appear. If your child reports any symptoms of a concussion, or if you notice the symptoms or signs of concussion yourself, seek medical attention right away. What can happen if a child keeps on playing with a concussion or returns too soon? Athletes with the signs and symptoms of a concussion will be removed from play immediately. Continuing to play with the signs and symptoms of concussion leaves the young athlete especially vulnerable to greater injury. There is an increased risk of significant damage from a concussion for a period of time after the injury occurs, particularly if the athlete suffers another concussion before completely recovering from the first one. Second Impact Syndrome is the effect of repeated concussions. If an athlete sustains a second concussion during this period, the risk of permanent brain injury increases significantly and the consequences of a seemingly mild second concussion can be very severe, and even result in death. It is well known that athletes will often fail to report symptoms of injuries. As a result, education of administrators, coaches, parents and students is the key to studentathlete s safety. If you think your child suffered a concussion Any athlete even suspected of suffering a concussion will be removed from the game or practice immediately. An athlete may not return to practice, game, competition or physical education class after an apparent concussion or head injury. This is regardless of how mild it seems or how quickly symptoms clear without medical clearance. The policy of the Rockton School District #140 requires athletes to provide the school with written clearance from a physician, licensed to practice medicine in all its branches, prior to returning to play or practice following a concussion or after being removed from a practice, game or competition due to possible head injury or concussion. You should also inform your child s coach or school if you think that your child may have a concussion. Remember it is better to miss one game than miss the whole season. The student must meet the academic criteria set forth in the Rockton School Board policy on Extracurricular and Co-Curricular Activities. 2. Written permission must be given by the parent(s)/guardian(s) for the student s participation, giving the District full waiver of responsibility of the risks involved. 3. A physical examination of the student must be conducted by a physician, nurse practitioner, or physician s assistant accompanying written statement assuring that the student s health status allows for active athletic participation must be submitted to the District only. The Superintendent shall maintain the necessary records to ensure student compliance with this policy. RULES AND REGULATIONS ELIGIBILITY In accordance with the school policy at SMMS, any athlete that is currently receiving a failing grade in ANY class is ineligible for that week s competition and practices. Eligibility status is determined on a weekly basis. Students who are ineligible are expected to use this time to bring his/her grades up so they may participate the following week. All Athletes must be in attendance for ½ day in order to participate in that day s event/contest (p.25 Student/Parent handbook). CURRENT PHYSICAL Proof of a current physical examination must be presented to the school before the student is allowed to participate in tryouts. Any physicals that may expire during the course of the season will need to be updated in order for that student to continue his/her participation in that sport. A current physical is one completed within the last year. No exceptions will be made. CHAIN OF COMMAND The chain of command at SMMS is as follows: Athletes Parent Coach Athletic Director Assistant Principal Principal Athletes and Parents should seek out their coach first for all team related matters. As a courtesy to all coaches, we ask that a 24-hour Rule be put into effect when making such arrangements, not the same night of a contest. The Athletic Director is the next line of command. Please respect the wishes of our administration in following this. Much thanks. SPORTSMANSHIP All athlete, coaches, and spectators have a responsibility to represent Stephen Mack Middle School with the utmost amount of respect and class before, during, and after each game. Any inappropriate behavior or conduct will NOT be tolerated.
