Congratulations on trying out for the new 2016-2017 SLAM Sirens Dance Team! This is your audition packet. It contains all of the important information that you need to know if you are chosen to become a member of the team. It is very important that you carefully read all aspects of the packet and turn in all completed and signed items by the deadline date given. If this packet is not completed and turned in by the deadline date, then you will not be permitted to try out! Here is what must be turned in on Monday, May 23 rd : 1. Audition application 2. Signed acknowledgment of the SLAM Sirens Dance Team Constitution. Preliminary Meetings Tuesday, May 10 th : Mandatory Parent/Dancer Meeting for all dancers interested in auditioning for the SLAM Sirens. SLAM Academy SLAM Sirens Dance Team Parent Letter Dear Parents/Guardians, Your son/daughter is interested in becoming a member of the SLAM Sirens Dance Team for the 2016-2017 school year. If your son/daughter is chosen to be a dancer, there are specific responsibilities and obligations, which your son/daughter must assume in order to qualify and remain part of the team. After reading the attached constitution (available at www.slammiami.com) and fully understanding the rules and regulations which govern this program, you and your child must sign and return the following: 1) application 2) acknowledgment of the constitution, 3) the release form, and 4) waiver form. Return the forms to Ms. Sapp on or before Monday, May 23 rd. Letters stating whether or not your student has been selected for the team will be handed out on Monday, May 30 th. There will be a mandatory informational meeting for parents/guardians on Tuesday, May 31 st from 6:00pm 7:00pm. Initial fees will be due at this meeting. The cost for a Sirens Dancer is $945 (with possible additional fees for out-of-town performances and competitions), which includes the following: 1. Gear. Warm-ups, shoes, practice/performance clothes, Camp and accessories. 2. Local Competition. The Sirens compete in at least one Regional and one State competition per year. 3. Orlando Competition. The Sirens compete in one National competition in Orlando each year. 4. Sponsorships. In previous years, several dancers were able to get assistance with their membership dues through various sponsorships of families and friends. 5. Payment schedule: $325 is due on May 31 st. Parents will have the option for pay with cash or check. It is important that the entire amount is paid in a timely manner so that I can order and receive items before the beginning of the school year. 6. Fundraising. Members will be expected to participate in ALL fundraisers, for which a dancer may use the funds to pay off the remaining $620 balance. Additional moneys that are raised from fundraisers will go towards competition and any other necessary fees that are associated with competition, so it is very important to fundraise as much as we can! We participate in WEEKLY bake sales. Being a member of the SLAM Sirens Dance Team has many rewards and is a valuable experience that students will remember for a lifetime. Additionally, your son/daughter will give pride and spirit to our school, students, and community through their participation! Audition Expectations & Procedures Auditions. They will be conducted by the SLAM Sirens Dance Coach Attire. Dancers must wear a black leotard, black shorts, nude dance tights, and black jazz shoes/bare feet. Dancers must have their hair in a perfect low bun, and may not wear jewelry of any kind. No gum. No food. Please bring a bottle of water, a positive attitude, and a big smile. Judging. Each participant is assigned a number before tryouts and will perform for the judges in small groups. The judges will be evaluating the number, not the name. Judges. The judging panel may be comprised of previous dance members, SLAM faculty, or dance professionals. Location. Auditions are held in the Dance Room and are closed to all spectators. Results. Decisions will be made by the coach, based on the individual dancer s strengths. Scoring. Candidates will be scored based on a score sheet of 50 points per judge. The score sheet is broken down into the following: Dance combination (Technique and Memory) Battement sequence (Technique and Memory) Pirouettes (Technique) Showmanship (Energy, Facial Expressions and Rhythm) Overall impression (Attitude, Attire and Teacher Recommendation)

SLAM Academy Middle/High School Dance Team Waiver/Release Form As the parent or legal guardian of, Name of Son / Daughter I agree to the following terms for participation with the SLAM Sirens Dance Team Program: 1. I grant permission for my son/daughter to participate in the SLAM Sirens Dance Team auditions/practice sessions/camps/games/competitions. I realize that dance is an activity where physical illness or physical injury (minimal, serious, or catastrophic) can occur; therefore, any medical/accidental expenses or liability that possibly results from an illness or injury to my son/daughter during the SLAM Sirens Dance Team auditions/practice sessions/camps/games/competitions will be my responsibility. I further acknowledge and understand that my son/daughter is assuming the risk of such physical illness or injury by his/her participation, and I further release Miami-Dade County, SLAM Academy Middle/High School and its representatives from any claims for personal illness or injury that my son/daughter may sustain during participation in this activity. My son/daughter is properly covered for any accident by the following insurance: Insurance Provider / Policy Number 2. SLAM Academy Middle/High School, its officers, regents, employees, and agents assume no responsibility for supervision of students outside of auditions/practice sessions/camps/games/competitions. I hereby authorize the dance coach, athletic trainer, or any other supervising adult to obtain medical treatment for my son/daughter for such an illness or injury during the activity.
