ZIVA Player contracts!! Notice to ZIVA Players/ Parents/ Legal Guardian: Please read this agreement (the Agreement ) very carefully. It describes the responsibilities and obligations that come with being a member of Zone-In Volleyball Academy. By signing and completing this form, you agree to the terms and conditions as described herein. The Parties: 1. Zone-In Volleyball Academy, hereafter referred to as ZIVA, 2. Player (Print full name):, and 3. Parent or legal guardian of Player if a minor or otherwise financially responsible for Player: (Print full name and relationship). PLAYERS SHALL MEET AND UNDERSTAND THE BELOW LISTED REQUIREMENTS AND CONSEQUENCES: 1.) Tournament Attendance: -Absence: Excused and Unexcused are as follows: -Excused: Excused are as follows: 1. Sick with doctors note or injury with doctors note 2. Conflict with school sporting event that they were previously committed to prior to signing their contract and/or stated on contract. 1st Excused results in no penalty 2nd Excused results in meeting with head coach, player, parent and refund and still be required to pay out the rest of players contract. -Unexcused: Unexcused are as follows: 1st unexcused results in one game suspension and a meeting with ZIVA Management to determine the future of the player on the team. 2nd unexcused results in removal from team with no refund and required to pay out rest of contract. - Late: One warning is allowed and then consequences are as follows: 2nd Violation results in no start 3rd Violation results in game suspension 4th Violation results in meeting with head coach, player, parent and refund and still be required to pay out the rest of player contract.

2.) Practice Attendance: -Absences (EXCUSED): 3 Excused Practices Allowed (Excused Are As Follows) Sick with doctor s note/ injury with doctors note Conflict with school sporting event that overlaps for the entire length of ZIVA practice/event that they were previously committed to prior to signing their contract and/or stated on their contract. -Absences (UNEXCUSED): Unexcused consequences are as follows: 1st unexcused results in a no-start 2nd unexcused results in game suspension. 3rd unexcused results in meeting with head coach player and parent with ZIVA management to discuss whether player will be dropped without refund and will still be required to pay out the rest of their contract. - Late: One Warning is allowed and then consequences are as follows: 2nd violation results in no-start 3rd violation results in game suspension 4th violation results in meeting with player, head coach, parent and refund and will still be required to pay out the rest of their contract. 3.) Players Participation in Non-Volleyball Events: Players are required to attend the following. Failure to do so will result in the same consequences as stated above in missed practices. Community Service Team required fundraisers Individual fundraisers: Money/products not returned will be added to player account. 4.) Zero-Tolerance Drug Policy: The player does NOT use illegal drugs or tobacco. -ZIVA related event Alcohol Violation 1st Alcohol Offense results in player removal without refund and required to pay out contract. -Non-ZIVA related event Alcohol Violation 1st Alcohol Offense results in forgiveness, with a tournament play suspension, player will still be expected to travel and sit on the sideline. 2nd Alcohol Offense results in player removal without refund and required to pay out contract.

5.) Players Code of Conduct: Players are required to encourage good sportsmanship to fellow players, coaches, officials and parents. Players will not use obscene language or gestures and will not physically or verbally intimidate, abuse or assault other players or coaches. In failure to follow the Code of Conduct, the 3 Levels of discipline are as follows: Level 1: Talking back, foul language, bad attitude, lack of effort, and disrespect to authority will result in a warning with one week for improvement before the next action will be taken. Level 2: Teammate bullying, continued back talk, continued foul language, continued disrespect to authority will result in player suspension of game time. Level 3: Continued failure to adhere to Players Code of Conduct with no improvement seen will result in meeting with head coach, player, parent and ZIVA management to discuss whether player will be dropped without refund and will still be required to pay out the rest of their contract. 6.) Parent Code of Conduct: The Player s Parent and/or Guardian promise to obey the Sportsman s Code of Ethics, and Conduct as listed below in the 3 Levels. The Player s Parent and/or Guardian assume all responsibility for associated visitors and guests adherence to the same. Level 1: Getting kicked out of a match, public disrespect to ZIVA coaches, management, director, use of profanity in front of players & coaches, inappropriate communication with ZIVA coaches (during practice or tournaments), or failure to pay bill on time results in a warning with one week for improvement before the next action will be taken. Level 2: Continued behavior of level one will result in player suspension of game time. Level 3: No fix will result in player removal without refund and required to pay out contract. 7.) Payment: The Player s Parent and/or Guardian understand that payment as scheduled is required and that each payment as received is non-refundable, without exception, regardless of the player s ability to participate in Club activities, including early withdrawal or termination from the Club. If you become fifteen (15) days in arrears on payments due on your account, the player, at the sole discretion of ZIVA, may be subject to immediate suspension from the team or termination from the club without further notice.

Payment Due on the 5 th of every month, if not paid player is suspended (practice or game) until payment is received. These missed practices and games will count as unexcused missed events. MISCELLANEOUS Conflict resolution: Parents have to set up a meeting to meet or talk with the Head coach. Never can a parent approach Coach during a practice or tournament. If not resolved then meeting will be set up with Management to resolve conflict. If not resolved meeting with Club Director with be set up. Playing Time: Equal playing time is not guaranteed. Coaches will do what they feel is in the best interest of the team as a whole. Release Policy: Extreme circumstances: i.e., family moves, season ending injury etc. a meeting will be held with management to discuss the release without penalty. Parent Guidelines: Kids that need a ride, get in contact with coach. Church offered Sunday mornings Cheering for team not yelling at refs Support your kids, they need to know you are proud Respect the coaches decisions and deal with questions in a respectful manner, set up a meeting.!!!!!!!! *Previously planned involvement in outside organizations i.e. school teams needs to be stated below.

Terms and Conditions: Zone In Volleyball Academy: ZIVA reserves the right to change practice times, days, and durations without notice. ZIVA solely determines content of instructional practices and the extent to which each player participates during practice and tournament events. ZIVA reserves the right to control entry into the practice facility. Players and/or visitors may be asked to leave the practice facility or tournament venue at ZIVA s discretion. ZIVA reserves the right to suspend and/or terminate a player s membership for non-adherence to the above requirements and policies, non-adherence to practice facility rules, or non-adherence to the terms and conditions of this Agreement. *The Player s Parent and/or Guardian has read, understands, and freely concurs with the stipulations, fees, terms and conditions of this agreement. *The Player s Parent and/or Guardian is capable of legally entering into a contract, in accordance with the laws of the State of Texas and freely does so by completing and signing this membership agreement, a legally binding contract.
