Safety Data Sheet Product Name: Carb-O-Philic 10% Cream Section 1: Identification Product Use: For removal of keratinized skin and for loosening, debriding and avulsion of devitalized nail tissues. Pharmaceutical for human use. Manufacturer: GERITREX Address: 144 Kings Bridge Road East, Mount Vernon, New York 10550 General Information: 914-668-4003 EMERGENCY Number: 1-800-736-3437 Classification of the substance or mixture: Emergency Overview: Section 2: Hazard(s) Identification Product description: This product is white to off white odorless cream. The main hazard associated with exposure during normal use and handling is the potential for irritation of contaminated skin, eyes. Individuals who have had allergic reactions to products containing the active ingredient, urea, or any other components of this product may experience allergic reaction to this product. Flammability Hazards: This product is combustible. This material may decompose and produce irritating vapors and toxic compounds of carbon including carbon oxides, nitrogen oxide, ammonia, biuret and cyanuric acid. Vapors may travel considerable distance to source of ignition and flash back. Vapor forms explosive mixture with air. Reactivity Hazard: This product is not reactive. Stable. Conditions to avoid, high temperatures, sparks, open flames and all other source of ignition. May form explosive peroxides on prolonged storage. Emergency : Emergency responders should wear appropriate protection for situation to which they respond.

Protective Equipment: General ventilation, eye and hand wash. Rubber boots and heavy rubber gloves. Section 3: Composition/Information on Ingredients Substance/mixture: Urea 57-13-6 40% Trolamine 102-71-6 Glyceryl Monostearate 123-94-4 Propylene Glycol 57-55-6 White petrolatum 8009-03-8 Mineral Oil 8042-47-5 Xanthan Gum 11138-66-2 Water 7732-18-5 Section 4: First-Aid Measures Eye: Eye irritation. Flush immediately with large amount of water. Eyelids should be held away from the eye ball to ensure through rinsing. Get immediate medical attention. Skin: Itching or burning of skin. Immediately flush with plenty of soap and water. Get medical attention. Inhalation: Remove to well ventilated area with fresh air. Seek immediate medical attention. Ingestion: Get immediate medical attention and contact poison control agency. Person developing hypersensitivity reactions should receive medical attention. Section 5: Fire-Fighting Measures Suitable Extinguishing media: Use dry chemicals, foam or carbon dioxide to extinguish fire. Water can be used to dilute spills and to flush them away from source of ignition. Fire fighting procedures: Do not flush down sewers or any other drainage systems. Exposed fire fighters must wear NIOSH-approved positive pressure self-contained breathing apparatus with full face

mask and full protective clothing. Unusual fire and Explosive Hazards: Dangerous when exposed to heat and flame. Vapors may concentrate in confined area. Combustion products: Irritating or toxic substances may be emitted upon decomposition. Thermal decomposition products may include oxides of carbon. Section 6: Accidental Release Measures Keep unnecessary people away, isolate hazard area and prevent entry. Stay upwind, keep out of low areas. Vapor protective clothing should be worn for spills and leaks. Shut off ignition sources, no flares, smoking or flame in hazard area. Small spills: take up with sand or noncombustible absorbent material and place into containers for disposal. The recommendation for protective equipment are made for handling bulk materials or emergencies. Under normal conditions, use of the finished product does not require any special ventilation, respirator or other protective equipment provided the directions for use are followed. Section 7: Handling and Storage Handling: Do not get in eyes. Keep container closed. Use good personal hygiene practices. Wash hands before eating or drinking. Remove contaminated clothing and clean before re-use. Keep away from heat and flame. Keep operating temperatures below ignition temperatures at all times. Use non-sparking tools. Storage: Store in tightly closed containers in cool, dry, well ventilated area away from heat and sources of ignition. Ground lines and equipment used during transfer to reduce the possibility of static spark-initiated fire or explosion. Store at ambient or lower temperature. Store out of direct sunlight. Protect against physical damage. Exposure limits: Not Established. Carcinogen Status: None Section 8: Exposure Controls/Personal Protection Engineering Control: Local exhaust ventilation may be necessary to control air contaminants. Use of local ventilation is recommended to control emission near the sources. Personal Protective Equipment (PPE): Mask with gas vapor filter is recommended. Rubber boots and Gloves. Eye Protection: Wear chemical safety goggles and face shield. Have eye-wash stations available

where eye contact can occur. Skin Protection: Avoid excessive large skin contact. Wear gloves. Additional protection may be necessary to prevent skin contact including use of apron, face shield, boots or full body protection. A safety shower should be located in the work area. Respiratory Protection: NIOSH approved respiratory protection should be worn. A NIOSH approved respirator for organic vapors is generally acceptable. Engineering controls are the preferred means for chemical exposures. Section 9: Physical and Chemical Properties Boiling Point: Not established Solubility in water: Soluble in Cold water and Soluble in Ethanol Specific Gravity: 1.13 ph 7.0 9.5 Molecular Formula: Not Applicable Odor/ Appearance: White to off white cream with mild but characteristic odor ammonia. Section 10: Stability and Reactivity Stability: Thermally stable at typical use temperature. Hazardous reactions/decomposition products: Thermal decomposition products may include oxides of carbon. Incompatible Material: strong oxidizing agent, alkaline metals, Heat. Section 11: Toxicological Information Signs and symptoms of Overexposure: Eye and nasal irritation, dizziness, vomiting, itching or burning of the skin. Individuals who have had allergic reactions to products containing white petrolatum may experience allergic reactions to this product. This product is not classified in Hazard category. Section 12: Ecological Information* (non-mandatory)
