This document contains training schedule information for ALL girls and boys, payment and forms information, and location of training facilities. Required forms and indoor fee must be mailed in together to: MSYSA/ODP, 9401 General Dr Suite 120, Plymouth, MI 48170. Make your check payable to: MSYSA/ODP. In order for players to participate, payment and forms must reach us by the deadline of October 19th. We will not be collecting these items on site. Bring both tryout t-shirts to each session, water and shin guards and soccer shoes and BALL. Coaches will be taking attendance at all sessions, if you can t make a session please e-mail your coach. Forms and payment in full will be required immediately if the supplemental tryout player is asked to continue. I do not need this in advance of the injury tryout.
