1. IMPORTANT NOTICE READ EQUIPMENT LABELS AND USER INSTRUCTIONS BEFORE USING THIS PRODUCT. BEFORE USING THIS EQUIPMENT IT IS ESSENTIAL THAT USERS ARE FULLY AWARE OF HOW THE EQUIPMENT OPERATES AND WHERE IT SHOULD BE USED. THERE SHOULD BE A PREDETERMINED ACTION PLAN FOR BOTH THE USE AND RECOVERY, SHOULD AN EMERGENCY ARISE. This product has been developed to enable the safe entry of personnel into a confined space or to an area of restricted access. Recommended use is in conjunction with the G-Davit, G-Winch for hoisting personnel and the G-Saver II fall arrest and retrieval device, to protect against falls occurring during use. The G-Saver II also provides a backup retrieval system for use in an emergency or an unforeseen occurrence. This equipment is for use where other means of access are not practical. When in use, please observe the Maximum Working Load of the G-Davit and of all associated equipment being used. The person on the rope end of the davit should never be left to work alone. There must always be another person present to supervise and operate the G-Davit and the equipment used with it. Both the person on the rope end of the davit and the equipment operator should be adequately trained and competent. PRIOR TO USE: A formal Risk Assessment should be carried out, considering use and the emergency rescue procedure. An emergency contingency plan must be in place to rescue the worker in the event of an emergency, before entering the confined space or area of restricted access to carry out work. Users should be fully conversant with the operation of the equipment. The Before Use Checks should be performed (see section 4 Inspection Before Use). The user should verify that the maximum weight to be lifted is within the maximum working load of all equipment to be used. Date 16.11.2015, issue 001 1
2. FLOOR MOUNT SOCKET The Floor Mount Socket has been designed and tested to allow the mounting of a Davit Arm with a maximum reach of 0.8m. The G-Davit is a lightweight transportable aluminium davit arm that is ideally suited for use with the floor mount socket. Once the Floor Mount Socket is installed, the G-Davit can be located into the socket for use. The Davit arm can then be rotated in the socket and extended for the optimal work position. When work has finished, the Davit can be lifted out of the socket and removed from site. It can be used as a fall arrest anchorage device and for the regular raising / lowering of personnel. For example: lifting personnel through areas of restricted access or for confined space entry. The following davit models will fit directly into the Floor Mount Socket: Davit Model Reach Working Height D150/80 0.4-0.8m 2.0-2.4m D150/60 0.28-0.6m 1.94-2.27m Date 16.11.2015, issue 001 2

3. SOCKET INSTALLATION The socket is attached to the base material with M16 bolts. The pull out strength requirement for each bolt is 7,120 N/m minimum. The installation bolts, any fastenings and the substrate to which the floor mount socket is fitted must be capable of supporting this pull out strength requirement. If necessary seek the advice of an appropriately experienced engineer a) Floor Material & Structure The floor mount socket can be installed on a high strength concrete base. Fresh concrete should be left for the correct amount of time before installation and use. Always check with the manufacturer's specifications and guidelines for the correct length of time for fully curing. Where there is any doubt that the base material will support the floor mounted socket we recommend that you consult with an appropriately experienced engineer. b) Socket Installation & Testing Installation should be carried out by a suitable trained or experienced installation engineer. Once installed and where possible, a proof load test of 612kg (+ or 20kg) is recommended. The installed assembly should sustain the force for a minimum duration of 30 seconds, without failure, permanent deflection, any signs of cracking or any signs of failure to the surrounding structure. Ideally this should form the basis of the installation certification, which should be kept by the user for future reference. Further information and assistance may be obtained from the manufacturer or the equipment supplier. Date 16.11.2015, issue 001 4
4. INSPECTION BEFORE USE Before each occasion of use, the Floor Clamp Socket should be visually inspected and checked according to the following instructions. This should be done by someone who is competent to do so. Inspection 1. Before assembly with the davit, visually inspect the general structure of the Floor Clamp Socket. Pay particular attention to all welds, the socket s four webs, the base plate and tubular socket body. Ensure no cracks or damage. 2. Check that all threaded fasteners on the socket are present and secure. 3. Visually check that the sleeves inside the socket are present and free of significant damage. Ensure that they are free of grit and dirt that will score the G-Davit s mast. Clean if necessary. 4. Ensure that the socket cap is present and retains the plastic sleeves. 5. Ensure that there is not warping or bending or warping of the base place and that the M16 securing bolts are present, secure and undamaged. Before fitting the davit mast, the socket end can be cleaned, if dirty, in order to help prevent grit and abrasive particles becoming embedded into the socket sleeve, which can cause premature wear and damage to the end of the davit mast. Before use, inspect the associated equipment, be clear on how it operates. Lifting of people: 200kg Lifting of equipment: 250kg Date 16.11.2015, issue 001 5
5. CONSIDERATIONS FOR USE Do not exceed the maximum working load of the Floor Clamp Socket. Should any doubt arise about the safe condition of this device DO NOT USE. Return to Globestock or a Globestock-authorised service agent for assessment, service and re-certification, before further use. The G-Davit weighs approximately 17kg. Locating the G-Davit into the socket should ideally be performed by two people. This equipment must be used by at least two people; one to operate the equipment and the other on the rope end. The equipment operator should maintain communication at all times with the person entering/exiting the confined space or edge. The G-Davit and associated equipment may only be used by persons who are trained and competent in its safe use. Personnel protective equipment should be worn when using this equipment. For example: Gloves, Safety Glasses, safety shoes, etc. Consideration should be given to operator restraint, edge barriers or other methods to reduce the risk of falling. Before work commences, a clearly understood, predetermined action plan should be in place and then implemented in the event of an emergency. Be aware of any medical condition that may affect the safety of the equipment-user in both normal and emergency use. If there is any doubt seek medical advice or do not use the equipment Date 16.11.2015, issue 001 6
6. SETUP If in any doubt please contact either your retailer, Globestock or a specialist for advice. Pre-use checks for all equipment must be completed. Where possible, the equipment should be inspected and setup in a safe area. Once completed the equipment can then be moved to the area of work and assembled ready for use. Two people may be needed to lift heavy or bulky items. Perform a final inspection of the assembled equipment before use. Setting-up the Equipment for Use 1. Hold the G-Davit, standing on the side furthest from the edge. Lay the base of the Davit on the top of the socket. 2. The G-Davit weighs around 17 kg, so it may be necessary for two people to raise the G-Davit into place. 3. Raise the davit from horizontal to vertical, keeping the base end in the top of the socket. When the main upright (mast) is vertical, lower the davit into the socket. 4. Setting up the equipment on the G-Davit should always be performed with the Davit between the operator and the edge/confined space entrance. The davit arm can be rotated to gain safe access to the top pulley and upper arm (boom). 5. Attach the G-Saver II / G-Winch to the rear of the G-Davit, as shown in fig. 2. 6. Setting up the equipment at an edge exposes the installer to the risk of a fall. Appropriate care and measures should be implemented to ensure safety. Date 16.11.2015, issue 001 7

7. INSPECTION & MAINTENANCE The Floor Mount Socket requires regular inspection and maintenance to ensure that it is safe to use. Repairs should never be carried out on site or in the field. Do not tamper with or modify the unit. 7.1 Periodic Examination (this satisfies the requirements of LOLER). The Floor Mount Socket should be periodically examined by a competent person at least once in every 6 months. The Examination Record Card, found on the page opposite in this booklet, identifies the main examination criteria. On passing this detailed inspection the record card can be completed, signed off and the socket returned to use. On failing examination, the socket must be removed from use. The Socket must then be sent to Globestock or a globestock approved service agent for a further examination, service, test and re-certification. 7.2 Cleaning, Storage and Transportation The Floor Mount Socket may be cleaned using warm water with a mild detergent. The socket is best stored in a clean, dry, chemical-free environment. When transporting the socket, it should be boxed or retained to prevent damage or deterioration. Date 16.11.2015, issue 001 9
