Life Sports 2 Beginner & Intermediate Zone For those who alpine ski or snowboard conservatively at slower speeds, on green and easy blue slopes, Instructors will help the students concentrate on developing speed control and explore tactics for skiing and riding on steeper slopes. Nordic skiers at this level ski comfortably on flats, but not on hills. Life Sports 3 Intermediate & Advanced Zone At a minimum, participants must be able to ski or snowboard comfortably on blue terrain at moderate speeds and can turn both directions. Nordic students must be moderate skiers, able to maneuver up and down hills while staying in control. Intermediate and advanced classes will cover various topics depending on ability level. Topics may include breakthrough to parallel, terrain park sessions, advanced carving, and introduction to racing. I agree to RELEASE, HOLD HARMLESS and INDEMNIFY Bogus Basin Recreational Association, Inc., and its/their employees, agents, contractors, officers, or owners from all claims for any cause, including negligence, which arises out of participation in or travel to and from Bogus Basin Recreational Association, Inc. Ski and Snowboard School. This release is binding as to any other persons, including family members, heirs and executors. If I am signing on behalf of a minor, I accept full responsibility for all medical expenses incurred as a result of the minors participation in or travel to and from Bogus Basin Recreational Association, Inc. Ski and Snowboard School. I also agree to HOLD HARMLESS and INDEMNIFY Bogus Basin Recreational Association, Inc. Ski and Snowboard School and Bogus Basin Recreation Area for any claims brought by or on behalf of the minor. Acknowledgement of Personal Information and Equipment Instructions I have accurately represented the above listed information and it is true and correct. I will not use any of this equipment to be provided to me during this transaction until I have received instruction on its use and I fully understand its use and function. I agree to verify that the visual indicator settings to be recorded on this form for downhill ski equipment agree with the number appearing in the visual indicator windows of the equipment to be listed on this form. Equipment Lease and Liability Release Agreement I understand that the ski/boot/binding system cannot guarantee the user s safety. In downhill skiing, the binding system will not release at all times or under all circumstances where release may prevent injury or death, nor is it possible to predict every situation in which it will release. In snowboarding, cross-country skiing, and Snowblade use, the binding system will NOT ordinarily release during use; these bindings are not designed to release as a result of forces generated during ordinary operation. I understand and agree, if a helmet is rented, that a helmet IS IN NO WAY A GUARANTEE OF SAFETY and that no helmet can protect the wearer against all foreseeable impacts to the head or body, and that the sports of skiing, snowboarding, Snowblade, and other recreational activities can expose the user to forces that exceed the limits of protection provided by the helmet. I also understand that the helmet cannot guard against injury to the neck, spine or any other part of my body, and that these limitations are INHERENT RISKS of any activity in which a helmet may be used. I understand that the sports of skiing, snowboarding, Snowblade, and other recreational activities involve inherent and other risks of INJURY and DEATH. I voluntarily agree to expressly assume all risks of injury or death that may result from skiing, snowboarding, and Snowblade use, or which relate in any way to the use of this equipment. I AGREE TO RELEASE the ski/snowboard shop, its employees, owners, affiliates, agents, officers, directors and the manufacturers and distributors of this equipment ( collectively PROVIDERS ), from all liability for injury, death, property loss and damage which results from the equipment user s participation in the sport of skiing/snowboarding/snowblade use, or is in any way related to the use of this equipment, including all liability which results from the NEGLIGENCE of PROVIDERS, or any other person or cause. I further agree to defend and indemnify PROVIDERS for any loss or damage, including any that results from claims or lawsuits for personal injury, death, and property loss and damage related in any way to the use of this equipment. This agreement is governed by the applicable law of this state or province. If any provision of this agreement is determined to be unenforceable, all other provisions shall be given full force and effect. THIS DOCUMENT IS A LEGALLY BINDING CONTRACT which supersedes any other agreements or representations by or between the parties. It shall be interpreted to provide as broad and inclusive a release of liability as is legally possible, but is not intended to assert any claims or defenses which are prohibited by law. If any part of this agreement is deemed void or unenforceable, the remainder shall be given full force and effect, The specific rights of the parties under this contract may vary from state to state. CLOTHING CHECK OFF LIST Staying warm, dry and comfortable will make your skiing or snowboarding experience much more enjoyable. How to do it? Dress in multiple, lightweight layers Layer 1: Closest to the skin BIG HINT: No cotton! Cotton retain moisture, causing your body to chill Long johns made from synthetic fibers and materials that wick moisture away from the skin keep you warmer and dryer Layer 2: Middle layer for warmth Fleece is a great insulating fabric, as are many synthetic fibers Choose pants, shirts, and sweaters that wick moisture and breathe to help you stay dry and warm Layer 3: Outer layer to stay dry Coats and snow pants that are water-resistant or water-repellant Warm, insulated, water repellent snow gloves/mittens will keep your hands warmer and dryer Fleece hat, wool beanie, or a ski/snowboard helmet Neck gator or scarf to cover your chin and nose Extra s: Goggles, sunglasses Sunscreen and lip balm Wrist guards for snowboarders Dry clothes for the car ride home Equipment: Black Diamond Sports at Bogus Basin discounts ski/board rental prices for School Night and Lifesports programs take advantage of the safe, newer equipment rather than borrowing or using old or ill-fitting gear If you borrow or buy used equipment, have an expert at a local ski shop look it over and properly adjust the bindings before hitting the slopes Courtesy Professional Ski Instructors of America

LIFE SPORTS PROGRAM GUIDELINES Questions? Contact us at 332-5137 or 332-5340 The following guidelines are very important to the success of the SCHOOL SKI/SNOWBOARD program. Please take time to read and understand this information. Students will not be allowed to register for this program the day of the trip! Any un-registered students that come up with their school will be subject to normal retail prices. Program rates are offered to 3rd -12th grade students only (ages 7-18). This program is not designed for adults, or young children. The rate for the packages that we offer is for a 5 hour duration period. All student participants are required to take a lesson. All participants must have a complete registration form including a signature from a parent or guardian before they will be allowed in the program. All rental information must be complete on the registration form to reserve equipment. Make sure checks are made out to the School/ Organization not to Bogus Basin. Registration. All registration is to be handled by the organization's program coordinator. No late registrations will be accepted! Student registration information needs to be registered online 7 days prior to the trip. Payment. Ski School staff will take your payment of a single school check when you check in on the mountain. Groups that wish to be billed after their visit will need to make prior arrangements with our school coordinator. The program fees are nonrefundable; therefore, any discrepancies in the bill (absent or additional students) should be taken care of prior to leaving the Ski School office. No pre-payments please. Chaperons. ONE CHAPERON PER 10-12 ELEMENTARY STUDENTS OR ONE CHAPERON PER 17-20 JR./MIDDLE & HIGH SCHOOL STUDENTS. The chaperon should be an employee of or appointed by your facility to help students before and after class and must be 18 these privileges are given to chaperons only and are non-transferable. Chaperon duties include; supervising students while riding the buses to and from the Bogus Basin, assisting students through the rental shop, and helping with any emergencies that may occur while the students are in lessons. The chaperons will be allowed to enjoy complementary rentals and lift tickets for the night. Chaperons may shadow lessons, but must first receive a chaperon pass from the life sport coordinator. In order for us to have the appropriate staff available, arrangements for students with special needs should be made prior to your visit. Indicate which students participating have special needs and explain. Arrangements can be made for chaperones specifically accompanying a student with special needs. Groups should arrive at Bogus Basin at least 30 minutes prior to the class start time. Date must be pre-scheduled. Contact Bogus Basin Group Sales at (208) 332-5137

LIFE SPORTS COORDINATOR The Life Sports Coordinator organizes the School Ski Visit. Duties include: 1. Promoting the program by flyer, announcements, etc. 2. Distribution of the registration forms 3. Collection of the forms and fees 4. Providing a list of participants with all information requested (print or type) 5. Appointing chaperons and briefing them on their responsibilities for the School visit 6. Assisting students throughout the day in: loading the bus, equipment rentals, lesson meeting areas, parent pick-up at school at end of day. LIFE SPORTS PROGRAM PROCEDURES The following procedures are designed to ensure a smooth running program for everyone. We appreciate your cooperation and look forward to seeing you and your students at Bogus Basin. PRE-TRIP: 1. RESERVE A DATE & TIME -Call Bogus Basin Group Sales at (208) 332-5137 -Please approximate the number of students (allowable numbers may be limited) -Please call 10 days in advance if you must reschedule or cancel your scheduled ski date 2. DISTRIBUTE REGISTRATION/RELEASE FORMS -Registration/release forms can be downloaded from the school registration website http://schools.bogusbasin.org -Every student must have a completed & signed form to participate -***Program fees are NON-REFUNDABLE 3. COLLECT MONEY & REGISTRATION FORMS: -ALL FORMS MUST BE COMPLETE - Check forms as students turn them in with payment. -Make sure all checks are made out to specific School/ Organization NOT Bogus Basin. -Parent or guardian of the student must sign forms -Arrange to collect several days in advance (all registration information must be entered online 7 days prior. 4. ENTER STUDENT REGISTRATION INFORMATION ONLINE -Access the school registration website http://schools.bogusbasin.org -Username is teacher e-mail; Password is first three letters of last name and first three letters of first name. -Registration page: Click Edit button to enter new student information. Use the Import Student function to duplicate student/ chaperone information if you group has multiple visits. -It is important to enter all student information. Make sure to click rental button at the end of drop down menu if renting. Check the Summary page to make sure all information is correct. 5. LOCK OUTS -If the registration information is not completed 7 days prior to the trip date the website will automatically lock out your ability to register students. -Contact Group Sales with any questions (208) 332-5137 -Office hours: Monday - Friday 9am -5pm. 6. A BILL WILL BE PROVIDED TO THE ADVISOR AT THE TIME OF CHECK IN. -Ski School Sales Associates will be available to take your payment when you check in on the mountain. Groups that wish to be billed can arrange that with the Ski School Sales Associate. If paying by check please use one School/Organization check. The program fees are non-refundable; therefore, any discrepancies in the bill (absent or additional students) should be taken care of prior to leaving the Ski School office. Pre-Payments are not possible.

SCH UP ON THE MOUNTAIN: 1. BUS -Students should be dressed in their ski apparel and ready to ski when they get off the bus. Bags and backpacks should be left on the bus unless the coordinator and chaperones designate an area in the lodge for kids to leave bags and backpacks. Please do not bring bags and backpacks into Black Diamond Sports. 2. FIRST STOP BLACK DIAMOND SPORTS. -Equipment will be ready for students upon arrival. -Chaperons are required to assist in rental shop until everyone has their equipment. -Students meet with their instructors on flat area in front of Black Diamond Sports and JR Simplot Lodge. -Beginners should CARRY their skis/boards to lesson area. Instructors will show them how to put on their equipment at the beginning of class. Note: Level I skiers do NOT use or need ski poles. -Students may not exchange SKIS for SNOWBOARDS or SNOWBOARDS for SKIS! Due to liability reasons, students must keep the equipment they signed up for 3. SKI SCHOOL STOP (J.R Simplot Lodge) - Coordinator only -Check in with ski school sales staff as soon as possible upon arrival -Update and verify participation list & summary sheet -Take care of program payment. -If you have arranged for us to bill your school, please take care of any discrepancies on the bill before you sign it. -Adjustments will not be made once the bill has been signed -Advisor receives student and chaperone lift tickets 4. MEETING AREA (on flats, between rental shop & beginner area) -Meet 10 minutes prior to class start time -Classes divided up by instructors according to selected ability levels. -Lessons will start promptly at 10:00am, 12:00pm or 2:00pm. -Lesson length will extend from start time. Late arrivals may result in shortened class times. -The Life Sport Coordinator will hand out student and chaperone lift tickets. Chaperons may pick-up their equipment once the students are out of the rental shop and signed-in with their instructors.
