All physicals are good for 12 months from the date of the physical. The NCHSAA Physical Form can be downloaded from the main athletic webpage of the BMHS Website by clicking on the physical form in the drop down box and then clicking go. All physicals must be completed by a physician on the NCHSAA Physical Form.
