Oklahoma Self Insurers Conference & Exhibits Hyatt Regency Hotel Tulsa Terms of the Contract We agree to abide by the regulations affecting the 2014 Oklahoma Self Insurers Conference and Exhibition. 1. No exhibit space shall be sublet or offered for use by any exhibitor without the approval of the convention management of the Oklahoma Self Insurers Association 2. No exhibits permitted that do not have at least one person in attendance at all times. Solicitation or distribution of literature is not allowed except in the exhibitor s leased area, and only by paid vendors/ sponsors. Please respect your fellow vendors! 3. Exhibit booth personnel shall wear an identifying badge at all times on the exhibit floor. 4. It is expressly understood and agreed, and the exhibitor agrees by accepting these contract conditions, that he will make no claim of any kind against the Oklahoma Self Insurers Association, or any of their employees or members for any cause whatever prior, during, or subsequent to, or in connection with said trade show, nor for any injury that may occur to himself or his employees while in the Hyatt Regency Hotel Tulsa, nor for any damages of any nature of character whatsoever, and such exhibitors will hold Oklahoma Self Insurers Association harmless from any claim of any nature whatsoever, including any function sponsored by Oklahoma Self Insurers Association during the period of October 30th & 31st, 2014 5. This contract is subject to all conditions under which the Hyatt Regency Hotel Tulsa has made available to the Oklahoma Self Insurers Association, and the right is reserved to cancel the exhibit space assignment of any exhibitor. All points not covered herein are subject to settlement by the Oklahoma Self Insurers Association and the Association reserves the right to make such changes, amendments and additions to the rules or floor plan as shall be necessary. 6. Exhibits are to be left in place through the closing hours of the day of the Exhibition, Friday, October 31st, 2014 All exhibit material must be removed from the Exhibit Hall after the close of the show at 5:00 pm, Friday October 31st, 2014. No breakdown of booths is allowed until AFTER the completion of the last break on Friday, October 31st, 2014 7. Set up is Thursday October 30th from 1:00 pm to 5:00 pm and 6:30-8:00 am Friday October 31st, 2014 8. It is expressly understood that being an exhibitor at the Oklahoma Self Insurers Exposition does not guarantee product/service exclusively or endorsement on behalf of the sponsoring or managing entities. 9. All Door prizes/ give-a-ways are the sole responsibility of the Exhibitor.

The Oklahoma Self Insurers Association Annual Scramble Golf Tournament Thursday October 30th, 2014 The OSIA Annual Golf Tournament has become a terrific way to spend time with your clients and associates in the industry. This will be our 7th tournament, the first time here at The Canyons at Blackjack Ridge. Each year it continues to grow and the more fun we have! The 18-hole "Sand Springs" course at The Canyons At Blackjack Ridge facility in Sand Springs, Oklahoma features 6,113 yards of golf from the longest tees for a par of 71. The course rating is 69.5 and it has a slope rating of 125. Designed by Floyd Farley, ASGCA/(R) Randy Heckenkemper, the Sand Springs golf course opened in 1958.
