To help you ensure your registration is complete, we have provided the checklist below. MJP ACADEMY FEES AND PAYMENT POLICIES Payment must accompany this registration At MJP, you have the following options to pay for your classes. Please include one of the following: Annual fees - One cheque (or cash) for a full year tuition dated September 1 Two Payments First payment: Half a year s tuition (5 months: September - December plus June) cash or cheque (Dated for September 1). Second payment: January-May cash or cheque (Dated for January 1). Monthly Payments (We do not accept cash on a monthly basis) One cheque dated for September 1 containing combined payment for your first month and June (First and last month s payment which are non-refundable). ALL CHEQUES PAYABLE TO: JOELLE SHEPPARD WITHDRAWAL POLICY Hopefully, you will remain with the studio for the full dance year, and then again annually, thereafter. However, should you find it necessary to withdraw, you must notify the studio in writing prior to the 1st day of the month. Non-attendance is not considered proper notice. If you do not give written notice, your account will continue to be charged until formal notice has been received. REFUND POLICY First month s fees and last month s (June s fees) are non-refundable. Refunds will not be given for days missed due to absences, holidays, or weather-related closings. Credit may be given in the event of a medical emergency. Situations such as this will be considered on a case-by-case basis. NON-PAYMENT POLICY Dancers with outstanding balances will not be allowed into class. This includes failure to provide payment by the first of a given month. Dancers will not be permitted into the classroom until payment has been made for that dancer. DELAYED PAYMENT OF FEES Delayed payments will result in additional charges to your account. Any payment not received on time will be subject to a 10% surcharge. If after an additional two (2) weeks, payment has still not been made, an additional 10% fee will be attached to the new balance. Further delays will be charged at a weekly rate of 5%. NSF CHEQUES $25.00 will be charged for all NSF cheques. After two NSF cheques, the remaining balance for the year must be paid in cash following the guidelines above. CLASS CANCELATION Due to low enrollment, a class may need to be cancelled. Dancers from a cancelled class will be combined with another existing class. If a dancer cannot make any other class times, we will attempt to continue to run the class for the remainder of the existing month and fees will be refunded for the remainder of the year from the first day of the following month (excluding June fees).

M.J.P. Academy of Irish Dance WAIVER, RELEASE and HOLD HARMLESS AGREEMENT I,, (name of participant/student), the undersigned, affirm that I am participating voluntarily in dance class (s) at M.J.P. Academy of Irish Dance, the Academy. I, together with my parent or guardian, if I am under the age of eighteen (18) or under a legal disability, represent covenant and agree, on behalf of myself and my heirs, assigns and any other person claiming by, under or through me as follows: A. I acknowledge that participating in Irish Dance class (s) the Activity involves certain risks (some of which I may not fully appreciate) and that injuries, death, property damage or other harm could occur to me or others. B. I accept and voluntarily incur all risks of any injuries, damages or harm which arise during or result from my participation in the Activity. These risks include, but are not limited to, the possibility of accident or illness while traveling to or from events, as well as any injury, death or property damage arising out of participation in the physical activity involved with the Activity. M.J.P. Academy strongly recommends that each participant have an annual physical examination and carry personal health and accident insurance. C. I waive all claims against M.J.P. Academy of Irish Dance, the Trustees of M.J.P. Academy of Irish Dance and/ or its or their departments, affiliates, employees, officers, agents or insurers (Released Parties) for any injuries, damages, losses or claims, whether known or unknown, which arise during or resulting from my participation in the Activity. I release and forever discharge the Released Parties from all such claims. D. I agree to indemnify and hold the Released Parties harmless from all losses, liabilities, damages, costs or expenses (including but not limited to reasonable attorneys fees, court costs and other litigation costs and expenses) incurred by any of the Released Parties as a result of any claims or lawsuits that I (or anyone claiming by, under or through me) may bring against any of the Released Parties to recover any losses, liabilities, costs, damages or expenses which arise during or result from my participation in the Activity. E. I have carefully read and reviewed this Waiver, Release and Hold Harmless Agreement. I understand it fully and I execute it voluntarily. I recognize that from time to time, there may be pictures taken of myself or my dancer. These pictures may be posted on the Internet for viewing by other students and / or the general public. I acknowledge that I am aware of these pictures and permit their posting on the M.J.P. Academy website. M.J.P. Academy maintains a record of all registered students. I recognize that my name and my dancer s name is recorded on this list. This list is shared with the M.J.P. Irish Dance Association for the purpose of their membership recording. The list is not shared outside of M.J.P. Academy or M.J.P. Irish Dance Association. I further confirm that this email is checked regularly. I understand that email is the primary method of communication to parents and dancers about important events during the year. PAYMENT OF FEES If I withdraw from the studio, I understand that the first and last month (June) fees are non-refundable and that NSF cheques will be subject to a $25 fee. I further understand that to withdraw, I must provide written notice prior to the 1 st day of the month. If I wish to withdraw and do not give written notice, my account will continue to be charged until receipt of my formal written notice. If payment has not been received by the first of the month, dancers will not be permitted to participate in class, competition or performances until payment has been received. Late accounts are subject to the surcharges as listed in the Fees and Payment Policy contained within the handbook. INITIAL INITIAL SHOWS & PERFORMANCES Cancelling your commitment to dance in a show or performance may affect your invitation to participate in future shows. Any dancer who arrives at a show after the others have finished the rehearsal will not be permitted to dance in the show. There are no exceptions. MJP Academy of Irish Dance MANDATORY WORKSHOP REGISTRATION FORM 2018-2019 Student Name (First and last) Age Class level The following workshops are mandatory workshops where dancers learn new choreography for the year. They are for dancers in their 2 nd year and up. These dancers SHOULD NOT miss these workshops as they will fall behind in their classes. **If you can only attend one day of any mandatory workshop, the cost is $20.00 for the day. But please do your best to make it to all of the days of your workshop. MANDATORY SUMMER WORKSHOP (At Crossings Dance studio in Midnapore) The following workshop will be for 2 nd year dancers in the Dublin level up to Athlone only. This is a 5 day long workshop to teach new choreography that dancers need for the year, and to work on drills to jump start everyone back into shape following the summer break. Each dancer will attend 1.5 hours each day. The latest time is for the Championship dancers doing Treble Jig/Slip Jig. Sunday, August 26 to Thursday, August 30, 2018 (1.5 hours per day per dancer) Sunday 9:00am-1:30pm and Monday to Thursday 5:00pm 9:30pm I have included: One $75.00 cheque dated for August 1 st, 2018 (Payment details below) MANDATORY INTRO TO HARDSHOE WORKSHOP (For Limerick level dancers) At Highwood Community Center This workshop is for Limerick dancers; dancers in their 2 nd year and up that are starting hardshoe for the first time. This will be a two day long workshop, 1.5 hours each day. Sat., December 1, 11:00am-12:30pm and Sunday, December 2 nd, 5:00pm - 6:30pm I have included: One $30.00 cheque dated for November 1 st, 2018 (Payment details below) CHAMPIONSHIP CHANGEOVER WORKSHOP - At Highwood Community Center This workshop is for dancers in Preliminary championships or Open championships level in competition only. Sat., December 1, 9:00am-11:00am and Sunday, December 2nd 3:00pm-5:00pm I have included: One $40.00 cheque dated for November 1 st, 2018 (Payment details below) Please note: -Cheques are payable to "Joelle Sheppard". -There is a $25.00 charge for all NSF cheques - Students wishing to withdraw from a Mandatory workshop must give written notice prior to the 1st day of the month that workshop will be held -Once registered for a workshop, the fees are non-transferable.

MJP Academy of Irish Dance WORKSHOP REGISTRATION FORM 2018-2019 Student Name (First and last) Age Class level SKILL WORKSHOPS -These workshops are for dancers of age 7 and up, in their second year of dance or more. There will be 5 of these two-hour workshops held throughout the dance year. These workshops will teach important skills and exercises that go above and beyond what there is time to cover in regular weekly classes such as: stretching, strengthening, endurance training, self-confidence, good practice habits, how to do tricks, nutrition. Dates (All are Saturdays at Highwood from 9:00am-11:00am): December 15, January 19, April 13, May 4, May 25 To attend the skill workshops, I have included: One $100.00 cheque dated for November 1 st, 2018 OR $100.00 Cash (Payment details below) *Skill workshops can also be attended on a class by class basis for a cost of $25.00/ workshop. 3 HOUR WORKSHOPS - These workshops are for our Galway and Athlone level dancers to help them get in some extra class time before competitions. Dates: January 12, April 20, May 11 Time: 8:30am-11:30am Location: Highwood Community Center One $90.00 cheque (or cash) dated for January 1 st, 2019 OR you can drop-in for $35.00 a class INTRODUCTION TO IRISH DANCE SUMMER CAMP 2019 Ages 5 and up! Saturday, August 10, 9:00am- 12:00noon. and Sunday, August 11, 2:30pm-5:30pm Come and learn how to Irish dance with us at our two-day workshop where you will do some crafts, see our older dancers perform and best of all, learn some Irish dance! Younger MJP dancers that already know how to dance are more than welcome to come and take the workshop too! To attend the introduction workshop, I have included: One $100.00 cheque dated for July 1 st, 2019 OR $100.00 Cash (Payment details below) You do not need to register for the following workshop but be sure to add this to your calendar. ST. PATRICKS SHOW PREP WORKSHOP (for dancers in their 2 nd year of dance and up) This workshop is for anyone wanting to attend the St. Patrick s day shows this year. This will allow us to rehearse with the dancers that are attending the shows all together. These workshops will also be drop-in at a cost of $15.00 per day. 3 hours per day, per dancer. Youngest dancers will only need to attend 1 hour at the start of each day. Saturday, February 9th 2019, 9:00-12:00 and Sunday, February 10 th 9:00-12:00 at Highwood community center. Please note: -Cheques are payable to "Joelle Sheppard". -There is a $25.00 charge for all NSF cheques -There will be no refunds for missed Workshops -Once registered for a workshop, the fees are non-transferable.

MJPIDA (MJP Irish Dance Association) Membership Application Who is MJPIDA and why should I join? The MJP Irish Dance Association (MJPIDA) is MJP Academy s non-profit parent support group. All of us work together to support our dancers. Any fundraising contributes to the dancers of the MJP Academy (school). MJPIDA owns and maintains the class dresses worn by dancers at shows and in competitions. MJPIDA also subsidizes building rental cost for the school and workshops, reducing your tuition and other fees. MJPIDA sponsors a Year-End Show for all dancers. All MJP Dancers are invited to participate. The show is an Irish Dance Performance presented for parents, families, relatives, neighbours and others. Class Dress Rentals Irish Dance is not only the dance steps and timing of dance; it requires costumes for show performances and competition. MJPIDA owns and maintains class dresses that may be rented for use throughout the dance year by members. As your dancer grows, a too small dress is traded for a larger dress anytime during the year to fit your dancer. Volunteer Points The benefits of MJPIDA membership requires help with various activities throughout the year from all member parents and dancers of MJPIDA. To ensure participation by all members, a volunteer point system is in place. First year dancers are only required to earn 10 points. You can earn these points very easily by participating in just two things! Like: Attending one meeting (5 points) and dancing in the year end show (5 points). Volunteer Point System operation is simple, 1. We require a $200 deposit cheque (post-dated for June next year) at the beginning of the dance year (September). 2. Earn the number of points required by the end of the year (June) and your cheque is returned. Required volunteer points are earned per family. Number of points required during the dance year is determined by dance level of the most senior dancer in your family. Points are very easy to earn. The directory allows parents and dancers to contact others for dance shows, carpooling, I need a ride to dance, etc. Information supplied on this membership application is for the sole use of MJPIDA and will not be shared or published outside the MJPIDA.
