SATURDAY, SEPTEMBER 27, 2014 DCR HATCH SHELL The JDRF Walk to Cure Diabetes has become JDRF One Walk in Boston and other select markets. We've changed the name of our flagship fundraising event to communicate our focus on a single goal: creating a world without type 1 diabetes (T1D). The new name and bold look at this year's JDRF One Walk reflect the powerful impact you are making on our efforts to turn Type One into Type None. Thank you! BEFORE THE WALK 1. We HIGHLY encourage all Walkers to pre-register online at http://walk.jdrf.org/onewalkboston. Not only does this make Check-In go more smoothly, it allows you to get to the fun and not wait in line. 2. You can mail in gifts to the JDRF office before the Walk or drop by the office at 60 Walnut Street, Wellesley Hills, MA 02481 (we d love to see you and thank you in person!). Be sure to read the enclosed Walk Money Tips Sheet. Turning in funds earlier is another great way to ensure a speedy check-in process on Walk Day! For directions to the JDRF office, visit our website at http://newengland.jdrf.org. 3. Walk Day Bag Policy: The police will be strongly enforcing a policy where no bags can be left on the Hatch Shell while you are on the Walk route. Please be prepared by only bringing bags you can walk with (small backpacks, cross-body purses, fanny packs, etc.). WALK DAY SCHEDULE 9:30 a.m. Check-In Opens 10 a.m. Welcome Remarks 10:30 a.m. Walk Starts! 11:30 a.m. Light refreshments to be served. Entertainment to be provided by Standing Room Only! WHEN YOU ARRIVE 1. The Walk will look a little different this year. The main point of entry, the Check-In Tent, is now where the Food and Beverage Tent used to be. Please see the enclosed site map to get acquainted with our new layout. 2. If you have donations to turn in and/or have raised $100 or more to earn a Walk t-shirt, please proceed to the Check-In Tent. General Check-In: If you have pre-registered online and have Walk gifts to turn in, donations should be counted and sealed in a completed Walker Collection Envelope. An accurate total of money contained in the envelope must be clearly written on the outside of the envelope. Every Walker turning in funds should turn in their own separate envelope. It is extremely important that Collection Envelopes are filled out completely and accurately. Be sure to read the enclosed Walker Collection Envelope Instruction Sheet. Collection Envelopes will be available at the Registration Tables near the Check-In Tent. Please see enclosed instructions on how to fill out your Collection Envelope. Unregistered Walkers: We would like to collect basic information for everyone attending the JDRF One Walk. If you or members of your team have not registered, please proceed to one of our Registration Tables to complete a Walker Collection Envelope. If you are turning in donations, please clearly write on the outside of the envelope the total amount enclosed and proceed to Check-In. If you have no money to turn in, please hand your completed envelope to a volunteer greeter. Express Check-In: We will have a few stations open in the Express Check-In area for those Walkers and Team Captains who have pre-registered, raised all of their funds online, and don t have donations to turn in on Walk Day. If you raised $100 or more you can pick up your t-shirt ticket here.

V1P Check-In: If you have raised more than $1,000, you are a V1P and there will be a designated Check-In line just for you. This is our way of making Walk Day a little bit easier for our top fundraisers! You will also pick up your V1P lanyard here for exclusive access to our V1P Tent. Get a T-shirt Ticket at Check-In: Did you personally raise at least $100 for JDRF? Thank you for your support! Be sure to get a white t-shirt ticket at Check-In and pick up your t-shirt at the T- Shirt Tent. NEW THIS YEAR T-Shirts for our T1D Walkers: JDRF is motivated by the millions of children and adults challenged by T1D every single day. We celebrate your strength and the sense of hope and empowerment that Walk Day brings. Let our volunteers know if you would like a special blue t- shirt identifying you as living with T1D and they will provide you with a blue ticket during Check-In. You can then pick up your T1D t-shirt at the T-Shirt Tent. Please note: These t-shirts are complimentary and are available on a first-come, first-served basis. FOOD AND REFRESHMENTS While we provide as many refreshments as possible, thanks to our in-kind sponsors, please understand that we have a limited quantity of food and beverage available and we are not able to meet all dietary needs. We ask that you please come prepared in order to enjoy the activities of the day. Food and beverage options include: apples, Stacy s Pretzel Thins, Kayem hot dogs, ENLIGHTENED ice cream bars, Dasani water, Coca-Cola products, Hint Water, and Polar Seltzer. We will also be serving DAVIDsTEA and clam chowder in our V1P Tent. Please be sure to thank our Water Stop #2 Partners, Auto Body Clinic/Modern Auto Glass, as well as the FOX25 team who will be joining us again at Water Stop #3. There is an additional option for purchasing refreshments at the Charles River Bistro on the Esplanade, just off the banks of the Charles River lagoon. WALK ROUTE Due to bridge construction, our Walk Route will once again be limited to the Boston side of the Charles River Esplanade. We will be offering a shorter Family Friendly route option this year. Please see the enclosed route map for details and remember to follow the JDRF signs. Volunteers will also be in place to help you navigate the Walk route. ENTERTAINMENT/ACTIVITIES Sponsor and Vendor Booths will be open before and after the Walk. This year we ll be joined by fantastic partners including Medtronic, Lilly Diabetes, Novo Nordisk, Insulet Corporation, Animas Corporation, Nova Diabetes Care, Sanofi Diabetes, and Tandem Diabetes Care. The Activities Tent is the place to be Stop by in the morning to get a temporary tattoo and your child s ID bracelet. Take a photo complete with fun props, and then post it on your favorite social media site using #JDRFOneWalk #Boston. You can also decorate an N and place it on the wall Turning Type One into Type None. Visit our New Family Welcome and Advocacy Tent to learn more about JDRF, our research efforts, and how you can become involved locally. *NEW THIS YEAR: If this is your first Walk, be sure to pick up your New Family Bandana here and enter our drawing to win Boston Celtics tickets! For our V1Ps, you and a guest are invited to the V1P Tent to meet our local volunteer leadership and have a conversation with Julia Greenstein, Ph.D., JDRF s Vice President, Discovery Research. Walk Day Entertainment will be provided by AMP 103.3, drummers from The Movement @ Berklee, and local cover band Standing Room Only. PRIZE INFORMATION To thank you for your commitment and hard work, we offer some great prizes as rewards for raising more than $200. You will receive an email with more details within 10 weeks post event. Note: Want even more of your money raised to go to T1D research? Log into your Walker Participant Center, under Edit Profile select I do not want a prize and 100% of your donation will go to JDRF.

Waiver Signature A B C IMPORTANT: Information pertaining to cash/check Walk donations! 1. Cash/check donations that have not been received by JDRF prior to Walk Day but have been entered into your Participant Center will temporarily disappear from your Participant Center and fundraising piggy bank on Monday, September 29 to avoid us counting them twice. 2. Immediately following the Walk we ll begin entering all money turned in on Walk Day and expect to be finished within four weeks. Once complete, the donations you turned in on Walk Day will appear on your fundraising page as confirmed donations.

1) Don t hold onto your Walk donations until Walk Day! Turn in your Walk donations as soon as you receive them: 1. Drop them off at the JDRF office (60 Walnut Street, Wellesley Hills, MA 02481). We are open Monday through Friday from 9 a.m. 5 p.m. 2. Send them to the office via mail, FedEx, DHL, UPS, etc. (please don t mail cash). 2) Write the Team name and Walker name on each check. Write the Team name and Walker name on each check and/or money order you turn in. This helps ensure all Walk donations are posted to the correct Team and Walker! 3) Don t turn in cash. Convert your cash donations into a check or money order before turning it in. Checks and money orders are more secure than cash. Sometimes children want to turn in the actual cash they worked so hard to raise. Consider taking them to the bank with the cash and converting it into a money order so they can see where their cash is going. This way they can still feel proud of their fundraising accomplishments while ensuring their gift is received safely by the office. 4) Be sure personal checks/money Orders/Bank Checks/Treasurer s Checks have the donor s name and address on it. Some personal checks list only the name of the donor. In this case if you know the address of the donor please write it on the check. Not all money orders/bank checks/treasurer s checks include the name and address of the person purchasing it. Be sure to write the name and address of the person who purchased the money order on the money order in addition to the Team name and Walker name. We are required by the IRS to send an acknowledgement letter for gifts of $250 and above; we need the donor s address to do so. For checks under $250, their cancelled check is their receipt. If the donor address is absent, the acknowledgement letter will be sent in care of the Walker so you may forward it on to the donor.

2014 JDRF ONE WALK BOSTON SATURDAY, SEPTEMBER 27, 2014 We strongly encourage all Walkers to use public transportation on Walk Day. Take the Red line to the Charles/MGH station: Walk over the footbridge to Storrow Drive and proceed to the Hatch Shell. Take the Green line to the Arlington station: Take a left onto Arlington Street, a left onto Beacon Street, turn right onto David E. Mugar Way, and cross over Storrow Drive using the Arthur Fiedler Footbridge. There is no parking available at the DCR Hatch Shell. Should you choose to drive, please be prepared to park at one of the suggested nearby parking garages. Boston Common Garage: Located underneath the Boston Common, the entrance is located on Charles Street directly across from the Public Garden. To walk to the Hatch Shell from the garage, turn right onto Charles Street, turn left onto Beacon Street, turn right onto David E. Mugar Way, and cross over Storrow Drive using the Arthur Fiedler Footbridge. From 93 North: Take 93 N to Exit 22. At Exit 22, turn left onto ramp (Chinatown/South Station). Bear left onto Surface Road (Surface Artery), turn right onto Kneeland Street. This road becomes Stuart Street. Turn right onto Charles Street. The garage entrance will be on your right. From 93 South: Take 93 S to Storrow Drive exit and go west on Storrow Drive. Get in the left lane and take the Back Bay/Copley Square exit (this is a left exit). Take a left at the light and take your first right onto Arlington Street. Proceed through set of lights. At second light, take a left onto Boylston Street. Go straight through lights and at second light, take a left onto Charles Street. The garage entrance will be on your right. From the Mass Pike: Take the Mass Pike Eastbound to the Prudential exit. Stay right for Copley Place. Continue on Stuart Street and take a left onto Charles Street. The garage entrance will be on your right. 2014 JDRF ONE WALK BOSTON SATURDAY, SEPTEMBER 27, 2014 LAZ Storrow Drive Lot and Charles Street Lot: Formerly the private Mass Eye and Ear parking lots, these lots are public on nights and weekends. Parking costs $10 per day. To walk to the Hatch Shell from the lots, take a left onto Charles Street, and then follow the signs for the Charles River Esplanade which will take you along Storrow Drive to the Hatch Shell. From 93 North: Take 93 N to Exit 26 Storrow Drive. Keep right at the fork and continue following signs for MA-3 N/MA-28 S/Storrow Drive and merge onto MA-3 N. Take the MA-3 N exit toward Government Center/Kendall Square. Storrow Drive Lot (NON Handicap Accessible): Stay in the left lane on Storrow Drive (West). Entrance to parking lot is immediately before approaching Government Center/Kendall Square Exit ramp. Charles Street Lot (Handicap Accessible): Proceed through the lights under (T) station; take a hard left reversing direction under the (T) station. Parking lot is located off the ramp to Storrow Drive (West). From 93 South: Take 93 S to Exit 26 (MA-3 N/MA-28 S/Storrow Drive). Merge onto MA-3 N and take the MA-3 N exit toward Government Center/Kendall Square. Storrow Drive Lot (NON Handicap Accessible): Stay in the left lane on Storrow Drive (West). Entrance to parking lot is immediately before approaching Government Center/Kendall Square Exit ramp. Take Storrow Drive (East) toward downtown Boston. Take Government Center/Kendall Square exit. Storrow Drive Lot (NON Handicap Accessible): Stay in the left lane on Storrow Drive (West). Entrance to parking lot is immediately before approaching Government Center/Kendall Square Exit ramp. Charles Street Lot (Handicap Accessible): Proceed through the lights under (T) station; take a hard left reversing direction under the (T) station. In order to promote a safe and fun event designed to raise funds for JDRF and comply with the state s requirements for our use of this public space, the New England Chapter has developed the following policy. Walk Day Team Placement Walk Team Tents will be awarded to the Top 10 Fundraising Teams. Team Tents are to be used as a meeting or gathering place for Walk Teams. Team Tents can be decorated and be used to serve food to Team Walkers. Team Tents will be clearly marked on the enclosed site map that will be sent to all Team Captains and will be on the JDRF website (http://newengland.jdrf.org). Team Captains are responsible for turning in all donations at the Check-In Tent before they start walking. Team Tents will not be guarded while Walkers are on the route. No items are to be left at or inside Team Tents. General Policies Without specific approval of the New England Chapter office, JDRF Walkers and other attendees of the event cannot: Solicit additional funds for Walk Teams through the sale of items, such as bracelets, etc. Distribute information from a diabetes or other corporate vendor. No independent vendors or clubs can utilize Team Tents or Tables for soliciting purposes. Collect names, emails or mailing information from any JDRF Walkers outside of collecting funds for the Walk. Drive on the Hatch Shell circle during Walk Day. Make use of tents/tables that haven t been designated as your Team s Tent/Table. Team Captains are responsible for the actions of their Teams.
