GU ALLSTARS INC. Welcome to the home of the GU Allstar Cheer program. We are delighted that you have chosen to become a part of our family. Our mission is to enrich the lives of the children and their families through the sport of competitive cheerleading and tumble. Our goal each day is to teach our athletes the value of commitment, hard work, integrity, leadership, self-confidence, positive attitude and a love for the sport. It is our goal to be the very best Allstar Cheer Program. Each and every family is important to us and our program. We invite you to come and see what it is like to be One of a Kind! After you have read and completely understand then please fill out the following page: Financial Policies & Procedures, Commitment Letter NEW ATHLETE S -Please be prepared to turn in a photocopy of your athlete s birth certificate and small photo of your child on the tryout date of May 20th, 2017. GU Allstars Informational Packet Tryouts-May 20 th -Closed to parents May 20 th with the exceptions of ages 6 and under (they have to be level 1). The athletes will be evaluated & placed per Levels 1, 2 and 3-4. We will not place teams until end of July to give athletes a chance to gain extra skills to move up a level. Please stay in the parking lot it will not take that long. June and July Mandatory Level Conditioning: $60 All Teams We will have mandatory LEVEL conditioning June and July. It will consist of conditioning, stunting and tumbling once a week. Your child can miss up to one practice per Month. We understand summer vacations, so please let us know if you have any concerns or scheduled vacation. Conditioning Schedule will be emailed out by May 26 th. Please just let me know about plans via email guallstars@yahoo.com. Team Choreography: This season we will have professional Choreographers from National World Winning Gyms coming in the gym to do our Choreography for selected teams. This Choreography will be last week in July / First week in August. Choreography is Mandatory for teams. Schedule out June 1 st. 2 Days a week/weekend and Saturday Practices: Start in August Teams will have Saturday practices ONCE a month starting in August- Schedule is attached. These are Mandatory. Schedule out June 1 st. Attendance Policy Attendance is VITAL to team success. Below is a reminder of the GUA attendance policy which has been enforced since August.

Excused Absences School event (need to know 2 weeks in advanced will be excused) Contagious Illness-Doctor s Note Funeral/Wedding/Family Emergencies **No practice can be missed two weeks prior to any competition. ** Consistent attendance violations Two or more unexcused absences will require a meeting and could lead to dismissal from the program. ** A No call or No Show could/will be grounds for automatic dismissal** Tardiness Please do not be late to any practices and be on time picking kids up from practice. We will charge a fee if you are continuously over 10 mins late picking up. If there is an accident or an emergency please let the team manager know via text that you will be running late. We understand Jacksonville traffic is a nightmare at times. * If it becomes a consistent being late then it will not be tolerated and could result in dismissal if not resolved.* New Parents - Parking Please do not park outside the front doors or park to drop off your athlete ANYTIME at either front door. Closed Practices Practices will be closed until we get use to the athletes. I will open practices when the teams are ready to show. This way we have NO parent coaches on the sidelines.

GU ALLSTARS, 2017 2018 Financial Policies & Procedures In addition to the time commitment and hard work, there is a financial obligation to being part of the GU Allstars program. Please read and carefully review all of the following policies and procedure regarding the 2017-2018 competitive season. At the time the athlete is selected for a team, the following fees will apply: Monthly Tuition TEAMS FEE DUE ALL TEAM $60 JUNE AND JULY TINY PREP $65 AUG THRU APRIL TINY TEAM $65 AUG THRU APRIL AGES 7 TO 8 $85 AUG THRU APRIL AGES 9 & OVER $95 AUG THRU APRIL *Sibling discounts are a based on per family basis and discussed one on one* This fee is determined by the athlete s age as of 08/31/2017. The Monthly Tuition Cost overs the athlete s i stru tio ti e two nights a week practice which includes one hour tumbling and Saturday practices. 2 ½ Hours one Night and 2 hours next night. There will be a Set Fee schedule **Your athlete has to have a parent, guardian or designated chaperone at all 2 day events. ** * Monthly Tuition is due for the entire 9 month period (Aug 2017- April 2018). Tuition does not fluctuate based on the number of practices in any month or canceled competitions. We have an online accounting/payment system Quick books where you will receive monthly statements, invoices, & pay online option.

* All payments are due on the 7 th of each month. Payments received after the 7th day of the month will incur an addition $25.00 late fee. After the next practice if payment is still not paid your child will be removed from practice. *unless the 7 th falls on a weekend* * Athletes who compete on multiple teams "crossovers" will be responsible for any additional entry fees they incur beyond their first team per competition pricing. * Any payment attempts resulting in a NSF (non-sufficient funds), declined credit card, expired credit card, returned check, etc. will incur an additional $25.00 fee. Will not have the option to use a check going forward. *A athlete s a ou t ust e urre t a d i good sta di g to parti ipate i practices/competitions/special events. We will not distribute any paid for items if you have a balance on your account. GU Allstars reserves the right to remove any athlete from their team at any time for failure to keep up with financial obligations. GUA has a NO REFUND Policy on tuition, uniforms, practice uniforms, bows or etc. *Contract commitment letter must be signed by June 7th. There are no refunds of tuition given for any unused portion of a month, any amounts or anything. If you quit before the uniforms are in they will be property of GUA. They will not be distributed to you even if they are paid for. GUA has a NO REFUND Policy on tuition, uniforms, practice uniforms, bows or etc. * Any monies received from an athlete/parent will be applied first to any overdue tuition/fees. All tuition/fees must be current before an athlete may collect any clothing/uniform or other retail items. *GUA STAFF reserves the right to modify/increase/decrease any fees for competitions, coaches fees. Etc, at any time depending on circumstances.

GUA Fundraising- we FUNDRAISE to make it happen!! Arena Events Year Round- All Concerts, Sharks Games, Giants Game Must be a part of the booster club. 100% Profit. Starts NOW.($25 Booster Club Fee) *Profit per Arena event is $60 to $90 and it goes straight to your account* Krispy Kreme June and January 75/25 of profit -Mandatory Cookie Dough- August- 70/30 of profit- Mandatory Sam s Club, Car Washes, and Ramona Flea Market- All season-100% profit Simply Delights-February 70/30 of profit- Mandatory Applebee s- April- Mandatory **Mandatory fundraisers do have an OPT OUT fee for those who do not wish to participate. 2017-2018 HOLIDAY SCHEDULE-THESE ARE THE ONLY DAYS THE GYM IS CLOSED JULY 4 TH - ENTIRE WEEK SEPT 4TH- LABOR DAY NOVEMBER 22 nd - 25th THANKSGIVING (Wednesday thru Saturday) DECEMEBER 23rd- THRU JANUARY 1ST -CHRISTMAS (Saturday thru Monday) MARCH 30TH THRU APRIL 1- Good Friday and Easter Weekend I understand some may have holiday travel plans. Please email me with any concerns or conflicts.
