Please initial here in understanding that the only time you will be excused from jumping, tumbling, yelling, motion capabilities is with a doctor s note as well as coach/advisor permission. Precautionary Awareness Please write all pre-existing and current-existing injuries, illnesses, precautions, allergies below. Please initial here in understanding that if a pre-existing or current-existing issue is not written below, we are to consider it non-existing. A coach/advisor has the right to request a doctor s note. Any future-existing issues will be handled with a doctor s note as well as coach/advisor permission. ***must be turned in April 26, 2018 before 6pm to be eligible to try out*** 1

Strike Policy I understand that a student may receive a strike if s/he is sent home by a coach/advisor (disregarding those sent home for illness/injury/coach or advisor permission). I understand that excused absences are as the following: those that turn in a doctor s note, emergency and/or are deemed clear before the absence occurs by coach/advisor. I understand that excused tardies are as the following: those with an emergency appointment + doctor s note, teacher note, emergency and/or are deemed clear before tardiness occurs by coach/advisor. I understand that my absence that does not fall under the preliminaries above will be constituted as a strike. I understand that if I am 5 minutes-15 minutes tardy with an unexcused reason, I will be staying an equal duration of that time past to condition/clean/other request made by coach/advisor. I understand that if I am past 15 minutes tardy with an unexcused reason, I will be staying an equal duration of that time past to condition/clean/other request made by coach/advisor and I will receive a strike. I understand that if I receive three strikes I may be removed from the team and ineligible to tryout the following year. Please initial here in understanding of strike policy: Each team under the Oakmont Cheer program has a separate strike rule. - Teammates will receive three strikes for their game-time team position. - Teammates will receive three strikes for their crowd-leader team position. - Teammates will receive three strikes for their show team position. - Teammates will receive three strikes for their STUNT team position. First Sport Priority For game-time participants: please initial here in understanding that cheer is your first sport priority and activity at Oakmont High School. Those that choose to participate in other sports/clubs/activities understand that those are all the second priority. Your second priority sports/clubs/activities will be participated in if not interfering with your first priority. An example would be: taking part in a track team. When you are on cheer and track, you must go to a cheer practice/activity/event in place of the track practice/activity/event if scheduled at the same time. As a reminder, your second priorities should be communicated with both your coach and advisor. (Please see Oakmont High School policy of first priority sports for further information.) For non-game-time cheer participants: I understand that if I have already signed with other teams as that being my first sport priority (football over fall, baseball over spring, other sports communicated with advisor) that cheer is held as my second sport priority. If I am not currently at my first sport priority I am expected to be at cheer practice. The only times cheer will become my first sport priority is for competitions, and I will communicate with my first sport priority that that is the case. The strike policy is applied to me when I am not at my first sport priority. I will be on time and I will be dedicated to the program. Please write below any sports/clubs/activities you are considering partaking in over the course of your time in the Oakmont Cheer program. Please write below if you are considering partaking in any class courses that may interfere with your time in the Oakmont Cheer program (ex. Please write below if you are working and where during your time in the Oakmont Cheer program. Respect I recognize that as an Oakmont Cheerleader I represent not only the school and organization but also my family, peers, and community. I will respect: coaches, advisor, teammates, classmates, OHS staff, parents, volunteers in all discourses (discourses include but not limited to: face to face, via internet, via social media, via text, via call, via letter, via email). I understand that my parent/guardian/family are all a reflection of my respect and person and with that I intial here stating that they will be held to the same respect standards as I. I understand that I can receive a disciplinary action referral if both I and my acclaimed family show signs of disrespect in any of the above discourses but not limited to the above discourses. Disciplinary Action Referral Offenses Please initial here with understanding that all disciplinary action referrals can be made but not limited to the offenses below: - Inappropriate use of social media/web - Swearing - Slander - Physical proof of being under the influence - Physical proof of participating with those that are under the influence - Photography of student with influence - Photography or physical proof of student partaking in parties that are deemed inappropriate by coach/advisor ***must be turned in April 26, 2018 before 6pm to be eligible to try out*** 4

- Photography or physical proof of student participating in/discussing/acting out to sexual orientation - Inappropriate and unnecessary PDA - Inappropriate photos taken by you or of you - Inappropriate photos you take of others - Unsportsmanlike conduct in any discourse - Defacing public property (teepee, graffiti, etc.) - Receiving a school detention - Being reprimanded for not following school dress code - Photography or physical proof of child using swear words/slander/middle finger - Bullying of any kind toward any person - Reposting of any slander, swear words, inappropriate photography, sexual orientation, and/or other inappropriate associations judged by coach/advisor - Having a grade under a 60% for a continuous duration of four weeks or longer - Failure to meet school tardy and absence requirements - Failure to abide by Oakmont event policy - Failure follow the rules enforced by the hosting party of any and all cheering events - Failure to abide by school field trip policy during events Disciplinary Action Referral Guidelines Please initial here in understanding of the disciplinary action referral guidelines following: - The first time a student receives a disciplinary action referral they are placed under their first offense, in consequence of being benched for four weeks. - The second time a student receives a disciplinary action referral they are placed under their second offense, in consequence of being benched for four weeks or removal from the team deemed necessary by coach/advisor. I understand that coaches/advisor have the right to create new disciplinary actions throughout the season and I will abide by them. ***must be turned in April 26, 2018 before 6pm to be eligible to try out*** 5

Social Media I understand that at practices, games, and specified events my cell phone is to not be used. I understand that my social media accounts are a representation of me. Anything on my Web or social media that is inappropriate will be deleted after signing this. I will be held liable for any future posts on my account or of me on other accounts. (Please recognize that Oakmont Cheer highly recommends you delete your social media accounts.) Grades Please initial here in understanding of grade and GPA guidelines: - Student must maintain a GPA of a 2.5 for game-time cheer participation. - Student must maintain GPA of a 2.0 for show, crowd, and STUNT cheer participation. - Grade checks may be requested by coach/advisor. - Those that do not have above the GPA requirements during a school required grade check will be liable for disciplinary action created by Oakmont school policy. - Those that do not have above the GPA requirements during a school required grade check more than once in a school year will be liable for a disciplinary action created by Oakmont school policy as well as disciplinary action created by coach/advisor. Dress Code Please initial in understanding of cheer practice dress code as the following: - Must follow school dress code - Must wear AACCA approved cheer shoes - Must wear Oakmont apparel/oakmont colored apparel/usa cheer apparel or otherwise approved by coach/advisor - Must have hair fully secured - No jewelry whatsoever Please initial in understanding of cheer game day/game/activities/functions: - Coach confirmed (ex. uniform, warm-ups, bow, hair, etc.) - Following cheer practice dress code as well as school dress code I understand that if I do not follow dress code I can receive a disciplinary referral. ***must be turned in April 26, 2018 before 6pm to be eligible to try out*** 7

Leaving a TEAM I understand that if I quit any team with OHS Cheer and STUNT, it may jeopardize continuing on other teams. Before making the decision of leaving a team, I will meet with the Advisor to communicate any feelings for wanting to leave any team. The Advisor will make the final decision for my continuing on the team and other teams of OHS Cheer and STUNT. I understand that leaving a team or being removed from the team may jeopardize my being able to try out for the following team in the next season.
