This document details the required PPE for. These requirements are based off the most recent hazard assessment and could change in the future if tasks change or injuries suggest different or additional PPE is needed. Issuance Date: 03/12/2019 Basic PPE for Everyday Work Safety Glasses (3M- 1VJZ4) Safety Glasses to wear over prescription glasses (Condor- 1VW17) Closed Toe Work Shoes (employee supplied) Safety glasses provide protection from projectiles, debris, dust, chemical splashes, aerosols. Provides protection to feet and toes from light falling objects and some chemicals. To wear during all work activities where projectiles, aerosols, or splash hazards exist. (i.e. Cutting, Drilling, Sanding, Cleaning) For everyday use during the work day. Steel-Toe Rubber Boots (worn over work shoes) (Tingley- D0466) The rubber boots provide protection where work shoes may absorb a chemical or get saturated with water. The steel toe provides impact protection. Use with normal work shoes when entering wet areas, chemical spills, or lifting heavy equipment that could fall on foot. 1

Ear Muffs (optional) (Condor- 26X623) Ear Plugs (optional) (Max Series- 9X497) Cut Resistant Gloves (MCR Safety Cut Pro- J4380) Utility Gloves (MaxiFlex- 34-874) Provides hearing protection from loud noises. Cut resistant gloves provide protection from abrasions, cuts, and punctures. The glove pictured are cut level 2 on a 1-5 scale. Abrasion resistance= level 4, Tear resistance= level 4, Puncture resistance= level 3, on a 1-4 scale. Provides excellent protection from abrasions and tears, but limited protection from cuts and punctures. Abrasion resistance = level 4, Tear resistance = level 3, Puncture resistance = level 1. All on a 1-4 scale. Cut resistance = level 1, on a 1-5 scale. The higher the number, the higher the level of protection. Use if bothered by noise in mechanical rooms or from saws, cutting machines, jackhammers, grinders, etc. Working with materials with sharp edges, burrs, valve handles, sharp pieces or when dealing with general construction cut hazards. When using a knife, a cut resistant glove should be worn General work gloves for lifting or holding materials that may scrape or abrade the hand. These gloves offer excellent dexterity 2

Working with High Heat and Severe Cold High Heat Resistant Gloves (Ironclad Kevlar- D1723) Heat Resistant Sleeves (Wells Lamont- 33TW12) Provides heat resistance when working around hot surfaces or materials. Palms are cut and abrasion resistant. Withstands temperatures up to 450º F. Used to protect the arms when reaching over hot pipes or other hot surfaces. Conducting work on steam, hot water, or high temperature hot water lines When reaching over hot pipes or other hot surfaces. Cryogenic Gloves (National Safety Apparel- D1616) Protection against ultra-cold temperatures. When working with liquid refrigerants (HVAC Technicians). Working with or Handling Sharp Objects Cut resistant gloves provide protection from abrasions, cuts, and punctures. Hospital and Cleanroom Environment Work Gowns (Provided by UIH) Caps (Provided by UIH) Shoe Cover (Provided by UIH) Protects immune compromised patients in the hospital. Wear in patient rooms denoting this PPE is required. Wear in procedural areas in the hospital (i.e. operating rooms, central sterile). Isolation Gown (Provided by UIH) N-95 Respirator (3M:1860S- 4MH51 and 1860-4MH50) Protects immune compromised patients in the hospital. Protects you against infectious substances like TB. Required when entering patient rooms identified with Infection Prevention Contact or Contact Plus Precautions. Wear when working in designated areas of the Hospital. Wear for general maintenance of building HVAC system (replacement of filters). Can be worn if concerned about potentially infected material (blood, bodily fluids), and infectious agents (bacteria and virus). An N-95 respirator requires medical clearance, training, and fit-testing. 4

Working from Heights (Near Roof Edges, Raised Platforms, and on Lifts) Full Body Harness (Honeywell Miller- 45J036) Personal Fall Limiter Fall Protection Kit (Honeywell Miller- 3RCC5) Cross Arm Strap (Honeywell Miller- 1AG81) Suspension Trauma Safety Device (Honeywell Miller- 5PB18) Duffle Bag (Honeywell Miller- 63RJ26) To prevent injuries and death from falls. When working on any kind of lift, when working over four feet without guard rails, working on fixed ladder taller than 24 feet, and when working within 15 feet of the edge of the roof. Working in Dusty and Hazardous Environments Applicable PPE Picture of PPE Characteristics When Required Dust Mask (optional) (Honeywell- 4VT68) Paper mask that keeps dust and debris from getting into your mouth and nose. Does not require fit testing or medical clearance. Can be worn for employee comfort in environments with dust and debris. Not to be used with silica debris, asbestos, or hazardous materials. 5

N-95 Respirator (3M- 4JF99) Half Face Respirator with Defender Vapor and Particulate Cartridge (Honeywell North- D9084 and 16M232) Protects from inhalation of infectious substances. Does not protect against asbestos or any chemical vapor. Air purifying respirators with defender cartridge, provides protection from asbestos, heavy dust, and from common chemical vapors. Wear for general maintenance of building HVAC system (replacement of filters). Can be worn if concerned about potentially infected material (blood, bodily fluids), and infectious agents (bacteria and virus). An N-95 respirator requires medical clearance, training, and fit-testing. Wear when working in a building penthouse near fume hood exhaust fans or on a live fume hood exhaust line in a building. Can be worn if concerned about high hazard environments with chemical vapors and/or friable asbestos. A half-face respirator requires medical clearance, training, and fit-testing. 6

When Working with Chemicals, Solvents, Oils and Lubricants Disposable Nitrile Gloves (Microflex- E6445) Disposable Nitrile Gloves (Ecoline- EB0840) Heavy Duty Chemical Resistant Gloves (Ansell- 3RZJ5) Chemical Resistant Apron (Condor- 11G029) Face shield and Headgear (Honeywell North- 3NMA1 and 3NLZ9) Protects from chemical hazards associated with a variety of chemicals used in FM. Protects the user from strong acids, bases, oils, and a variety of flammable solvents. Provides chemical splash protection to the arms and torso. Face shields provide extra chemical splash protection when handling chemicals in larger volumes or in situations where splashes are more likely to occur. Use when working with small volumes of cleaning solvents, lubricants, organic and flammable solvents. Use when doing work inside a laboratory fume hood. When working with large quantities of solvents, corrosive liquids (acids or bases) or devices that contain acids and bases. 7

Electrical Work (Arc Flash Protection) ARC Flash Suit Kit (Category 2: 8 Cal/cm 2 ) (Oberon Company- 53PX57) Electrical Gloves (Salisbury- 4T488 4T489, and 4T490) Protects employees from electric shock and burns. The kit includes arc rated coveralls, arc flash face shield and balaclava. Protects employee s hands and wrists from electric shock and burns. When conducting emergency or nonemergency electrical testing. Hard Hat (MSA- D0369) Ear Muffs (optional) (Condor- 26X623) Ear Plugs (optional) (Max Series- 9X497) Protects the head from electric shock and burns. To protect employees from the impulse sound resulting from an electric arc which can exceed 160 decibels. During fuse replacement. Operation of circuit breakers and motor starters without covers installed properly. When making any energized electrical repairs. 8

Hot Work Tinted Safety Glasses (Shade 3) Cutting, Soldering, and Brazing Prevents eye damage while soldering. Wear tinted safety glasses of shade 3 when soldering. (Edge Eyeware- 5RXX6) Tinted Safety Glasses (Shade 5) (Edge Eyeware- 5RXX7) Cuffed, Leather Work Glove (Condor- D1603) Cuffed, Leather Work Glove (Condor- D1575) Welding Gloves (Tillman- D1606) Welding Gloves (Lincoln Electric- 49CE32 and ) Welding Prevents eye damage when brazing and cutting. Protects your hands and wrists from slag and sparks. Protects your hands from burns, sparks, and high-heat injuries. Gauntlet cuffs provide additional wrist protection. Also offers some abrasion resistance. Wear tinted safety glasses of shade 5 when brazing and cutting. When cutting, soldering, and brazing to prevent slag and sparks from getting onto hands and wrists. Wear when TIG welding. These gloves should be worn when arc welding or MIG welding. 9

Welding Hat (Steiner- 38RL35) Protects the top of the head from slag, sparks, and metal dust. Wear when welding to protect the head along with a welding helmet. Welding Helmet (Sellstrom- 464V21) Welding Jacket (Steiner- E9417) Prevents arc eye and retina burns, which can lead to loss of vision from highly concentrated ultraviolet and infrared rays. Ultraviolet rays can also damage uncovered skin. This flame-resistant jacket offers protection from sparks in welding applications. Wear when welding to protect the eyes and face. Wear when welding to protect the upper body from slag, sparks, and metal dust. Work that Generates Silica Dust (Applies to Pipefitters When Using Tools on Table 1 of Silica Standard) Applicable PPE Picture of PPE Purpose Required Goggles (Dewalt- 3RYH4) Coveralls (optional) (Dickies- H4069) Protect the eyes from silica particulates. To keep the work uniform clean and prevent silica dust from being carried home. Wear when working in a dusty atmosphere similar to a fog like haze. The goggles prevent dust from entering the eyes. Wear when grinding or cutting concrete silica to protect work uniform. 10

Cut Resistant Gloves for power tools (Magid PPD520-10) N95 Respirator (3M- 4JF99) (See Table 1 Silica Guide) Reduces impact to the palm when using power tools and provides 360 degrees of cut resistance. Cut resistance=5, on a 1-9 scale. Abrasion resistance=3, Puncture resistance =3, both on a 1-4 scale. Protects the lungs from silica dust. Wear when using power tools to cut concrete for pipe chase ways, or when drilling holes in concrete. Depending on the tool used in table 1 of the Silica guide, an N-95 respirator may be required. Respirator selection is based on the tool and duration of work. Cutting concrete to run pipe chase ways or drilling large holes in concrete for brackets may require N-95 respirator depending on the duration of the task. See table 1 of Silica guide for details. Respirators require fit testing, training, and medical clearance before use. 11

Working in Environments with Low Stationary Objects (i.e. Overhead Pipes) Bump Cap (ERB Safety- J5343) Hard Hat (MSA- D0369) Designed to protect the wearer from minor head bumps and lacerations, not intended to protect from falling or flying objects. Same purpose as the bump cap but also protects from flying and falling objects Use when working around low ceilings, overhead piping, or hanging items. Also wear when going above false ceilings. When there s a risk of impact with moving objects of any type, a hard hat must be used. (Westward- 12F682) Kneeling on Surfaces for Extended Periods Applicable PPE Picture of PPE Purpose Required Protects the knees Knee Pads from fatigue and injury. When kneeling on the floor, hard platforms, and roofs for extended periods of time. I certify that the hazard assessment required by 29 CFR 1910.132(d) has been performed to assess what personal protective equipment is needed for this workgroup. This personal protective assessment was also guided by a review of injury and illness data.
