1. SKINERGY Background SKINERGY leg covers are manufactured by RSLSTEEPER a UK based company. RSLSTEEPER is a Company providing prosthetic & orthotic clinical services and prosthetic products to over 25 counties worldwide. The company employs over 300 people. As well as providing clinical services the company is a specialist manufacturer of upper extremity systems, RGO s, and Assistive Technology devices. RSLSTEEPER is the largest provider of rehabilitation care to the UK National Heath Service. The company was delighted therefore when it was awarded the Independent Living Design Award for Innovation by the British Healthcare Trades Association for its Skinergy product. SKINERGY leg covers were first introduced for Seattle s split-toe prosthetic feet. Now they are available for both split-toe and closed-toe feet. Split-Toe leg covers are designated by LC# for Leg Cover. Recently RSLSTEEPER added the closed-toe leg covers (or Non-Split toe). These are designated by NS#. Because of their elasticity and the range of sizes, RSLSTEEPER states that they will fit all commercially manufactured prosthetic feet. Prosthetic feet are designated by length expressed in mm. The leg covers fit prosthetic feet ranging from 220-300 mm and are available in four sizes. Each size fits essentially two foot sizes (i.e. 220mm fits 220mm & 230mm feet). To properly fit various leg shapes, each foot size is available in two calf sizes. Again because of their elasticity, these leg covers fit a wide range of leg shapes. SKINERGY leg covers are available in 12 colours. The lightest, 00 may suit people of Celtic or Scandinavian origin whereas colour 19 is very dark and may be suited to people of central African origins. The most popular colours are; 0, 1, 3, 5 and 15. Less popular are colors; 7, 9, 11, 13, 17 and 19. RSLSTEEPER stock reasonable quantities of the popular colors and a few of the less frequently used colours, thes will be made to order but may take a few days longer. 2. Product Description SKINERGY leg covers are made from a silicone-based polymer called Tufsil 2 using a seamless manufacturing process. Tufsil 2 was chosen for its superior properties: excellent stain and tear resistance and significant elasticity. In fact, the product can be stretched in any direction by over 25%. This allows it to conform accurately to the underlying shape and to flex in areas where it covers moving components such as ankle movement. SKINERGY leg covers are approximately 1mm (0.04 ) thick and weigh less than 200gm (½ lb). The exact weight depends on the size and amount of material removed above the top of the prosthesis. Because these covers are thin, they can often be used as a direct replacement for fabric covers without changing the foot. The finished prosthesis should always be checked with the range of shoes to be worn. With some feet, it may be necessary for the workshop to modify the internal cosmetic cover material. SKINERGY leg covers stretch to fit two calf sizes. Because of their elasticity, they may support themselves from the tension against the inner cosmesis. However if they don t, they can be bonded at the brim/top of the prosthesis. The adhesive supplied with the leg cover can be used for this purpose. The leg cover should not extend above the top of the socket for additional suspension or for improved cosmesis because it can constrict the leg and cause problems. They may however, be used to form a smooth transition between the socket and skin. Also, since some individuals are sensitive to siliconebased materials, contact with the skin should be avoided. 3

3. Prosthetic Feet SKINERGY leg covers are highly elastic and therefore will fit most manufacturers prosthetic feet. They are available in four foot sizes covering feet ranging from 220 to 300 mm. Because of SKINERGY s elasticity, each leg cover actually fits two sizes. For example, LC22 is for a 220mm foot, but it will fit up to 230mm feet. SKINERGY fits all standard cosmetic foot covers or shells. In general the larger the width of the foot and the higher the function the less cosmetic SKINERGY will appear due to stretching and creasing as the foot mechanism moves through its position. 4. Care & Maintenance SKINERGY leg covers are quite durable. Tufsil 2 is an excellent product for this application. Although they are stain resistant, if they do pick up dirt, they can be cleaned with common soap and water. Contact with sharp objects and abrasive surfaces should be avoided to prolong the life of these covers. SKINERGY leg covers are not designed to be water-proof and therefore should not be immersed in water or other liquids. Small holes could allow moisture to penetrate the leg cover causing damage to the inner components. CAUTION: Silicone becomes very slippery when wet. Patients should be advised of this matter before they ever come in contact with water. It is advisable the product is not worn near swimming pools etc. 5. Nail Varnish/Polish SKINERGY leg covers will accept nail polish/varnish applied directly to the surface, however adhesion is generally poor. Because of the flexibility of the leg cover, the varnish is likely to flake-off rather quickly. To improve adhesion, it is necessary to create a stable interface between the silicone and the varnish. This can be accomplished by taking a small piece of clean fine cotton or linen cloth. Cut nail shaped pieces from this fabric. Smear the adhesive supplied with the leg cover onto the nail bed of the leg cover. Position these fabric nails onto the glue, pressing firmly to force the glue into the fibers. Cure for about 1 hour or gently warm the surface with a hairdryer to speed the reaction. Then paint the surface with nail varnish. Usually three coats will be required to provide a smooth surface. A new coat of nail varnish can be applied as necessary. The varnish can be removed with a regular acetone-based nail polish remover, but do this cautiously as this may result in some cumulative damage to the leg cover. Do not immerse the leg cover in acetone and limit your exposure to this chemical. Alternatively, with care the nail may be peeled off without damage to the leg cover. Finally, wash the surface of the leg cover with soap and water. Loctite 401 or 424 can be used to bond the interface but the nail will be less flexible and may craze. Nails made using these glues cannot be removed without damage to the leg cover. 4

7. Installation & Securing the Brim Plastazote (PE foam) or equivalent is used under SKINERGY leg covers because it allows minimum distortion under the compressive force generated. Soft foams can be used but expect them to be compressed by the cover. This may mean that it is difficult to match the contra-lateral leg and to accommodate socket shape smoothly. These foams are typically used in the manufacture of above knee cosmetic covers and are much softer to allow flexion/extension of the prosthetic knee without affecting function. SKINERGY leg covers are best installed by lubricating the exterior surface of the leg cover with a water-based lubricant. Turn the cover inside-out and roll it back on itself. Then place it over the prosthetic foot and roll it up the leg expelling any trapped air in the process. Once in place it can be trimmed with a scalpel blade for a clean cut at the brim. Clean the exterior with soap and water. Once the SKINERGY leg cover is dry, and after all adjustments and maintenance procedures have been completed, the leg cover can be secured to the brim of the prosthesis with the adhesive supplied, if this is necessary. The procedure is as follows: Remove any traces of foam, glue and dust from about 1/2 " (1) around and below the socket brim. Clean this area with a detergent-free isopropyl alcohol wipe. Fold back the upper part of the SKINERGY cover and clean the matching area with a wipe, removing any lubricant, powder, finger marks, etc. Place a thin coat of adhesive on the cleaned socket area. Roll the SKINERGY over the socket edge and if necessary, secure in place avoiding trapped air. The adhesive will fully cure in 24 hours but will provide an adequate fix in 1/2 hour, faster if warmed gently with a heat gun. Note that careful cleaning and only a thin coat of adhesive is required for the best results. This adhesive will allow the cover to be peeled away from the brim. Use care to avoid damaging the leg cover. A similar technique can be used with the patches provided to reinforce access holes. Be sure to cover the entire patch and allow the two surfaces to completely bond before punching the hole. For a more permanent bond, Loctite 424 can also be used to secure the leg cover. Sometimes it is difficult to bond to certain plastics. In these cases apply a thin cotton strip to the socket using a contact adhesive. When this glue has dried raise the surface fibers with glass-paper and then proceed as above. We recommend that you practice these techniques using surplus materials to become familiar with the application. With some split-toe feet it may be necessary to grind cosmetic foam material from the tips of the toes and to increase the space between the first and second toes. If this is done, be sure that the keel is not affected during the grinding process. 8. Using SKINERGY for Above-Knee Prostheses with Continuous Cosmesis SKINERGY is not designed as an Above-Knee leg cover, however it is possible to use these covers for this application where a stocking extension around the knee and over the thigh is acceptable. A below knee section is made of Plastazote and is rough shaped to provide a suitable match for the users sound leg. It is good practice to pack the foam to the tube at the ankle to minimize compression in this region. The BK cosmesis is trimmed horizontally 2 (¾") below the virtual knee center. An 6

anterior M/L cut at 135 º from the vertical is made, removing a wedge in front of the anterior surface of the knee unit. The posterior aspect of the foam is then removed with a curved M/L cut to give clearance for the maximum range of motion of the shin. The AK cosmesis is made using a regular soft foam and is rough fitted to the BK cosmesis. The whole is assembled onto the prosthesis, the two elements are glued to each other and to the ankle. Final shaping to the users requirements are now made. Two stockings of the appropriate color are fitted, one at a time, to cover the whole cosmesis. If a split toed foot is used, glue and trim the stocking as necessary, sealing to avoid laddering. Pull on the SKINERGY, trimming about 1 (½") above the glue line between the cosmesis. 9. Discontinuous AK Cosmesis; RSLSTEEPER supplies fabric reinforced silicone sleeves for use in their upper limb program. These can be supplied in colors to nearly match the SKINERGY range. For prosthetic builds with slim thigh sections, these covers can be suitable for discontinuous AK applications. Please contact us for further details. 10. Warranty SKINERGY leg covers are warranted against manufacturer s defects for a period of three (3) months. Most cosmetic cover suppliers do not warranty their products. To avoid damage, leg covers should not be exposed to sharp objects, abrasive materials, excessive heat or chemicals. Leg covers should be protected with shoes or other appropriate footwear at all times. If any signs of chafing appear on the SKINERGY leg cover, check the opposing surface and see if there is any sharp surface. If there is, correct this before doing further damage to the leg cover. 11. Frequently Asked Questions Skinergy Questions and Answers What is the warranty? We provide 3 months warranty from dispatch. Warranty provides for exchange or credit against manufacturing defects. Damage caused during installation or by footwear together with accidental holing or tearing under circumstances that we cannot control is not covered by this warranty. 7

Proof of damage is by return to your supplier, stating the order number and date of receipt. Under some circumstances a high quality digital image, showing the area of damage, providing the information required and the circumstances of failure, e mailed to the supplier or sales@rslsteeper may also be accepted. Will Skinergy make my leg waterproof? No, Skinergy is designed as a cosmetic covering. Liquids or dusts entering the prosthesis, either through a hole or tear in the cover or by penetration between the socket and the covert may lead to corrosion, bacterial or fungal growth and other unwanted effects. It is important that the user inspects for defects and entry points on a weekly basis. When the prosthesis is returned for maintenance we recommend that the cover be removed to check for and remedy any damage. Time should be allowed so that the clinic can carry out this work. Can I use it; I am an AK amputee Skinergy is designed to cover to knee level. Although additional material is allowed that can extend above the knee. In the technical literature provided to the installer, techniques are described that allow a fabric cover or stocking to be used with the Skinergy when fitted to above knee prostheses. If a good colour match is chosen a pleasing effect can be achieved. If the cover runs over the knee it will impede the action of the knee mechanism. It way also buckle providing an unacceptable appearance. The cover can be fitted initially without permanently attaching it to the stocking or foam cosmesis. Please discuss the options with your prosthetist so that you can determine what is acceptable. How many colours do you have? We make Skinergy in two colour ranges. The colours available are based on many years of experience and, we hope will provide a near match for most populations. It would help us to improve our product if the nearest match is not really close, you would discuss this with your prosthetist and make it known to us. We can then judge the market need to create a range to plug the holes in the range. The ranges use slightly different manufacturing techniques. We supply a standard range in 12 colours. The material has a uniform colour although the cover has a variable texture. This range extends from the lightest, 00 often used for people of Celtic or Scandinavian origin to 19 which is a very dark colour suitable for some people of African origin. We have a second range of 9 colours that extends from 00 to 13. This range is called Truefinish. The material used incorporates small coloured elements that provide a variable appearance. We believe that this range creates a more lifelike skin. In some countries this is only available by special order. We use a Photospectrometer to match batches of material during manufacture and compare the colours produced with electronically recorded master recipes. We compare cured samples of finished material under four different light sources. This process should ensure good batch to batch consistency within the limits of the process. Colour swatches can be viewed at the clinical centre or approximate colours are accessible on this web site. We cannot make special colours to order. 8

What feet are suitable? We believe that most commercially available adult feet can be covered with Skinergy. Two ranges are available, Closed Toe,. where all the toes are moulded without spaces and Split toe, in which the big toe is separated from the other four. No child s sizes are available at the present time. The range covered includes feet measuring 220mm to 300mm, four sizes cover this range. The cover stretches to cover all foot widths but the toe detail fits more closely on narrower foot shells. Will this work with Torque/Shock absorbers Yes, there is no evidence to suggest that Skinergy will be affected by the rotation caused by the action of a Torque or shock absorber. Clearly there may be some temporary wrinkling during the action mode of these components. It is unlikely that Skinergy will impede the movement of these devices. Can the cover be repaired We provide an adhesive and matching patches with Skinergy. These could be used to seal small holes although they are provided to reinforce access holes for adjustment tools. We do not recommend that these materials are used for anything but temporary repairs. Are these available on the National Health Service (UK only). The Government has made money available for cosmetics at NHS centres in England, Wales and Northern Ireland. Current provision does not cover all centres and individual supply depends upon local policy. Some NHS centres have provided covers without accessing this special fund, for instance some Scottish centres have provided covers on an individual basis. RSLSTEEPER do not control or seek to influence local ordering policy, either in general or for specific individuals. We will however, provide information and training on request to centres or where requested individual prosthetists and other medical and health professionals. Are Skinergy suitable for feet with Patient Adjustable Heels Yes, The two most common PAH mechanisms are fitted to; the Total Concept (Elation ) foot.. Adjustment of heel height is through the cover at the ankle. And the Blatchford (Endolite) Multiflex PAH foot. In this case access holes for the adjustment tool are punched in the appropriate locations at the heel. Reinforcement patches are adhesed in place prior to forming the hole. Are Skinergy suitable for Partial Feet Yes, Skinergy is recommended for partial feet. The sleeve can be rolled down for dressing and rolled back up the calf to provide suspension. Care must be taken, when Skinergy is allowed to press against the skin, to ensure that it is not too tight, restricting circulation and causing excessive skin marking. Two sizes of calf are available. During manufacture care must be taken to avoid sharp edges or hard surfaces that may damage the cover during use. 9

How do you recommend finishing Full technical instructions are included with the packaging. Cut with a sharp blade against a firm and stable surface. We recommend that the cover is trimmed just above the proximal socket edge. Can I paint the nails Skinergy can be painted but varnishes and paints peal off quickly. A better solution is described in the Technical information provided with the cover. The technique described involves making linen or cotton nails and using the adhesive supplied. The cotton nails can be painted and repainted. They can also be removed if necessary. Please follow the instructions provided. Can I use Skinergy for suspension We do not recommend that Skinergy is used as a suspension. This is because the pressure on the skin may restrict flexion and circulation. However, It may be trimmed so that it covers the edge of the prosthesis and just touches the skin to provide a smooth and cosmetic transition. Can I use Skinergy without shoes We do not recommend that Skinergy be used without shoes. Contact with harsh and abrasive surfaces or sharp stones will damage the cover and this damage is not covered by our warranty. At a minimum, light slippers or flip flops should be worn. Can I wear on the beach So long as appropriate beach footwear is worn and water and / or sand do not penetrate the cosmesis Skinergy can be worn on the beach. If you intend to tan before or during your holiday and you anticipate wearing Skinergy, discuss colours with your prosthetist in good time to arrange for a suitable replacement where this is possible. How do I clean Skinergy We recommend a regular, daily clean with a damp cloth using regular soap and water. Dry thoroughly after cleaning. We do not recommend using solvents to remove soiling on Skinergy. How do I stick it at the top? Full technical instructions are provided in this document and in the pack. A suitable adhesive is supplied for this purpose Can you leave it long? We do not recommend leaving the cover long as this may cause constriction and skin marking, particularly behind the knee. If it is necessary to do this, cut the edge of the Skinergy with a scalloped edge to relieve pressure at the edge. Scissors that leave a scalloped edge are available from regular tool suppliers. 10

SKINERGY Leg Covers ORDER FORM SKINERGY split-toe (LC) and closed-toe (NS) leg covers are available in four foot sizes and two calf sizes. The Part Number designates the foot size which is stated in (22 = 220 mm). The side (L or R) is stated next. Each foot size has two calf sizes small and large designated by the trailing letter (S or L). Both styles are available in 12 colors. Specify Quantity of each size needed (left-hand column) and the colors (right-hand column) and FAX the form to LTI.
