Event Description OTA club-level one day team tournament leading to a finals day at a central GTA tennis club. Recognition for the winning and finalist teams at the 2015 Rogers Cup presented by National Bank Team Definition 1 U12 Girls singles player 1 U12 Boys singles player 1 U12 Girls Doubles team 1 U12 Boys Doubles team Eligibility Open to Girls and Boys U12 - as of Jan. 1, 2015 Team consists of 6 players, 3 boys and 3 girls Players must be a member of an OTA club Players cannot have ever played in a two star or higher OTA tournament Full team rosters including the team captain and all players date of birth must be submitted prior to date of club level event Players who play at club level event must also play in final event All players advancing to finals day must bring a completed and signed waiver form to the registration desk Each player can only play for 1 team and in one category

Rules of Play Club level event Boys and girls singles tournaments single elimination draw or boxed round robin format dependant on number of entries. (Use box format if there are under five players). Playoff for third place in elimination draws In box format teams will play matches against all other players in box Objective is to produce one winning team which will represent the club at final event Winner of each draw (boys and girls) will represent the club as the singles player at the final tournament. Finalist and third place finisher will form doubles team and represent the club at the final tournament Pro set - 1 st to 8 games no ad, receivers choice sudden death game at 7 all. In box format, total games for in all matches will determine final position in case of tie, head to head will be first tie-breaker, followed by total games for, followed by total games against, followed by coin toss In elimination events, winners, finalists and third place finisher will be determined by round based play

U10 Pro set 1 st to 6 games no ad, receivers choice sudden death game at 5 all. TOURNAMENT/EVENT PARTICIPANT WAIVER FORM This Waiver must be signed by Participant or Parent/Guardian of Participant (if participant is under 18 years old) for the mini Rogers Cup finals event. WAIVER The undersigned acknowledges and agrees to assume all risk for bodily injury, waive all claims for injury and property damage and release and hold harmless Tennis Canada, the Ontario Tennis Association, the ICTA, its management and partners, the participating players and volunteers from any liability for personal injury and property loss or damage whether occurring prior to, during or subsequent to the mini Rogers Cup finals event, and consents and grants permission to, Tennis Canada, the, the organizers of this tournament or its or their respective officers, agents, representatives, and/or assigns to use (without, for greater certainty, any further consent or fee being required) any photography, videotape or other electronic recording device of my image taken, filmed or recorded during, or in connection with, my participation in this event, for any promotional, news or other purpose.
