It is recommended to stay in the same level for fall/spring to get the most out of the program. 2.5-3 year olds are the exception if the child has aged up. Class demonstrations and recital on stage, for all levels, will be held in the spring. Memphis Youth Musical Theater will present Snow White onstage at the Sandy Hook Theater. Date/time TBA. Auditions will be held the second week of class. Discounts and Payment Info If you are a full time Racquet Club Member, you may deduct $10 from your fee total. If you have siblings in the program, you may deduct $10 from your fee total. If you are taking 3 or more classes a week, you have option to pay ½ tuition now and ½ by 2 months into the semester. For the half payment option, you must email info@balletchildren.com for a form. Children's Ballet Theater Nutcracker Information Students who are in SK and older, up to 18 years, can be in the Children's Ballet Theater Nutcracker Saturday December 16 and Sunday December 17 on the beautiful GPAC stage. SK-1 students will be assigned one night to dance. We invite any PK & JK students who are attending the performance to present flowers to the ballerinas and Clara at the end of the show. They can wear their prettiest dress and a CBT headpiece. More info coming! NOTE to Level 3-6 : During Nutcracker rehearsals starting late September, Level 3-6 should save time for possible rehearsal between 11:00 am - 1 pm. Exact Schedule TBA. Mandatory Nutcracker Meeting with Costume Fittings will be held at the Racquet Club. Detailed schedule to be emailed. WAIVER OF LIABILITY, INDEMNITY AGREEMENT, AND ASSUMPTION OF RISK In consideration of permission to use, today and on all future dates the property, facilities, and services of Children s Ballet Theater, Memphis Youth Musical Theater, Racquet Club, Christ Methodist School or Church, Roulhacs, St Agnes, or Second Baptist (CBT programs) I, on behalf of myself, my heirs, personal representatives, or assigns, do hereby release, waive, discharge, and covenant not to sue, CBT programs, its directors, officers, contract workers, volunteers, independent contractors, and agents from liability from any and all claims arising from the ordinary negligence of any agent of Children s Ballet Theater or any of the aforementioned parties. This agreement applies to 1) personal injury (including death) from accidents or illnesses arising from the participation in our Ballet School activities including, but not limited to, organized activities; classes; observation; performances at GPAC theater, Buckman Theater or any other theater; and individual use of facilities, premises, or equipment; and to 2) any and all claims resulting from the damage to, loss of, or theft of property. Indemnification and Hold Harmless: I agree to HOLD HARMLESS AND INDEMNIFY Children s Ballet Theater from all claims resulting from negligence and to reimburse them for any expenses in investigating and defending a claim or suit if my claim is withdrawn, or to the extent a court or arbitration determines that Children s Ballet Theater is not responsible for the injury or loss. Severability and Venue: The undersigned further expressly agrees that the foregoing waiver and assumption of risks agreement is intended to be as broad and inclusive as is permitted by the law of Tennessee and that if any portion thereof is held invalid, it is agreed that the balance shall, notwithstanding, continue in full legal force and effect. Likewise, I agree that if legal action is brought, it must be brought in Memphis, Shelby County, Tennessee. Acknowledgement of Understanding: I have read this waiver of liability and indemnification agreement and fully understand its terms. I understand that I am giving up substantial rights, including my right to sue. I acknowledge that I am signing the agreement freely and voluntarily, and intend my signature to be a complete and unconditional release of all liability to the greatest extent allowed by the State of Tennessee. The undersigned parent and natural guardian does hereby represent that he/she is, in fact, acting in such capacity, has consented to his/her child or ward s participation in the activity or event, and has agreed individually and on behalf of the child or ward, to the terms of the accident waiver and release of liability set forth in Section 1. The undersigned parent or guardian further agrees to save and hold harmless and indemnify each and all of the parties referred to in Section 1 from all liability, loss, cost, claim, or damage whatsoever which may be imposed upon said parties because of any defect in or lack of such capacity to so act and release said parties on behalf of the minor and the parents or legal guardian. I hereby waive any claim of liability on behalf of Children s Ballet Theater programs, for any efforts to obtain treatment from a doctor or hospital for my child in event that my representative or I cannot be reached. IMPORTANT - REFUND POLICY: 75% refund is given if a student withdraws within 24 hours prior to a second class. No refunds will be given after this time except in the case of a physician-verified illness. There is no pro-rating and no makeup classes. Tuition is not transferable to another session. Students cannot attend class until full payment is made. There is a $30 fee for cancelled checks. CBT reserves the right to cancel classes due to inclement weather conditions or teacher illness. In the event of a weather condition, call the CBT office one hour prior to your class time at 921-0388 for a recorded message.
