GENESEE VALLEY BILLS MANDATORY BACKGROUND SCREENING The GENESEE VALLEY BILLS takes the safety of our participants very seriously. The GENESEE VALLEY BILLS will require that all coaches and board members who are on the fields with the players complete a background check. Purpose It is the intent of this policy to establish certain guidelines wherein the GENESEE VALLEY BILLS ( GVB ) can seek to protect children registered on the GVB football and cheer squads by investigating the background of volunteers who will be coaching our programs. I. Statement Criminal background screening on coaches is conducted by an outside third party which specializes in such work, and overseen (in specific instances) by a screening committee, a subset of the Genesee Valley Bills ( GVB ) Board of Directors ( Board ). This is executed in an effort to protect the children who participate in organized football, cheer, and related activities associated with GVB. II. Definitions GVB recognizes a coach is any person who leads and instructs players at practices and/or games - or portions of practices and/or games - for any football and/or any cheer team. The criminal background screening is mandatory, there are no exceptions. III. Procedure All candidates for the above mentioned positions must sign an Authorization and Consent Form for Criminal History, which gives GVB the right to check criminal history records and verify social security numbers. This work is executed directly, through www.protectyouthsports.com, and the secure URL. No other personal information (e.g. work history, financial, credit, etc.) is checked or researched. The company executing background checks has agreed to such terms contractually, and confirms such direction annually. The company never forwards any individual's information to the league where no issues are discovered. All information is strictly confidential and will not be made public under any circumstances. All criminal history record information will be promptly destroyed at the end of the current GVB season. A list of offenses prohibiting an applicant from participation as a coach or board member with Genesee Valley Bills is provided on the last page of this packet.

BACKGROUND CHECK AUTHORIZATION & CONSENT FORM In connection with my application to serve as a volunteer with GENESEE VALLEY BILLS, I understand that a consumer report and/or investigative consumer report, as defined by the Fair Credit Reporting Act (15 U.S.C. 1681), will be requested by GENESEE VALLEY BILLS for volunteer purposes, from Protect Youth Sports, Inc., ( Protect Youth Sports ), a consumer reporting agency as defined by the Fair Credit Reporting Act. These reports may contain information about me relating to my criminal history and social security number verification. Such reports may be obtained at any time after receipt of this Disclosure and Authorization and if I serve as a volunteer, throughout the course of my volunteer service, as permitted by law and unless revoked by me in writing. State Consumer Reporting Requirements Background Investigation Residents of New York only: Under state law you have the right to inspect and receive a copy of any investigative consumer report requested by Client by contacting Protect Youth Sports directly. COMMUNITY YOUTH FOOTBALL LEAGUE ZERO TOLERANCE POLICY In an effort to make youth football and youth cheerleading a more desirable and rewarding experience for all participants, the Community Youth Football League Board of Directors have instructed the participating local boards to adhere to certain points of emphasis relating to sportsmanship. This policy is designed to require all players, cheerleaders, coaches, helpers, officials, board members, administrators, parents and spectators to maintain a sportsmanlike and educational atmosphere before, during and after all CYFL functions (games, practices, competitions, etc.). Thus, the following points of emphasis will be implemented: PLAYERS / CHEERLEADERS During games an On-Field Official or Judge will asses a Zero Tolerance un-sportsmanlike penalty whenever a player or cheerleader: 1. Openly disputes or argues any decision by an official or judge. 2. Uses obscene or vulgar language at any time, even if it is not directed at a particular person. 3. Visually demonstrates any sign of dissatisfaction with any official / judges decision. Any time that a player or cheerleader persists in any of these actions, a game disqualification will be assessed and the appropriate paperwork will be submitted to the league president for further action. COACHES During games an On-Field Official or Judge will assess a Zero Tolerance un-sportsmanlike penalty whenever a coach: 1. Openly disputes or argues any decision by an official or judge. 2. Uses obscene or vulgar language in a boisterous manner to anyone at any time. 3. Visually demonstrates any sign of dissatisfaction with any official / judges decision, including intent to incite the officials, players or spectators. Any time that coach persists in these actions, a game disqualification will be assessed. OFFICIALS Officials are required to conduct themselves in a business like, sportsmanlike, impartial and constructive manner at all times. The actions of officials must be above reproach. Actions such as baiting or inciting players or coaches are strictly prohibited. On field officials are ambassadors of the game and must always conduct themselves with this responsibility in mind. PARENTS / SPECTATORS Parents / spectators will display appropriate behavior at all times. When parents / spectators display inappropriate and disruptive behavior that interferes with other spectators or the game, the game will be stopped for the purpose of removing the offender(s) from the spectator s viewing and game area. Once removed, play will resume. Lost time will not be replaced and violators may be subject to further disciplinary action by the local governing board. This inappropriate and disruptive behavior shall include: 1. Use of obscene or vulgar language in a boisterous manner to anyone at any time. 2. Taunting of players, cheerleaders, coaches, officials or other spectators by means of baiting, ridiculing, threat of physical violence or physical violence. 3. Throwing of any object in the spectators viewing area, sidelines or playing field, directed in any manner as to create a safety hazard.
