We would like to invite you to the 2 nd Annual 2014 Eastern Iowa Taekwondo Championship. The Eastern Iowa Championship will be held on September 20 th. Registration will take place between 8am and 9:30am and the competition will begin at 10am at the North Liberty Recreation Center, 520 Cherry Street, North Liberty, Iowa. Early entry fee will be $55 for forms and sparring. If you would like to participate in power breaking, creative breaking, or jumping high front snap kick breaking, or creative weapons forms please add $10.00** per competitor per extra event. Boards will be provided for power breaking and jumping high front snap kick will be with balloons. Creative breaking must bring their own boards. 2014 Eastern Iowa Taekwondo Championship REGISTERED STUDENT ORGANIZATION PARTICIPATION AGREEMENT, ASSUMPTION OF RISK, RELEASE AND WAIVER OF LIABILITY You have requested to participate in an activity sponsored by the Directors of the Eastern Iowa TKD Championships. All Taekwondo activities involve risks and it is important for you to have information about your selected activity and to provide Eastern Iowa TKD Championships Directors with information about yourself before participating in this activity. This Participation Agreement, Assumption of Risk, and Release and Waiver of Liability (Hereafter Participation Agreement ) must be read carefully and signed by all participants who take part in the activities offered at the Eastern Iowa Taekwondo Championships. PLEASE READ THIS AGREEMENT CAREFULLY. IT IS A LEGAL CONTRACT AND AFFECTS ANY RIGHTS YOU MAY HAVE IF YOU (OR YOUR CHILD) ARE INJURED OR OTHERWISE SUFFER DAMAGES WHILE PARTICIPATING IN THE EASTERN IOWA TAEKWONDO CHAMPSIONSHIPS. In consideration of Eastern Iowa TKD Championship Directors allowing me to participate in any way, including instructional classes, any Taekwondo tournament, practice, clinic, and related events and activities and travel to and from events or activities of Eastern Iowa TKD Championships, I agree and understand the following: Nature of Taekwondo Activity: Taekwondo is a contact sport. Contact sports are strenuous and require educational information about the necessary skills involved before the activity may be completed in the safest manner possible. Prior to your participation in any Taekwondo activity, you will receive activity information and have the opportunity to ask any questions you may have. Inherent Risks and Dangers of Taekwondo Activity: I understand and appreciate that the risks and dangers are inherent when participating in contact sport activities. I acknowledge and fully understand that I will be engaging in a contact sport that might result in serious injury, including permanent disability or death, and severe social and economic losses due not only to my own actions, inaction or negligence, but also to the actions, inaction or negligence of others, the rules of the sport of Taekwondo, or conditions of the premises or of any equipment used. Further, I acknowledge that there may be other risks not known to me or not reasonably foreseeable at this time. I understand that these risks and dangers could result in property damage and personal injury, including death, and I agree to accept all risks associated with the sport of Taekwondo and this activity whether present or future, known or unknown, arising from, or as a result of my voluntary participation in Taekwondo activities. Medical Emergency Permission and Financial Responsibility for Medical Treatment The health history provided for the participant is correct and complete to my knowledge. If an injury or other medical condition occurs or arises, I hereby give permission to the Cyclone Martial Arts Club staff or volunteer to provide routine health care and seek emergency treatment including x-rays or routine tests. I agree to the release of any record necessary for treatment, referral, billing or insurance purposes. I understand that Eastern Iowa TKD Championships does NOT carry any health insurance for participants and that I am financially responsible for charges and hereby guarantee full payment to the attending physicians or health care unit. In the event of an emergency where I cannot decide for myself (or if the parent is not available to make a decision for a child participant), I give permission to the physician/hospital selected by the Eastern Iowa TKD Championship Directors, staff or volunteer to secure and administer treatment for me (my child), including hospitalization initial date Behavior Expectations of the Participant: Successful participation in Taekwondo activities requires all participants to abide by rules and regulations of this activity in general and to the specific activity selected by the participant. This Eastern Iowa TKD Championship Participation Agreement, Assumption of Risk, and Release and Waiver of Liability shall be governed by and construed under the laws of the State of Iowa, which shall be the forum for any lawsuits arising from or incident to this Agreement. I, (participant) ASSUME THE OBVIOUS AND THE INHERENT RISKS AND LIABILITIES of participating in a Taekwondo activity or event and hereby RELEASE, WAIVE, DISCHARGE AND COVENANT NOT TO SUE the Eastern Iowa TKD Championships Directors and any of the officers, servants, agents and employees of the above-mentioned entities (hereinafter referred to as RELEASEES) for any liability, claim and/or cause of action arising out of or related to any loss, damage or injury, including death, that occurs as a result of my participation in the above-described activities. I agree to INDEMNIFY AND HOLD HARMLESS the RELEASEES whether injury is caused by my negligence, the negligence of the RELEASEES or the negligence of any third party. I further agree that this Participation Agreement shall bind the members of my family and spouse, if I am alive, and my heirs, assigns and personal representatives, if I am deceased, and shall be deemed as a RELEASE, WAIVER, DISCHARGE AND COVENANT NOT TO SUE the above-named RELEASEES. I hereby further agree that this Participation Agreement shall be construed in accordance with the laws of the State of Iowa. Base Points - Each participant always begins with 50 points. b. Participants have 60 seconds to set up and 60 seconds to execute their breaks. c. At the participant's discretion, he or she will attempt to break a minimum of 1 board and a maximum of 10 boards. (All boards are 1-inch pine boards.) d. Participants may not cover any part of the body that will be used for breaking a board with any bandages, tape or any other material. The referee must approve any injuries that may need to be covered. e. Props CANNOT be used, and boards cannot be altered. (For example, board fragments cannot be put on the top of the board to create an unfair advantage.) f. Participants must provide their own holders. Holders will ONLY be allowed on the competition floor during the participant s event. There are no uniform requirements for board holders. (However, a taekwondo uniform is HIGHLY encouraged. If board holders are NOT in a taekwondo uniform, clothing must be suitable for a family environment.) ONLY martial arts shoes are allowed on the mats. g. The participant may attempt to break each technique a maximum of 2 times. Broken boards may not be replaced on the 2nd attempt. (For example, if a participant does a split kick in the air and only one board breaks, the participant CANNOT replace broken board with a new board on the 2nd or 3rd attempt.) h. The following will complete each participant s score: a. Creativity (10 points) b. Execution of Requirements (10 points) c. Presentation (10 points) d. Taekwondo Spirit (10 points) Deductions a. Participant exceeds the 60-second time limit for setup ( 1 point deduction) b. Participant exceeds the 60-second time limit for execution (1 point deduction) c. Participant exceeds the maximum number of attempts per break, maximum of 2 attempts (1 point deduction per infraction) d. Unsportsmanlike conduct (1 point deduction for each occurrence)

Downward Knife Hand Power Break (16 years of age and older ONLY) 1. Each participant will have one attempt to break with a Knife Hand Strike in an area approximately 2 meters x 2 meters. The floor will NOT be matted. 2. The participant will attempt to break 1-inch pine boards with no spacers. Boards may not be taped together. 3. All boards will be placed on a board holding stand, which is 4 inches tall from the floor. The board holding stand will be placed directly on the floor. 4. The width of the board holding stand will be at the participant s discretion. 5. The participant's break must be done within 30 seconds. Once the referee declares Si Jak (begin), the participant may not touch the boards. 6. The participant must use the side of the palm (from the side of the first knuckle of the little finger to the front side of the wrist) for Knife Hand. Fingers can be bent; however, the hand cannot be closed in a fist position. 7. The participant must always keep at least 1 foot (left or right) on the floor while breaking. (The participant cannot jump off the floor with 2 feet to break the boards.) 8. Participants may not cover the breaking hand with any bandages, tape or any other material. The referee must approve any injuries that may need to be covered. 9. Each participant must PRIVATELY complete a form that states the number of boards he or she will attempt to break prior to the start of the division. 10. The participant who breaks the most boards will be declared the winner. 11. If two or more participants break the same number of boards, the participant who attempted to break the greatest number of boards will receive a higher score. (For example, Participant #1 attempts to break 5 boards and only 4 boards break. Participant #2 attempts to break 4 boards and succeeds in breaking 4. Therefore, Participant #1 will receive the higher final position within the division, because they attempted to break more.) 12. In the event of a tie, there will be an overtime round. (Add 1 additional board) 13. If there is still a tie at the end of the overtime round, the winner will be declared by body weight. (The lightest person will be declared the winner.) A scale will be provided at the event. Deductions a. Participant crosses outside of the 2 meter x 2 meter ring (1 board deduction) b. Participant disobeys the referee s instructions (1 board deduction) c. Participant exceeds the 30-second time limit (1 board deduction) d. Unsportsmanlike conduct (1 board deduction for each occurrence) (For example, trying to distract the other participants, not showing respect to others, etc.) Disqualifications e. Participant attempts to break the boards twice f. After attempting the break, participant falls down after breaking, touching the ground with any part of the body above the knees (The hand that broke the boards IS permitted to touch the ground after the break. Also a knee touching the ground is allowed.) g. Participant jumps off the floor with both feet to break the boards

Individual High Jump Kicking Balloon 1. Each participant has a maximum of 30 seconds to attempt to pop a balloon with Running Jumping Front Snap Kick wearing a special sock. 2. Each participant has only one attempt to break the balloon for each round. 3. The running distance is approximately 6 meters, and the starting board height will be determined by the referee. 4. The order of participants (who pops first) will be determined by height. The shortest participant will attempt to break first. 5. Participants must pop the balloon to advance to the next round. Balloon must be popped by the kicking technique to be considered a successful pop. 6. Once the participant passes the designated halfway mark, an attempt is counted upon approach. (For example, once the participant crosses the designated halfway mark, they may not return to the starting position to attempt to break again.) 7. After each round, the balloon height will be raised incrementally at the referee s discretion. 8. The participant to pop the highest balloon will be declared the winner. 9. In the event of a tie for 1st place, the height will be raised until a participant FAILS to pop the balloon. (If all participants fail to pop the balloon, then 1st place will be awarded to the SHORTEST participant.) 10. Disqualification a. If any part of the body touches the floor besides the feet, it is an automatic disqualification.

Power Breaking (Turning Back Kick or Skip (step) Side Kick) 1. Each participant will have one attempt to break with a Skipping Side Kick in an area approximately 1 meter x 2.5 meters. 2. The participant will attempt to break 1-inch pine boards with no spacers. Boards may not be taped together. 3. The participant's break must be done within 30 seconds. Once the referee declares Si Jak (begin), the participant may not touch the boards. 4. The participant must use the heel of the foot/foot blade. The middle of the boards must be at least as high as the participant s waist for Skipping Side Kick breaking. 5. Participants may not cover the breaking foot with any bandages, tape or any other material. The referee must approve any injuries that may need to be covered. 6. Each participant must PRIVATELY complete a form that states the number of boards he or she will attempt to break prior to the start of the division. 7. The participant who breaks the most boards will be declared the winner. 8. If two or more participants break the same number of boards, the participant who attempted to break the greatest number of boards will receive a higher score. (For example, Participant #1 attempts to break 5 boards and only 4 boards break. Participant #2 attempts to break 4 boards and succeeds in breaking 4. You can ONLY register for one or the other. Not both. WEAPONS COMPETITION RULES Weapons will be divided into divisions by age, and rank as follows: AGE DIVISIONS: (5-6), (7-8), (9-10), (11-12), (13-14), (15-16), (17-32), (33-39), (40+) BELT DIVISIONS: (WHITE, YELLOW, ORANGE), (GREEN, BLUE, PURPLE), (RED & BROWN), (BLACK) Male and Female participants compete together for weapons only. Only approved martial arts weapons may be used. Weapons will be inspected by the judges before competing. Competitors may supply their own music or props. JUDGING WILL BE DETERMINED BY: Base Points - Each participant always begins with 50 points. The following will complete each participant s score: A. Creativity (5 points) B. Execution of Requirements (10 points) Compulsory taekwondo kicks required Colored Belts 2, front kicks 2 Round House, 2 Side Kicks ), Black Belts - 2, front kicks 2 Round House, 2 Side Kicks, 2 Turning Back, 2 Turning whip. C. Level of difficulty(10 points) D. Rhythm and flow of routine (10 points) E. Overall Presentation (5 points) F. Taekwondo Spirit (10 points)
