On seeing or hearing the warning signal all transfer operations must stop immediately. (Refer ppendix.9 SPC Emergency Warning Signal) This section has been prepared to include relevant details from the Port Botany Emergency Plan, the SPC Bulk Liquids Berth Emergency Shutdown Procedure (refer ppendix B.11), and the SPC Ship/Shore Checklist Fire Instructions (refer ppendix B.12). 6.4.1 PERSON DISCOVERING FIRE OR HZRDOUS SITUTION T THE WHRF MUST: 6.4.1.1 VOPK TERMINLS USTRLI OPERTIONS PERSONNEL: a) INITITE THE EMERGENCY WRNING SIGNL by pressing the emergency warning signal button located on the inside or outside of the Wharf Shelter. b) IMMEDITELY STOP LL WORK. If a shipping transfer is occurring INSTRUCT THE SHIP (OR TERMINL) TO STOP PUMPING and then CLOSE WHRF LINE BLOCK VLVES at the Vopak manifold. c) NOTIFY the Vopak Terminals ustralia Security Officer and Terminal Operations personnel by two-way radio. d) NOTIFY the SPC Port Officer (SSO). e) MINTIN CLOSE TWO-WY RDIO COMMUNICTION with the Vopak Emergency Controller or TEMO (or other Terminal Operating personnel), who will activate the Terminal Emergency Procedures Plan to

We may be required to stand by to disconnect wharf hoses from the ship. 6.4.1.2 VOPK CONTRCTORS, VOPK VISITORS, VOPK NON-OPERTIONS PERSONNEL: a) NOTIFY the Terminal Operations personnel (either Vopak Terminals ustralia or other berth operators) who are operating facilities at the wharf. b) EVCUTE the wharf and SSEMBLE at the EMERGENCY SSEMBLY RE, if accessible, located at the Bulk Liquids Berth (BLB) menities Building and await further instructions. 6.4.2 PERSON HERING OR SEEING THE EMERGENCY WRNING SIGNL, HERING THE FIRE LRM, OR BEING NOTIFIED OF FIRE OR HZRDOUS SITUTION T THE WHRF MUST: 6.4.2.1 VOPK TERMINLS USTRLI OPERTIONS PERSONNEL: a) IMMEDITELY INSTRUCT THE SHIP (OR TERMINL) TO STOP PUMPING (unless the ship has been notified) and close the wharfline block valve at the Vopak manifold. b) NOTIFY other Terminal Operations personnel by two-way radio (unless they have been notified). c) MINTIN CLOSE TWO-WY RDIO COMMUNICTION with the Vopak Emergency Controller, TEMO (or other Terminal Operating personnel), who will activate the Emergency Procedures Plan to provide assistance and take appropriate action to close all necessary valves to isolate wharf to Terminal pipelines and tanks. d) EVCUTE the wharf and shut isolating valves in the culvert. e) SSEMBLE at the EMERGENCY SSEMBLY RE, if accessible and safe, located at the Bulk Liquids Berth (BLB) menities Building and await further instructions.

VOPK CONTRCTORS, VOPK VISITORS, ETC.): a) STOP WORK IMMEDITELY, turn off or isolate all equipment ND NOTIFY the Terminal Operations personnel (either Vopak Terminals ustralia or other berth operators). b) EVCUTE the wharf and assemble at the EMERGENCY SSEMBLY RE, if accessible and safe, located at the Bulk Liquids Berth (BLB) menities Building and await further instructions. 6.5 PIPELINE CORRIDOR EMERGENCY 6.5.1 PERSON DISCOVERING FIRE OR HZRDOUS SITUTION IN THE PIPELINE CORRIDOR: 6.5.1.1 VOPK OPERTIONS PERSONNEL: a) INITITE THE EMERGENCY LRM by immediately CONTCTING THE SENIOR OPERTION TECHNICIN activate the following responses as appropriate: Terminal assistance (internal alert) Emergency Services as required Fire Brigade, mbulance, Police Terminal Fire larm Terminal Evacuation larm Port Botany Emergency Radio & larm System b) IMMEDITELY STOP LL WORK. If a shipping transfer is occurring INSTRUCT THE SHIP (OR TERMINL) TO STOP PUMPING ND TO CLOSE WHRF PIPELINE VLVES at the VOPK WHRF MNIFOLD and at the MIN HOSE EXCHNGE. c) UNLESS DELING WITH THE EMERGENCY, EVCUTE THE PIPELINE CORRIDOR and SSEMBLE at the nearest safe location (or the Vopak Terminals ustralia emergency assembly area) and await further instructions.

Document No.: OPS09.1 Page: 44 of 205 6.5.1.2 OTHER PERSONNEL a) NOTIFY THE SENIOR PERSON T THE WORK SITE (either Vopak Terminals ustralia supervisor or other supervisor) and NOTIFY (via radio or telephone - internal ext. 941, external 9666-4455) THE SECURITY OFFICER of the emergency situation details. b) EVCUTE THE PIPELINE CORRIDOR (IF NECESSRY) and SSEMBLE at the nearest safe location (or emergency assembly area) and await further instructions. 6.5.2 PERSON HERING N EMERGENCY LRM OR BEING NOTIFIED OF N EMERGENCY MUST: 6.5.2.1 VOPK OPERTIONS ND OTHER PERSONNEL: a) STOP WORK IMMEDITELY, CLOSE DOWN OPERTIONS, REMOVE LL SOURCES OF IGNITION (i.e. turn off or isolate all power tools, welding machines, equipment, etc. in use). b) SSEMBLE T THE NEREST SFE LOCTION (or emergency assembly areas) where a headcount will be made, and await further instructions.

EMERGENCY RESPONSE & CONTROL INTRODUCTION The main consideration of Emergency Planning is the protection of people, property and the environment from hard during an emergency situation. This is achieved by developing an emergency plan that implements a system able to respond automatically to any emergency and that leads to the most effective outcome possible under the circumstances. The Plan must be dynamic and interactive, ensuring ongoing relevance to the needs of the faulty and all stakeholders by continual monitoring, review and consultation. ll of the stages are inter-related and details of the Plan must be continually evaluated and revised as appropriate. The Vopak Site Emergency Planning process is based on the following illustration. DEFINE IM CONSULTTION DEFINE OBJECTIVES DEFINE PRMETERS DEFINE SYSTEM MNGE SYSTEM MONITOR ND REVIEW The critical part of the preparation of the Plan is that it needs to be preceded by careful planning, based on an appreciation of the hazards and understanding of the possible emergency scenarios, their possible impacts and the availability of emergency response resources both internal and external to the facility.

Document No.: OPS09.1 Page: 46 of 205 7.1 EMERGENCY ORGNISTIONL STRUCTURE The organisational structure during the initial period of an emergency, and for the particular internal or external alert, shall be: In certain circumstances the level of emergency may require the activation of the: VOPK USTRLI CRISIS MNGEMENT PLN This plan is integrated with the Terminal Emergency Plans and is primarily intended to manage the business recovery and insurance, media liaison and corporate reporting aspects of a significant emergency situation. FIGURE 7.1: EMERGENCY ORGNISTIONL STRUCTURE The Emergency Controller will be the first available person, as shown on the "List of uthorised Company Personnel for Emergency Controller and TEMO (Terminal Emergency Management Officer)" (refer ppendix B.6). The TEMO will be the second available person, as shown on the list. EMERGENCY CONTROLLER For example: Operations Coordinator TEMO Site Terminal Manager Rescue Evacuation Traffic Control First id Control and Damage Control Communications (Internal & External) The following sections describe the duties and responsibilities of Company Personnel involved in an emergency at this site.

Document No.: OPS09.1 Page: 47 of 205 7.2 PERSONNEL RESPONSIBILITIES ll personnel must have a sound working knowledge of this plan. ll operating personnel present on the premises will be responsible for the initial local control of the emergency. 7.2.1 EMERGENCY CONTROLLER This function is to operate at the site of the emergency to co-ordinate and direct Vopak Terminal Emergency Teams. He will provide information and assistance to the Emergency Services at the scene of the emergency. The Emergency Controller, during operational hours, will be the first available person on the "List of uthorised Company Personnel for Emergency Controllers & TEMO s" (refer ppendix B6 List of uthorised Company Personnel Contact List). Outside operational hours the Emergency Controller will be the first person on the list to arrive on site and will remain Emergency Controller until relieved by a more senior person on the list. The Emergency Controller will make certain at all times that the minimum number of people are used during an emergency, and that only those people concerned are at the scene of the emergency. The four most critical keys for prompt control are:- 1. COMMUNICTION - internal alarm systems, then 000 for fire brigade, ambulance and police, Port Botany Radio and larm System. 2. PRESERVTION OF LIFE - evacuate to the assembly area or offsite evacuation. Rescue if risks allow. 3. CONTINMENT 4. COOLING/EXTINGUISHING - With water or foam. Duties & Responsibilities: 1. Obtain two-way radio, obtain and wear Emergency Controller's orange safety helmet and orange vest for easy recognition. (Note: Three sets of safety helmets (ie. Emergency Controller and TEMO) are kept at the Security Office, the Site Terminal Manager s office and the Safety Equipment Shed (near Operations Building). 2. Establish as quickly as possible radio contact with the TEMO

Document No.: OPS09.1 Page: 48 of 205 and account for all personnel on site, in particular those in the affected area. 3. Proceed directly to the site of the emergency and take direct control of the emergency. Take action as required utilising facilities and personnel. 4. Upon the arrival of the Emergency Services, the Emergency Controller must brief the Officer-in-charge with the following information: a) location and type of emergency b) details of personnel injured or trapped c) action taken to date d) location of all personnel on site e) details of product involved (eg MSDS etc) f) any other relevant information The Emergency Controller will then hand over the responsibilities of directing the emergency operations to the Officer-in-charge of the relevant Emergency Service and will offer whatever assistance may be required. t this stage, the Port Botany Emergency Plan is activated at the discretion of the head of a Combat gency in consultation with the Terminal Emergency Controller. 5. Determine the magnitude of the Emergency and liaise with the TEMO for assistance, as required. 6. Keep TEMO informed of the emergency status. 7. When additional personnel and equipment are available, deploy as necessary. 8. Decide whether to cancel alarm if conditions warrant a cancellation. 9. Decide on the requirements for evacuation of affected area, the entire site and/or from the Port Botany area. 10. Notify as soon as practicable those customers whose products have been involved in the emergency. 11. Liaise with TEMO on release of personnel and equipment during termination stage. 12. Following an emergency, decide, after consultation with the TEMO, emergency services and authorities as appropriate, how/when/if to resume normal operations.

Decide whether other uthorities need to be informed of the Emergency, eg, Sydney Water, Energy ustralia, Natural Gas Co, WorkCover (Dangerous Goods Branch). 15. llocate personnel if required to do so by an uthority to assist cleaning or containing waste residue resultant from an emergency. 16. t completion of the emergency, hold debriefing meeting and minute all proceedings. 17. Set up a recovery strategy in line with the SPC Port Botany Emergency Plan. 18. Prepare investigation reports and recommendations for actioning. 19. Contact all major clients (and relevant product owners) - refer requirements in Section 8.2. 20. Revoke all existing Work Permits and re-issue as appropriate. 21. Refer ppendix D.1 Emergency Controller Checklist for guidance 7.2.2 TERMINL EMERGENCY MNGEMENT OFFICER (TEMO) The TEMO will assist the Emergency Controller and is responsible for: establishing and coordinating the emergency communications in support of the Emergency Controller and Emergency Services. providing technical assistance providing back-up coordination for the emergency During an emergency the TEMO will be stationed at the Security Office with a Vopak radio (to maintain contact with Vopak personnel at the actual emergency site). The magnitude of the emergency may require the setting up of the Port Botany Emergency Operations Centre (EOC) and activating the Port Botany Emergency Plan. The EOC is located at the Port Services Botany Survey Compound Meeting Room (Inter-Terminal access road off Penrhyn Road). This action is decided by the Police Site Controller or head of a Combat gency. If so, the TEMO must report to the Emergency Operations Centre. The TEMO, during operational hours, will be the second available

Outside operational hours, the TEMO will be the second person on the list to arrive at site, and will remain TEMO until relieved by a more senior person (if available) on the list. The TEMO will utilise the two-way radio situated at the Security Office and establish radio contact with the Emergency Controller. Duties & Responsibilities: 1. Obtain two-way radio, obtain and wear the TEMO's orange safety helmet and orange vest for easy recognition. (Note: Three sets of safety helmets (ie Emergency Controller and TEMO) are kept at the Security Office, the Site Terminal Manager s Office, and the Safety Equipment Shed (near Operations Building). 2. Set up emergency control centre in the Security Office. 3. Immediately establish radio contact with the Emergency Controller and confirm the level of emergency. 4. Meet all requests and obtain all information for the Emergency Controller. 5. s people become available, advise the Emergency Controller who may direct them to duties as required. 6. llocate Vopak personnel as necessary to assist the Security Officer in performing his/her duties, e.g. traffic control, telephone system, etc. 7. Notify as soon as practicable all customers whose products have been involved in the emergency (refer customer contractual requirements as appropriate). 8. rrange: - Headcount - First id or Medical help - Security - Telephone manning - Traffic control - Manning of evacuation point, if evacuation is ordered - Handover to the appropriate external emergency services. Notify, as required: (refer ppendix B.1 - Emergency Telephone Log) - Emergency Services - Neighbouring companies and Sydney Ports Corporation

EP, WorkCover uthority, etc.) - Prince Henry Hospital - Utilities providers (i.e. Water Board, GL, Sydney Electricity) NOTE: Copies of the checklists are kept in the Security Office - Emergency Information Boards. 9. Should additional personnel be required, the TEMO will arrange for call-ins. 10. rrange for maintenance aid, as required. 11. Ensure that there is constant observation and control of the operation of the fire pumps in the fire water and foam pump house. The person designated to do this must have a two-way radio. 12. t the conclusion of the emergency, call a meeting of operators and contractor personnel, and as appropriate, Emergency Services and relevant authorities, to discuss the start-up procedure. 13. For guidance, refer ppendix D.2 TEMO Checklist NOTE: ll work permits should be revoked and must be renewed before work recommences. 7.2.3 DURING NORML HOURS THE SECURITY OFFICER ROLE IS FUNCTION OF THE LOGISTICS CO-ORDINTOR OR HIS DELEGTE. DURING OTHER HOURS THE SECURITY ROLE IS FUNCTION OF THE SENIOR OPERTIONS TECHNICIN ON SITE. The Security Officer is responsible for contacting the Emergency Controller and TEMO, contacting emergency services for all emergencies, contacting neighbouring companies and SPC (via Port Botany Emergency Radio and larm System), and for contacting the Contract Security Company Security Centre for after operational hours emergencies. The Security Officer is also responsible for site security and to control movement through the main gate. Only properly authorised personnel are to be admitted during an emergency. MEMBERS OF THE PRESS, OTHER NEWS MEDI, ND THE PUBLIC, MUST NOT BE DMITTED. ll media enquires are to be handled by the Managing Director or his nominated media liaison representative.

Document No.: OPS09.1 Page: 52 of 205 Note: dditional Vopak personnel may be appointed by the TEMO to assist the Security Officer in his/her duties or the Security Officer may request nearby Vopak personnel to assist. Duties & Responsibilities: 1. On hearing the Fire larm, or being notified of an emergency, stop all traffic from entering the Terminal. Traffic to be parked away from the entrance gate. 2. On receipt of details of emergency, if requested, contact Emergency Services immediately through one of the following (in order of preference): 3. Phone 000 and request appropriate emergency service(s). 4. Using the SPC Port Botany Emergency Radio and larm System to request assistance from BLB and/or neighbouring site to notify the appropriate emergency service(s). 3. Inform the Emergency Controller and TEMO of emergency, if available. 4. If required, activate the Terminal Evacuation larm and evacuate the Security Office taking relevant personnel and truck movement logs. 5. Standby to attend to the phone system, the Port Botany Emergency Radio and larm System, and to inform appropriate authorities or neighbouring companies, as directed by the Emergency Controller or TEMO. 6. If emergency occurs after operational hours, contact the Site Terminal Manager immediately upon discovery of emergency situation. 7. Facilitate the exit of all trucks sent from the emergency area and instruct drivers to park at a safe distance away from the main entrance gate. 8. Two-way radio contact to be maintained between Emergency Controller and TEMO. The TEMO must respond to any requests made by the Emergency Controller. 9. Close traffic and pedestrian gates. Lock one set (exit gate), but leave the other set unlocked. 10. Manually open the security gates using the key provided. (Note: The Logistics staff shall ensure that when they evacuate their office that the two boom gates at the Weighbridge are raised.) 11. Man the gate. Permit access of emergency vehicles and essential personnel. Log all vehicles and personnel entering the

Document No.: OPS09.1 Page: 53 of 205 Terminal, enabling a detailed check to be made at the conclusion of the emergency. Personnel admitted to the site must be authorised by the TEMO. 12. KEEP THE TERMINL ENTRNCE CLER T LL TIMES. Direct all vehicles not authorised to enter the Terminal, away from the entry road. 13. Emergency Service vehicles required at the emergency scene, by the Emergency Controller, should be directed to the scene without delay. 14. Emergency vehicles not immediately required at the emergency scene should be marshalled outside the main entrance in Friendship Road. 15. Police are responsible for traffic control on the public road, to keep the roadway clear and spectators away from the Terminal entrance. 16. The emergency situation MUST NOT be discussed with members of the news media or the public. The news media should be directed to await a press release from the Managing Director or his nominated media liaison representative. 17. ttend all other duties as requested by the TEMO. 18. The Security Officer will continue to operate under emergency conditions until instructed to resume normal duties by the TEMO. 7.2.4 FIRST ID TTENDNT The health and safety of personnel, and provision of First id to the injured, is of prime importance. ll efforts should be directed to caring for the injured. First id ttendant(s) will be designated by the TEMO. Each must be a Vopak employee having a current First id Certificate. The TEMO will also designate an Emergency First id rea(s) where injured persons shall be treated if practicable. This area may, or may not, be the existing First id Centre, depending on the situation. Duties & Responsibilities: 1. The person(s) manning the Emergency First id rea(s) will establish and maintain contact with the TEMO as soon as possible. Radio communication must be used and maintained. 2. The Emergency Controller will arrange for the injured to be sent to the Emergency First id rea(s). If it is necessary for the

Document No.: OPS09.1 Page: 54 of 205 First id ttendant to go to the injured person, the TEMO will arrange for manning of the Emergency First id rea(s). The Emergency First id rea(s) are not to be left unmanned during an emergency. If extra help is required, the First id ttendant should contact the TEMO. 3. The First id ttendant will do one of the following, depending on the seriousness of the injury: a) MINOR - render the proper First id and, if practical, return the employee to work. If not practical, allow recuperation time at the Emergency First id rea. b) CHEMICL to render the proper first aid reference to the MSDS of the chemical involved may be required c) OTHER - injuries requiring professional medical treatment. The First id ttendant will call the TEMO to arrange to have the injured person taken to: Prince Henry Hospital, nzac Parade, Little Bay; or Prince of Wales Hospital, High Street, Randwick. When the injury is caused by exposure to a chemical, a copy of the MSDS for the chemical involved will be sent to the hospital with the injured person. The TEMO will arrange immediate access of the ambulance to the Terminal, and have it directed to the proper location. Company vehicles may be used in place of an ambulance if the First id ttendant judges it to be quicker, and the injury allows transport by car. The TEMO will arrange transport. NOTE: The names of any casualties that occur during an emergency must remain confidential. 7.2.5 TERMINL EMERGENCY TEMS The Terminal Emergency Teams, or Hose Teams, shall be made up of Vopak Staff and Operators as designated at the time of the emergency by the Emergency Controller. Duties & Responsibilities: Under the direction of the Emergency Controller, provide all assistance necessary to control, reduce or stop the cause of the emergency, and to minimise any secondary damage which may propagate the incident. Where necessary, and if safe to do so, carry out search and rescue operations for persons unaccounted for.

Document No.: OPS09.1 Page: 55 of 205 7.2.6 EMERGENCY OPERTIONS CENTRE (EOC) The magnitude of the emergency may require the setting up of the Port Botany Emergency Operations Centre (EOC) and activating the Port Botany Emergency Plan. The EOC is located at the Port Services Botany Survey Compound Meeting Room (Inter-Terminal access road off Penrhyn Road). This action is decided by the Police Site Controller or head of a Combat gency. If so, the TEMO must report to the Emergency Operations Centre (EOC). 7.3 PROCEDURES The Vopak Site Emergency Planning process is designed to identify all hazards, which will reasonably be expected to initiate, or contribute to, an emergency. This process involved identifying hazards of several different types:- hazards arising from the Dangerous Goods (generally Class 3) stored in the facility hazards arising from activities or equipment associated with the facility (Loss of Containment (LOC), plant, machinery, transport, electrical), and natural hazards (floods, cyclones, lightning strikes) that could impact upon the safe operation of the facility. Each development stage of Vopak Site included a range of Safety Studies:- Preliminary Hazard nalysis HZOP Final Hazard nalysis Fire Safety Study From these studies a list of potential incidents were derived. It is not feasible, or appropriate, to develop individual systems to respond to every potential incident identified in the above-mentioned hazards identification exercises. screening technique was utilised to produce a representative set of incidents; the screening stops were:- eliminate localised incidents that would not require activation of the emergency plan consolidate incidents that have similar materials, inventories, discharge rates, discharge locations and types of emergency response actions and taking into account the potential impacts, potential escalation and the manner in which a release (LOC) or failure scenario could escalate; and

Many of the products stored in the Terminal are non-flammable, i.e. Combustible C1 or C2, Class 6, Class 8, or non-dangerous goods. Some products have the potential to generate a toxic/flammable vapour cloud. It is imperative to ascertain the actual product involved in a spillage or leakage incident as quickly as possible in order to verify if a fire risk, or toxic vapour risk is present. This information is critical in deciding which of the following action plans must be put into place. For spillages or leakage with fire risk and/or a toxic vapour cloud risk refer Figure 7.4. For spillages or leakage without fire risk refer Figure 7.5. Decontamination Procedure The following sets out the procedure to contain and decontaminate areas that have been affected by a loss of containment. - Contain the spill using sand or soil prevent run off into drains and waterways. - Do not allow vapour or liquid to collect and remain in hollows or sumps.

Document No.: OPS09.1 Page: 65 of 205 7.3.1.6 BOMB THRET Bomb Threat is a warning received by any means of a threatening intention to detonate an explosive device having been placed to cause risk or damage to the Terminal, pipeline corridor or Bulk Liquids Berth. ll bomb warnings are to be treated seriously. Necessary actions and precautions must be taken to minimise danger to the public, employees and the plant until the risk has been removed or until the warning has been proved to be unfounded. Quick and accurate communications are essential. ll staff who may have to handle these initial communications must be thoroughly familiar with these procedures. 1) blank copy of a Bomb Threat Checklist (ustralian Bomb Data Centre form) is kept near every telephone except those in public areas, i.e. at the following locations: - Logistics Office Foyer - Extension 924. copy of the Bomb Threat Checklist is provided in ppendix B.4. The Bomb Threat Checklist should not be kept where it may be visible to the general public, i.e. Contractors, Truck Drivers, etc. 2) ny person receiving a (telephone) bomb threat should:- i) Take it seriously. ii) iii) iv) Keep the person talking and seek as much information as possible. Utilise the Bomb Threat Checklist during, or immediately after receiving, the threat. Share the call, where possible.

If this is not possible, then great caution must be exhibited during there use. 7.3.2 SPECIFIC EMERGENCY PLNS 7.3.2.1 GENERL This section provides additional information for managing specific types of emergencies or specific hazardous materials. Document No.: OPS09.1 Page: 73 of 205 7.3.2.2 PROPYLENE OXIDE EMERGENCY Propylene Oxide (PO) is a flammable liquid with a low boiling point and a high vapour pressure, it is also a known carcinogen (refer Material Safety Data Sheet for further details). Propylene Oxide is stored in Tank 46. This tank is of 1650 m 3 capacity and constructed as a low pressure storage tank to PI 620 - Recommended Rules for Design and Construction of Large, Welded, Low-Pressure Storage Tanks. The tank is provided with Nitrogen gas blanketing and with a water scrubbing system for product vapours. Control Panel larms n alarm panel located in the ex-jortl Control Room (adjacent to the Logistics office), monitors the facility for various pressures, temperatures, fluid levels, and flow. Table 7.1 details the types of alarms, remarks, and action to take in the event of an alarm condition. dditionally, a siren and strobe light are mounted on the tank top that will activate in event of one of the alarms below (except the two loading pump alarms of temperature high and no flow ). Check vapour return to ship. IF NOT SSOCITED WITH SHIPPING OR PIGGING: 1. Check that the vapour return pipeline vent slop tank has the correct water level in it. 2. Partially open the ball valve on the vapour return take off line to the slop tank. 3. Pressure will let down through the scrubber. 4. Cool tank with water deluge if action 1-3 fails to control pressure. 5. Check operation of nitrogen pad control valve. This valve should not be passing nitrogen into tank when tank pressure exceeds 13.8 kpag (2 psig). 1. Check pressure relief valve for faulty operation 2. Check that water supply valve is open and that water is available. Water will flow to scrubber until correct level is reached and ball cock shuts off. 2. Check that vent flow to scrubber is not excessive. If flow is high check tank pressure and operation of tank pressure control valve. 1. Immediately radio ship to stop pumping. 2. Check that all product and vapour return valves are open between ship and shore tank. 3. Recommence pumping when pressures have equalised and return to normal. 1. Check that suction & discharge valves are open. 2. Check that the outlet and the tank emergency block valves are open. 3. Check that all excess flow valves are set. 4. Check that tanker valves are correctly set to receive product. 5. Check that truck and tank pressures are balanced. 1. Check as in 1-5 above. Use alcohol foam, dry powder or carbon dioxide. Cool tank with deluge water. Use self-contained breathing apparatus and PVC suit, gloves and gumboots.

Document No.: OPS09.1 Page: 77 of 205 PROPYLENE OXIDE SPILLGE Minor Spillage In the event of a minor spillage, isolate area, wear coverall, PVC gloves and goggles. Hose down with large amounts of water. If spillage has occurred near a tanker, DO NOT MOVE TRUCK OR CLOSE THE BTTERY MSTER SWITCH. fter the area has been cleaned down a Gas Free clearance must be issued to ensure that all vapours have been dispersed and that there are no flammable gases in the area. Only after the issue of the Gas Free clearance can the truck be moved. Hose Leak Failure of the stainless steel loading hose will cause the excess flow valve to operate. * Immediately close the remotely operated tank emergency block valve and stop the transferring pump. * If spillage is large, isolate area. Decontaminate using large volumes of water. Persons carrying out the decontamination must wear selfcontained breathing apparatus, PVC gloves, full PVC suit and PVC gumboots. The area must not be entered by unprotected persons until after the clean up is completed, and all vapours have dissipated. Gas Free clearance must be issued to ensure, by testing, that all vapours have dissipated and that there are no explosive or flammable gas mixtures. Only after the issue of a Gas Free clearance can the truck be moved. ON NO CCOUNT MUST THE TRUCK BTTERY MSTER SWITCH BE OPERTED OR THE TRUCK STRTED UNTIL THE RE HS BEEN CERTIFIED GS FREE.
