Extreme Adventure Ltd Risk Assessment Activity Spider Mountain What are the Fall from height through webbing rings Descend one level at a time. Use webbing levels to halt rapid descent. Matting provided to break fall. A one way policy to run so participants are always climbing in the same direction listening to climbing and descending advice from Instructor Descending onto other climbers Good observation by other climbers whilst descending Likelihood of Risk (L)- 3 Overall Risk (S x L)= 3 listening to climbing and descending advice from Instructor. Footwear removed prior to climbing Attempted exit through side/ top netting listening to participation advice. Overall Risk (S x L)= 1 Regular inspection of netting integrity and security straps Minor bodily injury to other climbers Consider safe route to ascend away from other climbers. What are the Injuries caused by people wandering into the site /Instructors Barriers are used to separate the public from the course. This means only users that have been safety briefed and fitted with safety equipment Likelihood of Risk (L)-1 Signage and the set out of site makes it clear where people can or cannot go not suited the course i.e. being too heavy or too small to deal with the elasticated webbing. This may result in a hazard to themselves or other climbers Clear signage showing the allowable user minimum heights accompanied and unaccompanied are displayed. Signage also dictates the maximum weight of a user. A pair of scales is on hand to ensure clarification MEDIUM Clear signage and training staff in the clear procedures and policies of the company to maintain a safe operation

What are the Tripping over anchorage points / spare equipment / electrical cable All anchor points must secure and installed as per manufacturer s instructions. All cables must covered to avoid trips etc Daily checklist carried out being kicked whilst climbing through tower by other users All footwear is removed by all before entering the climbing tower Instructor trained being hit by another as they slide down Strict control by staff to ensure only one person at a time slides down and that the exit point of the slide is clear i.e. other participants standing on the end of the slide MED Constant vigilance by staff sliding off the end of the slide and onto the ground The slide is designed to slow the rider down at the end of the slide, and as an extra precaution a safety mat is laid at the bottom also. Care should be taken in rainy conditions MED Constant vigilance by staff

Calculation of Risk Evaluation Severity (S) Severity of Risk is judged by evaluating the effects of the hazard if the risk occurs. This is evaluated as Minor = 1, Major = 2, Serious = 3 Risk Likelihood (L) The likelihood of the harm occurring is evaluated on the following basis: Unlikely =1, Possible = 2, Likely = 3 Overall Risk Overall Risk is calculated by multiplying the figure for Severity (S) x Likelihood (L).
