MEDICAL SERVICES AND CONTROL All players are obliged to present a Health Certificate M-4 Form issued 22 nd October 2017 or later. The organizer will provide first aid. All other medical services should be paid in accordance with the Rules of the medical service. 8. OFFICIAL VOLEYBALL RULES AND BALLS The competitions shall be played in accordance with the CEV Rules of the Game in force. The only Ball to be used for all matches is the MIKASA MVA-200. 9. UNIFORMS The playing uniform must be in conformity with FIVB Rules of the Game. (Player s names are recommended but not mandatory).
Each team must have available 2 sets of playing uniforms Captain s set and Libero s set. 10. EXPENSES EXPENSES COVERED BY THE ORGANIZER All transportation expenses from the Tbilisi Airport to the hotel and competition haa and back. The hotel and food costs max for 21 persons (12-14 players, 6 officials+ 1 referee) per team, for a total max of 8 double rooms and 5 single rooms per team. EXPENSES COVERED BY THE PARTICIPATING DELEGATIONS Each team shall bear its own international traveling expenses to the international airport of arrival/departure. Each team must pay a contribution fee to the food & lodging to the organizer for each of the official member of its delegation, amount to: 250 EURO (i.e. 50, - EURO x 5 [five] days) except referee. This amount must be paid through bank transfer to the following bank account by November 14, 2017 and must be settled even if the duration of the stay is shorter. Lodging and accommodation expenses of all referees, including those who are coming with teams have to be paid by organizer. Referees can be accommodated in double rooms. A maximum of 3 (three) extra persons, in addition to the 20 official team members may accompany their respective delegations at their own expense (the names have to be communicated to the organizers, using the EEVZA-05 form). The extra persons are entitled to single rooms for the price of 100. - EUR (the price includes all services provided to the official team members). All hotel and food expenses incurred after 12h00 of the day following the last match played. All insurance not borne by the Organizers shall be borne by participating teams. All telephone and mini-bar costs in the hotel (on the arrival day, the Team manager of each team must leave his credit card number at the reception desk for this purpose). 11. PRELIMINARY INQUIRY Preliminary Inquiry will be carried out by the organizers on the 22 nd of November 2017 according to the time of the teams arrival. The following persons must be present: Team manager, Head Coach. The following items are to be controlled: EEVZA-5 Form - team registration EEVZA-05bis Form final team registration Passport or ID cards Health certificate of each player Medical doctor accreditation certificate (if any) Physiotherapist s accreditation certificate (if any) Copy of a payment order (contribution fee) 2 (two) sets of player s uniform (captain & libero) If Medical Doctor or Physiotherapist have not CEV/FIVB Accreditation he/she needs relevant confirmation letter by concerned National Federation. 12. INFORMATION FOR THE TEAMS
Each participating National Federation must send the EEVZA 05 Form before the starting of tournament within 30 October 2017 at latest. Each participating team is obligated to have EEVZA M 4 Form filled. All the matches will be played in three winning sets. National Federation, hosting an EEVZA Championship, 1 (one) MONTH BEFORE THE START OF THE COMPETITION must sent by e-mail a scanned document form EEVZA-07 to EEVZA Headquarters 13. LAUNDRY Organizer will ensure laundry of 12-14 sets of uniforms (1 pair of socks, 1 t-shirt and 1 pair of shorts) during match days (except last competition day). 14. VISAS A visa is required for certain EEVZA member countries entry into Georgia. Each Federation, if necessary, must apply visa in time at the Embassy of Georgia in their country.
