Coding and Documentation The goal is always to work toward better documentation for the following reasons: Avoid misinterpretation by third parties (such as auditors, payers, attorneys) Justify medical necessity Provide a more accurate clinical picture of the quality of care provided Support current & future initiatives aimed at improving quality and reducing costs, such as value-based purchasing 14
