HOW TO FILL IN YOUR PASSPORT APPLICATION FORM Return to: USA-UK Registrar Please check website for current address and prices: www.american-saddlebred.co.uk Timescales: USA-UK strongly recommends all paperwork is send via secured post to the Horse Registrar. Passport Packs are available online and a paper copy will be sent upon request within 7 days of receipt of a stamped addressed envelope. Passports will be issued within 15 working days of receipt of a correctly completed application form with full payment received. For purebred Saddlebreds who are awaiting ASHA registration, they will be entered onto our Appendix A registry (see below) and can be upgraded onto our Main Registry once the required conditions are met passports will need to be returned to the USA-UK Registrar to be updated. All equines must have a passport by the age of 6 months of birth or before 31 st December of the year of birth, whichever date occurs later. Please read these explanatory notes BEFORE completing and returning the application form. All horses MUST be microchipped before a passport can be issued, which must comply with ISO 11784 and 11785 and should not begin with the 826 UELN UK code. Microchips MUST be fitted by a Veterinary Surgeon. Step 1: Read all the instructions carefully. Please use block capitals. Step 2: If you are applying for a passport, then a Veterinary Surgeon must fit a microchip and return the markings with the microchip site indicated as and complete the ID form (Parts A and Parts B). Part 1 must also be completed with the horse and owner details, which should be completed by the person applying for the passport. The completed form should be returned with the correct fee to the USA-UK Horse Registrar (see website for current fees). PHOTOGRAPHIC ID MAY ALSO BE INCLUDED WITH YOUR APPLICATION and is MANDATORY FOR ALL PINTOS (skewbalds & piebalds) 2 colour photo s, one of each side of the equine, plus front and rear views, must be sent for pintos. Step 3: The microchip must comply with ISO 11784 and 11785 and should not begin with the 826 UELN UK code and can only be fitted by a qualified Veterinary Surgeon, who should not be the owner of the horse. Step 4: The written description should be written in BLOCK CAPITALS or TYPED as these need to be legible. If we cannot read the writing, then it will hold up your passport application while we have to check with the veterinary surgeon what was written! It should also match the silhouette. White markings should be drawn hatched (shaded) in red ink. For white markings on the leg state how high the white extends i.e. to fetlock or to mid cannon bone. If there are no white markings, this in itself is an identifying feature and should be stated on the legs, as should white or striped hooves. Whorls should be shown as an X and if there is feathering with ----X----. Prophets Thumb (a distinct indentation) should be shown as a triangle. Acquired marks scars should be recorded by the use of an arrow pointing to the scar. If your horse has less than 3 (three) whorls, then detailed description of their chestnuts MUST be taken and included in the application (photographic evidence acceptable). Please note: if your horse cannot be identified from its passport you will not be meeting the requirements of the legislation and the horse may not be eligible for registration. Step 5: If your horse already has a passport, and you are applying for Registration only, then you ONLY NEED TO COMPLETE Part 1 and return the form, the original passport and the correct fee to the USA-UK Horse Registrar (see website for current fees). PHOTOGRAPHIC ID IS MANDATORY FOR SKEWBALDS/PIEBALDS. Step 6: Sign the declaration at the bottom of the application form (Part 1) and return with the correct fee.

DETAILS OF EQUINE Further information Name - Prefixes / suffixes must only be used if they are owned by you/the breeder and registered with the Central Prefix Registry. If an equine is bred by a breeder with a registered prefix, that prefix should be given to the equine on registration. Duplicate names (Saddlebreds only) or distasteful names are not permitted. A distasteful name is one, which, in the opinion of the USA-UK Committee, is distasteful and/or likely to cause offence, in which case, the name will not be permitted. Equine s pedigree - Please enter the name and registration number of AT LEAST the sire and dam. If these are not known, please enter not known. If the sire and / or dam are known, but not registered, please enter as much information as is known e.g. approx. Date of birth, owner s details etc (even if only Dobbin, 1989, Mr Kentucky, Maidstone, Kent). Please submit covering certificates (these are available to download from our website and may be used by ANY BREED applying for one of our passports). Breed Please put specific breed if known (e.g. Saddlebred etc.). Do NOT put heavy type or Cob etc. If breeding is not known, please put NOT KNOWN. Verification of breeds can only be made by the respective breed society. If your horse s sire and dam are NOT registered with the United Saddlebred Association-UK or the American Saddlebred Horse Association (in America), then their details will not be on file if you wish the details to be included within the passport, please send a copy of their respective pedigrees. Please note we can only verify Saddlebred breeding. Equines of a specific Breed: you are encouraged to obtain your passport from your breed society in the first instance. We can, however, issue passports to any equine, but will check with the relevant breed society that an existing passport does not exist for the equine in question before issuing new passports for equines of a specific breed (non-saddlebred). AMERICAN SADDLEBREDS: Please show USA-UK Registration Number for both sire and dam. If one, or both are NOT USA-UK registered, please submit a copy of registration (e.g. with another ASB society). Sire, dam and foal being registered must be DNA tested for proof of parentage for all purebreds before acceptance onto the USA-UK Main Registry. If a Saddlebred is registered with: ASHA, CRLC Canada or SHBSoSA (South Africa) then proof of parentage will be accepted, but the DNA Profile Number and the Name, address and fax number of the lab must be submitted at the time of application. Rescued equines reasonable checks will be made for an existing passport, but a passport may be issued but will be signed by us in Section IX part II as not for human consumption. Colour This must be the main body colour. In the case of Skewbalds and Piebalds, please give the base body coat (e.g. Bay and white). Pinto s / Paints MUST submit photo s with their application. Place of birth Please enter the county (if born in UK) or country and district (if born abroad). If place of birth is not known, please enter not known. Height Please give approximate adult height measured to wither (highest point at base of mane). May be entered in either hands (units of 4 inches) or centimetres, Please ensure you specify which! RECORDING OWNER This must be completed and the owner must sign the application form. Please include nationality of owner. EQUINE IDENTIFICATION This must be completed by a Veterinary Surgeon. TRANSPONDER /MICROCHIP all equines MUST have a microchip and we will undertake reasonable checks that the transponder number and silhouette is completed correctly, legibly and by a veterinary surgeon. If you already have a passport and are applying for Registration only, then only Part 1 of the application form needs to be completed AND the original passport submitted a Veterinary Surgeon is not required to complete the form in these cases.

It is the owner s responsibility to notify USA-UK of changes of ownership within 30 days and to return the passport for update. Updates to passports (e.g. change of ownership) will be made within 10 working days of receipt of all necessary information and fees. Upon death of the equine, the owner must notify USA-UK and return the passport immediately. The passport may be returned to the owner, once invalidated, upon request. DUPLICATE/DAMAGED/REPLACEMENT PASSPORTS please see the specific form for this, available on the website. Horses applying for a passport after 31 st December of year foaled will all be issued with a replacement passport and signed out of the human food chain. GRADINGS (any sex) AND STALLION LICENCING please see the specific form for this, available on the website. FRAUD It is our duty to carry reasonable checks to ensure that passports and passports applications are valid and correct, include checking applications and returned passports against existing records and sample passports for consistency. When signs of fraud have been suspected, or if there is any evidence of modification or tampering then these will be reported immediately to the horse passports enforcement authority. BREACHES OF THE REGULATIONS Any offence under the Regulations must also be reported by USA-UK to the horse passports enforcement authority. These include, but are not limited to: when a passport is applied for outside the statutory time limit (6 months of birth or before 31 December in the year of birth) this is with the exception of rescued equines and those who have purchased a horse without a passport; when a passport is applied for when one already exists for that animal; failure for the new owner to update owner details within 30 days; failure an owner to return a passport for updating within 6 weeks of notification that the passport does not meet the current legal requirements; signs of unofficial changes to passport details, tampering or fraud.
