Tackled football is for children entering 4 th through 8 th grade in the fall of 2014. Attached form must be turned in at the Storage Shed behind the Columbus High School on Saturday May 17, 2014 between 9:00 am and 2:00 pm. It is important that the players are there for the enrollment for proper equipment and jerseys. NOTE: Parents of all players will be required to help during home games with the food stand, chains, and clock; this will help keep the cost down. Parents are required to work at two (2) games during the season. Those may include Freshman, JV or the Youth Football games. 2014 Registration Fees Registration Fee: $150 (Columbus School District Participant) $190 (non-columbus School District Participant) 4 th and 5 th Grades Only Fund Raising Option: The Columbus Football Organization offers a Fund Raising option to help reduce the fees for participating in the 2014 Fall Tackle Program. Registration Fee for the Fund Raising Participant: $80 (Columbus School District Participant) $120 (non-columbus School District Participant) 4 th and 5 th Grades Only All fees include a refundable $20 deposit. This deposit will be returned at the end of the season based on two requirements. 1. Equipment must be returned on time and clean based on the cleaning instructions provided. 2. The parent and/or guardian sign up and works two (2) games during the season. This will include working at the youth games or food stand at the JV or Freshmen games. Coaching does not count towards the deposit refund. Uniform Costs: $55 for both youth and adult sizes ***Please Note: All players entering 4 th grade or new to the program will be required to purchase a game jersey. You may also purchase a new jersey at sign up if you have out grown or you need to replace yours due to wear.***
Players I/we, the parent of the above player for the position on the football team, hereby give my/our approval to participate in any and all football activities, including transportation to and from the activities. I/we know that participation in football may result in serious injuries and protective equipment does not prevent all injuries to players, and do hereby waive, release, absolve, indemnify and agree to hold harmless the local Columbus Football Organization and City of Columbus, the organizers, sponsors, participants and persons transporting my/our child to and from activities for any claim arising out of any injury to my/our child whether the result of negligence or for any other cause, except to the extent and in the amount covered by accident or liability insurance. PARENT & ATHLETE AGREEMENT As a Parent and as an Athlete it is important to recognize the signs, symptoms, and behaviors of concussions. By signing this form you are stating that you understand the importance of recognizing and responding to the signs, symptoms, and behaviors of a concussion or head injury. Parent Agreement: I have read the Parent Concussion and Head Injury Information and understand what a concussion is and how it may be caused. I also understand the common signs, symptoms, and behaviors. I agree that my child must be removed from practice/play if a concussion is suspected. I understand that it is my responsibility to seek medical treatment if a suspected concussion is reported to me. I understand that my child cannot return to practice/play until providing written clearance from an appropriate health care provider to his/her coach. I understand the possible consequences of my child returning to practice/play too soon.
