BIG ISLAND JUNIOR GOLF ASSOCIATION WWW.BIJGA.org 2015 ENROLLMENT AGREEMENT CONSENT AND AUTHORIZATION The Big Island Junior Golf Association (BIJGA) was founded in 2002 with its primary goal as promoting life skills through the game of golf. To achieve its goals, the BIJGA has developed a program to provide children of different age and skill levels opportunities to play golf. Participants develop fundamental golfing skills at the various clinics organized by the BIJGA as well as learn about themselves. Participants not only discover their physical and emotional peaks, but also learn about their physical and emotional limits. When they decide to play in golf tournaments organized by the BIJGA, participants learn to make decisions in a environment of competition best of all, while they are learning they are having fun. For the serious members, the Big Island Jr Golf Association can also help participants understand the sometimes confusing and complicated procedures required to compete in Statewide and even National Junior Golf Tournaments. Tournaments are planned for this season at various golf courses on the Big Island for the BIGISLAND JGA members children ages 6 & under, 7-8, 9-10, 11-12, 13-14 and 15-17. Club Plays will be scheduled throughout the year/season and will be open to all age groups whenever possible. Support is needed in many areas, so please expect to be called upon to assist the various committees assigned to ensure that your child s golfing experience is not only challenging but also fun! In consideration of my/our child s participation in the BIJGA program, the following are agreed upon: 1. I/We agree to follow all applicable golfing and safety rules in force at any golf course where a BIJGA event is held (see attached BIJGA Rules and Regulations). 2. I/We agree to supervise and instruct my/our child so that they understand that he/she must follow all applicable golfing and/or safety rules in force at any golf course while a BIJGA event is held or failure to comply may be reason and good cause for the BIJGA to suspend or disqualify my/our child from participation at any BIJGA event. If I/we do not know the rules I/we agree to inquire with and age group coordinator of the BIJGA event as to the nature of the rules and agree to explain the applicable rules to my/our child. 3. I/We agree to follow all applicable rules of golfing and rules etiquette established by the United States Golf Association and to instruct my/our child of such rules. I/We understand that the failure of my/our child to follow the applicable rules of golfing and rules etiquette established by the United States Golf Association may be reason and good cause for the BIJGA to suspend or disqualify my/our Jr from participation at any BIJGA event. 4. I/We agree to be responsible for my/our child s actions and behavior to defend and indemnify the BIJGA for any loss or injury resulting from my/our and/or my/our child s negligence. 5. I/We agree to volunteer for BIJGA events in which my/our child is eligible to participate in. I/We understand the notice of the assigned task of volunteering will be prior to BIJGA events. If unable to fulfill the responsibility to volunteer, I/WE shall bear the responsibility of finding a substitute to serve in my/our place and to inform my/our child s age group coordinator with the name and telephone number of the person serving as my/our substitute scorekeeper/volunteer. I/We understand that a lack of required volunteers needed to operate an event may result in cancellation of my child s age group tournament. 6. I/We authorize the BIJGA to release, furnish and/or publish my/our name, telephone number, and my/our child s name in any way to BIJGA in order to allow the BIJGA to conduct its business. 7. I/We authorize the BIJGA to release, furnish and/or publish my/our child s name and/or photograph in any way to media or publication sources. 8. I/We agree and understand that as the parent and/or guardian of the child participating in any BIJGA event, I/we am/are responsible for ensuring that my/our child is physically fit to do so and that I/we have obtained any applicable medical clearance required to ensure my/our child can safely participate in such events. 9. I/We understand that there are inherent risks of personal injury and/or property damage in my/our child s participation in any BIJGA event. With full knowledge of such risks, whether foreseen or unforeseen, on behalf of myself/ourselves, my heirs, my personal representatives, my assigns, and my/our minor child, I/we agree, except to the extent of any applicable liability insurance coverage available from any source including the BIJGA or its agents, employees, representatives, officers, directors, contractors, vendors, and others: 2

a. To assume any and all risks of injury, loss or damage which may arise out of such participation, including but not limited to: 1) the rendering of any first aid and/or medical treatment arising therefrom, and 2) the provision of transportation to any BIJGA event by the BIJGA or contractor hired by the BIJGA to provide such transportation. b. To waive and release any claims against he BIJGA, and/or its agents, representatives, employees, officers and/or directors, both in their professional and personal capacities, for any and all injuries, losses or damages connected with or arising out of such participation, and c. To indemnify and hold forever harmless the BIJGA, and/or its agents, representatives, employees, officers and/or directors, both in their professional and personal capacities, form and against any and all claims which may arise out of such participation. 10. I/We agree and understand the rules and duty of a spectator within all BIJGA events on or near the golf course and will abide by the following criteria or rules of a spectator: a. Spectating is observing and watching without communication. b. A spectator must remain beyond 50 yards from any player when following a group, and observe players on the Tee Box or Green from a reasonable location that will not disturb participants. c. While spectating, observers must walk on cart paths and/or on the side of all fairways. Walking on fairways while following a group is not allowed unless you are crossing the fairway perpendicular to the green and tee box. d. Spectators might be a relative or friend and do not adhere to this agreement. Therefore, friends and relatives must check in with our BIJGA committee prior to events and receive permission from the starter or clubhouse before walking on the golf course. This policy applies to applicable golf courses that allow spectators. e. Spectators must observe without disrupting players. Quiet photos are ok but cell phones are not allowed on the golf course during play. f. A player will be disqualified if a spectator communicates with a player or violates the rules of this policy. g. The BIJGA committee can suspend a player or spectator at any time. 11. I/We agree to wear proper Golf Attire at all BIJGA events on or near the golf course. This rule applies to all that adhere to this agreement to include parent, guardian, and Jr Golfer member. Proper attire includes a collared shirt, shorts, skorts, skirts, or slacks, belt if pants have loops, and covered shoes. The following is not allowed: denim, slippers, surf shorts, short skirts, cargo shorts, bare feet, tee shirts without collar, and tank tops. All volunteers and spectators must adhere to the Golf Attire Policy. Members must wear BIJGA golf shirts to all BIJGA events on the golf course. 12. In the event of any medical emergency, my/our child s name is and the name, address and telephone number of his/her physician is as follows: Physician s Name: Ph # Physician s Address: 13. I/We agree that a physician may treat my/our child whenever necessary until other arrangements can be made by me/us and that if a representative of the BIJGA is available and qualified to render first aid, then such representative shall be authorized by me/us to render first aid to care for my/our child. 14. I/WE REPRESENT THAT I/WE HAVE READ AND UNDERSTAND THE CONTENT OF THIS ENROLLMENT AGREEMENT, CONSENT AND AUTHORIZATION. 3

15. I/WE REPRESENT THAT I/WE UNDERSTAND THE NATURE OF THIS ENROLLMENT AGREEMENT, CONSENT AND AUTHORIZATION AND THAT THIS IS NOT A MERE RECITAL. 16. I/WE HAVE HAD AN OPPORTUNITY TO ASK QUESTIONS ABOUT THIS ENROLLMENT AGREEMENT, CONSENT AND AUTHORIZATION AND ANY OF IT CONTENTS. 17. I/WE EXECUTED THIS ENROLMENT AGREEMENT, CONSENT AND AUTHORIZATION AS MY/OUR FREE ACT AND OF MY/OUR FREE WILL. I understand that the BIJGA has allowed my/our child,, to participate in the BIJGA events in reliance upon my/our review and execution of this Enrollment Agreement, Consent and Authorization. BIJGA RULES AND REGULATIONS All Junior Golf participants in the Big Island Junior Golf Association program must abide by the following rules and regulations: 1. Must abide by the USGA Rules of Golf and local rules as announced or posted. 2. Must exercise proper golf etiquette at all times. 3. Be considerate of your fellow competitor and play the game according to the rules. 4. We are guests of the golf courses, and we should conduct ourselves accordingly. We expect courtesy and proper behavior at all times. 5. No loitering or wandering in the pro shop and restaurant-bar area unless you are there to make a purchase. 6. Junior must be at the golf course and check in with the person in charge at least 20 minutes before tee-time or time of departure for outside courses. Penalty 2 strokes. 7. Juniors must wear collared BIJGA golf shirts to all events on the golf course. Penalty 2 strokes. 8. No denim shorts, cargo pants, or surfer shorts. Penalty 2 strokes. 9. Shirts must be tucked in and belts worn if pants/shorts/skorts/shirts and belt loops. Penalty-2 strokes. 10. Jr s in the 9-12 age group will play with walking scorekeepers. Caddies are permitted for ages 8 and under. Players are not allowed to drive golf carts without a driver s license. 11. Friends, guardians, or parents may not accompany or communicate with player during tournament play or club play (unless you are a caddy or scorekeeper). Failure to abide by this rule will result in disqualification of player. Tournament chairperson may give special permission to this rule. 12. Unauthorized PICK-UPS will automatically suspend the junior; only the scorer or tournament chairperson may authorize a PICK-UP at any time. 13. Electronic enhancements such as GPS devices or range finders are not allowed in tournaments. 14. A junior golfer may be reprimanded, penalized, disqualified, suspended or dropped from the program for any of the following infractions: a. Littering on the golf course b. Failure to replace divots on fairways, tees and greens c. Failure to rake sand-traps d. Throwing clubs e. Swearing or using offensive and abusive language. f. Cheating g. Fighting or endangering other junior golfers in any way h. Smoking i. Drinking alcohol beverages j. Use of drugs k. Inappropriate and/or harmful behavior including, but not limited to, insubordination, harassment, or assault l. Stealing, gambling, or vandalism Additional rules and information may be provided by tournament chairperson. 5
