INTRODUCTION: Welcome to Easter Seals Superior California. Easter Seals is a non-profit, community-based health and rehabilitation agency licensed by the County of Sacramento and the State of California. Easter Seals is accredited by the Commission on the Accreditation of Rehabilitation Facilities (CARF). The staff is here to assist you in setting, reaching, and maintaining your warm water therapy goals through our Aquatic Services. Ensuring the quality of care you receive at the Easter Seals Superior California is important to us. If you ever have any questions or concerns, please call us at 916.485.6711, from 8:30 a.m. to 4:30 p.m., Monday through Friday. *Easter Seals reserves the right to refuse services to anyone at anytime for any reason. SERVICES: Warm Water Therapy: Services are conducted in a heated, wheelchair accessible pool, maintained at a range of 92 to 94 degrees. A prescription is required for everyone using the pool. All individuals must be able to function independently in the pool and changing room, or with the assistance of their own aide/family member. Easter Seals pool staff does not assist clients with their personal care needs. Pool Pal Program: Easter Seals requires that you provide your own aide/family member if you are unable to use the pool independently. However, you may set up Pool Pal sessions with one of our Aquatic Rehabilitation Assistants to help you in the water. The fee for a Pool Pal is $15 per half hour; $25.00 for one hour sessions. When clients sign-up for a Pool Pal, they will be charged whether they attend their scheduled session or not. For more information, please contact the Director of Aquatic Services or our Admissions Coordinator at 916.485.6711, ext. 109 to schedule an appointment to sign a Pool Pal Agreement. ENTRANCE CRITERIA: Clients must be 18 years old to use the pool independently. If they are under 18 years, they must have a parent/guardian with them at all times in the pool, or enroll in our Pool Pal Program. A photo I.D. is required. Clients must pass the Megan s Law screening process. All clients are required to have an active physician s prescription or a Chiropractor signed Pool Authorization on file with Easter Seals prior to receiving services. For emergency purposes Easter Seals needs to maintain a current listing of all of your medications. Please make sure you notify the Director of Aquatics with any changes as they occur. All clients must be continent (Full control of bladder and bowels). All pool clients must schedule and complete an Intake/pool orientation with the Easter Seals Admissions Coordinator. A photo I.D., your physician s prescription, and other related documents are required. CONTINUATION OF SERVICES: Clients are responsible for renewing their prescription before the expiration date. Clients cannot use the pool facilities after their prescription has expired. Clients will be notified by mail 30 days prior to their prescription expiring. - 3 -

DAILY CHECKING IN/OUT PROCEDURES: For safety reasons all pool clients must check in and out. Easter Seals requires everyone to use their bar code card. Upon completing your orientation session, Easter Seals staff will issue you a bar code card. The bar code card must be scanned prior to your pool therapy session and at the completion of your session. If you do not scan your bar code card, the following may occur: 1. Written warning for your first offense, which will be placed in your file. 2. Termination from the pool services after three (3) written warnings in a 12 month period. AQUATIC REHABILITATION ASSISTANTS: The Easter Seals Warm Water Therapy Pool Aquatic Rehabilitation Assistants are not licensed Physical Therapists, but they are trained in the following: Pool Safety Child and Adult First Aid and CPR and AED Hoyer Lift Wench Gait Training Proper Exercise Techniques Each of our Aquatic Rehabilitation Assistants spends more than 15 hours a week working 1:1 with clients to develop an individualized, person-centered therapy routine. The Easter Seals Aquatic Staff works as a team to provide quality services. CLIENT ORIENTATION: The purpose of the new client or returning client orientations is to introduce new pool clients to the pool environment. Aquatic staff meets with the clients to create a personalized exercise plan and review pool rules, regulations and daily checking in/out procedures. SCHEDULING AND RESCHEDULING: The Admissions Staff will schedule an intake appointment that will cover intake paperwork, payment options, and a pool orientation. You will be provided with a reminder telephone call two days prior to the orientation appointment. Easter Seals wants to ensure all clients receive ample time to complete all aspects of their orientation. However, if a client does not keep his/her appointment the following rules will apply: We reserve the right to reschedule your appointment if the required information is incomplete or if you are late. In addition, if a client is late for the appointment, the orientation time may be reduced. If a client is unable to keep his/her appointment, they must call the Admissions Staff at 916-485-6711 24 hours prior to their appointment time to reschedule their orientation. After three (3) unexcused enrollment appointments the individual will need to wait one year before reapplying for services. Special accommodations for missed enrollment appointments will be taken under advisement by the Director of Aquatics. (Such as medical excuses from a physician) - 4 -

MEDICAL HOLD: If you are unable to use the pool due to serious medical conditions, proper documentation from your physician, along with a letter to hold EFT status should be directed to the Finance Department for consideration of a possible adjustment. The Finance Department needs a new EFT form upon your return. Director of Aquatic Services will need a release from your physician when you are able to return to the pool program. FEE STRUCTURES AND OPTION PLANS: Easter Seals charges fees that are reasonable and fall within the acceptable range. Bills are mailed monthly and are due and payable within 15 days to Easter Seals Superior California. Payment for services is due upon receipt of your bill unless payment arrangements have been approved in advance by our Finance Department. We accept cash, checks, and preauthorized payments Electronic Fund Transfer (EFT), MasterCard, and Visa or through Bank accounts. ENROLLMENT FEE: A non refundable $30.00 enrollment fee is required at your intake appointment. OPTION ONE: THE ELECTRONIC FUND TRANSFER (EFT) option is available based on the EFT plan. The pool fee is $55.00 per month, and is billed whether clients attend the pool program or not. The $55.00 fee will be automatically taken out of client s checking or charge card accounts on the 15 th of every month. A 30 day written notice is required to cancel your EFT plan. Pool fees will default to $65.00 per month upon cancellation of an established EFT plan. Upon cancellation of your EFT plan, clients cannot sign up for EFT for one (1) year. OPTION TWO: MONTHLY FLAT RATE CHARGE. Pool services are billed on a monthly basis; therefore, if you attend the pool any day during a calendar month (example: October 1 to October 31), you will be billed the entire monthly fee of $65.00. Clients on a sliding scale fee agreement who choose Option Two will be billed an additional $5.00 per month Service Fee. OPTION THREE: PRE PAYMENT. Clients may prepay the full fee of $325.00 for five (5) consecutive months and receive the sixth (6 th ) month free. Whether you attend or not. Note: Clients who prepay under Option 3 for a total of six (6) consecutive months of service will have their payment applied only to those consecutive months. Clients may not apply any unused pool time to a period beyond their initial six (6) month timeframe. * Returned checks, returned preauthorized payments (EFT) and balances older than 30 days may be subject to additional collection fees of $20.00 a month. * Returned preauthorization payments (EFT) payments will result in charges defaulting to the full fee price. - 5 -

POOL OPERATIONS: Pool Hours: Spring - Summer Season: (Daylight Savings Time) Monday through Friday 7:00am to 7:00pm Saturday 8:00am to 1:00pm Fall - Winter Season: (Standard Time) Monday through Friday 7:00am to 6:30pm Saturday 8:00am to 1:00pm Note: Clients may not enter the pool less than 30 minutes prior to closing time and will leave the water 30 minutes prior to closure. Pool Closures: The pool may close early due to severe weather conditions. The pool remains open during rainstorms, but for your safety, may close without warning during lightning storms. Holiday closures and other time changes will be posted on the pool doors. The pool may also close for periodic maintenance. ANNUAL POOL CLOSURE: To ensure the health and safety of all pool customers and compliance of state and county requirements, Easter Seals will have an annual maintenance, upgrade, and renovation closure. The annual pool closure will be announced and posted at least three (3) weeks in advance of the closure. * THE ANNUAL POOL CLOSURE WILL NOT RESULT IN PRORATED POOL FEES DURING CLOSURE DATES CONDUCT: Easter Seals respects the dignity of all individuals and expects individuals served to treat everyone with dignity and respect as well. While at the Easter Seals Facility, Easter Seals expects all pool clients to conduct themselves in a courteous manner which may include, refraining from loud, abusive, offensive, insulting, demeaning language, profanity, lewd conduct or any conduct that harasses or is bothersome to other clients, guests or staff. Easter Seals is open to suggestions and recommendations to improve the pool environment, however, Easter Seals will not adhere to any individual (s) encouraging bulling or coercion type behavior. (Refer to the Appeals Procedure) SUSPENSION/TERMINATION: Easter Seals has the sole discretion whether or not an individual may continue pool therapy services, if he/she violates Easter Seals or Warm Water Therapy Pool policies and /or rules or if a client has engaged in any conduct that Easter Seals considers being inappropriate or unacceptable. Based on the severity of the violation, a pool client may be terminated immediately from pool services. - 6 -

POOL RULES: 1. All clients requesting an aide must have a written authorization from their referring physician. 2. Aides are required to know and abide by pool rules and follow the directions of all Aquatic Staff. The client or client s aide must report all incidents (falls and/or injuries) to a staff member immediately, even if medical attention is not needed. 3. Proper swimwear attire is expected at all times. No cut-offs, frayed or street clothes are permitted. 4. Per Easter Seals Infection Control Policy, Easter Seals prohibits clients with oxygen tanks, open lesions, and/or contagious rashes to enter the water. This includes Athlete's Foot Fungus. 5. To protect the health and safety of all pool clients, adults who are incontinent or wearing disposal under garments, catheters or external colostomy bags are not permitted in the water. 6. Clients with a bout of diarrhea are required to remain out of the pool for two weeks after the symptoms are cleared up. 7. Oils, Hairpins, Band-Aids, Gum, etc., are not permitted in the pool at any time as they can damage the pump and filter system, resulting in pool closures and costly repairs. Wear only waterproof sunscreen. Please refrain from the use of aerosols or powders in the locker room. 8. Using Pool shoes/socks can prevent accidents. Please use caution in and around the pool area and dressing rooms at all times. Pool deck surfaces and floors may be wet and slippery. 9. To prevent slipping in the shower rooms, bar soap is not permitted. Liquid soap is provided. 10. Leave all valuables at home, or lock them in your vehicle. Easter Seals is not responsible for items brought into the pool area. 11. Food, beverages, cans, glass bottles, and tobacco are not permitted in the pool area. Only water in a plastic bottle will be allowed poolside. 12. The pool is 3 to 5 feet deep. No diving is permitted. Please enter the pool by the ramp, or the two sets of stairs at the South end. Please do not use the ladders at the North end, as these are emergency exits only. 13. Special rules apply to lap swimmers. Please review these rules prior to participating in lap swimming. A copy of the rules can be obtained by asking Aquatic staff. 14. To protect pool clients from over exertion, undue fatigue and overexposure to pool chemicals, clients may only use the pool one time per day for up to three (3) hours at a time. 15. Everyone must shower before entering the pool. 16. Lockers are available in each dressing room for use while you are using the pool. Articles are not to be left in the lockers once your session is completed. Locks are permitted during your workout time only. Do not leave them overnight. Articles left in lockers at the end of each day will be placed in the Lost and Found box by the Pool Office. Articles left in the Lost and Found box will be donated to a charity at the end of each month. Easter Seals is not liable for any personal items lost or stolen. 17. Pool equipment is provided as a courtesy for everyone s use. Please share in the responsibility of maintaining and storing the equipment properly. Please return kickboards, weights and equipment to a safe location on the pool deck or storage area. - 7 -

18. Please leave all pool maintenance to Easter Seal Maintenance staff. 19. Any fund raising, sales or soliciting activities must be approved by the Easter Seals President/CEO. 20. Easter Seals is a Drug Free/Alcohol Free Facility. Anyone exhibiting signs of being under the influence of drugs and/or alcohol will be prohibited from entering the pool area, and may be terminated from pool services. 21. Any behaviors that undermine the therapeutic atmosphere at the pool or intimidate, threaten, frighten and/or present a safety hazard to self or others, are unacceptable. Clients are required to act with courtesy and respect for one another and Aquatic Pool staff. 22. To ensure the health and safety of all Easter Seals clients, staff, visitors, all incidents must be reported immediately to staff. Staff is then able to determine if additional medical treatment is required. 23. Easter Seals Warm Water Therapy Pool is maintained at 92 degrees, but may vary from day-to-day from approximately 91 to 94 degrees. If you feel chilled, please get out of the pool. When water is warmer than 93 degrees, please exercise at a slower pace and limit your time in the water. *If a client violates the rules of the pool, the Aquatic staff may require the person(s) involved to leave the pool immediately to prevent disruption of services to other users. *Violation of pool rules may be cause for temporary exclusion from the pool. Repeated violations may result in termination of pool use. *Any disagreement with Aquatic Staff is to be directed to the Director of Aquatic Services. *Rules are subject to change and/or may be revised at the discretion of Easter Seals. If you have any questions, comments or concerns about your warm water therapy or exercise program, please contact the Director of Aquatics Services at 916.679.3160. SAFETY: It is the intention of the Easter Seals Management to provide safe and healthy environment and to establish and insist upon safe practices at all times by the pool clients. When entering the Easter Seals Center parking lot you must come to a complete stop. Upon entering the parking lot proceed to the RIGHT and follow the arrows directing you in a counterclockwise direction. Be respectful and courteous of others. Unusual Incidents (falls and/or injuries): Must be reported to a staff person immediately, even if medical attention is not required. For safety reasons pedestrians are to use the sidewalks as a point of entry or exit. Smoking is only permitted outside designated smoking area near the Easter Seals Enterprises building. - 8 -

APPEALS PROCEDURE If there is a program-related question, complaint, or problem, please relate it to the Director of Aquatic Services. If uncomfortable doing this, or the Director is the source of the problem, condones the problem, or ignores the problem, please report the problem to the Vice President of Adult Services to review your concerns and/or issues. The Vice President of Adult Services will respond in writing within ten working days of meeting with you. The VP of Adult Services will return a response in writing. If this alternative is not satisfactory, then please direct your questions, concerns, complaints, and/or problems to the President/CEO by sending a letter to the above address. The President/CEO upon gathering all information will respond in writing within 30 days of receiving the complaint. CONCLUSION Easter Seals depends upon community support in order to provide services for those unable to pay full fee. We hope that you will continue to support our organization when your warm water therapy is completed and our services are no longer required.
