Stevenson High School Hockey Association, Inc. (SHSHA) P.O. Box 542 Lincolnshire, IL 60069 To: Subject: 2011-2012 Prospective Players and Parents Tryout Information, Required Forms and Fees Tryout Information The Stevenson High School Ice Hockey team is an official Stevenson High School sport. There will be both a Varsity and Junior Varsity (JV) team. In order to participate in team tryouts you are required to turn in both of the following Stevenson High School forms PRIOR to tryouts: 1. Athletic Participation Form Indicate your choice of Ice Hockey, which is considered a winter sport 2. Athletic Physical Form These forms can be found either at Stevenson High School s website or in the summer issue of the Minuteman publication. The completed forms must be turned in to the Stevenson High School Athletic Office. All forms should be turned in no later than Tuesday August 23 rd. Stevenson High School will not allow any student /athlete to participate on the ice for tryouts unless these forms are turned in to the athletic office There are NO EXCEPTIONS!!! All players must have full hockey equipment, including a mouth guard and neck guard. Players should bring a light-colored jersey and a dark-colored jersey to the tryouts. All tryouts will be held at: Twin Rinks Ice Pavilion 1550 Abbott Court, Buffalo Grove (Entrance on Weiland just south of Aptakisic) The tryout dates and times are as follows: Tuesday August 23rd - Registration only 6:00pm 7:30 (players can participate in the scheduled summer clinic) On Ice Saturday August 27th 12:00pm 1:30 Last names A L 1:40pm 3:10 Last names M Z Sunday August 28th 12:00pm 1:30 Group I 1:40pm 3:10 Group II Tuesday August 30th 7:00pm 8:30 Group I 8:40pm 10:10 Group II Thursday September 1st 7:00pm 8:30 Group I 8:40pm 10:10 Group II

The group breakdowns will be determined by the coaches and will be posted at the rink and on the website after the last session each day. For example the groups for Sunday August 28th will be posted at Twin Rinks after the end of the last skate on Saturday August 27th. The final team selections will be posted at Stevenson High School outside of the Athletic Directors office late in the morning on Friday, September 2 nd. Selections are indicated by student ID #. Required Forms There are additional forms that must be completed, which are required either by SHSHA or Chicago Metropolitan High School Hockey League (CMHSHL), as follows: Fees - SHSHA tryout application, hold harmless agreement and (if applicable) upperclassmen selection form (all attached) which require signatures by both the player and a parent - A copy of the player s USA Hockey online registration receipt. - CMHSHL participation form. This form will be provided to the players after team selections have been determined. These are required to be completed, including the doctor s authorization section, in order to participate in league games. The total (non-refundable) tryout fee is $225, which includes the $43.00 USA Hockey Registration fee you are required to pay directly to USA Hockey. The remainder of the tryout fee ($182.00) is due with your Tryout Application. The player fee for the 2011 2012 season is $3,450.00, which includes a $200.00 Ad Book Minimum (see below). A check for one-half of the season fee ($1725.00) is due with your Tryout Application - to be refunded should a player not be selected for ANY team. If a player quits after they have been selected to ANY team they will forfeit their $1,725.00 payment and it will NOT be refunded. The second installment of the season fees, in the amount of $862.50, is due by October 10th, 2011. The third and final installment of the season fees, in the amount of $862.50, is due by January 14 th, 2012. If fees are not paid when due the player will be suspended and will not be able to participate until all fees are current. Please make out separate checks, payable to Stevenson High School Hockey Association (or SHSHA), for each (tryout fee and ½ of the season fee). A transaction fee of $25 will be charged for any check returned for insufficient funds. We are unable to accept credit card payments. A $200 minimum Ad Book fee for each player is INCLUDED in the season fees. We hope that players will participate in Ad Book/Yearbook fund raising activity as it allows families to earn a refund equal to 70% of ads they solicit IN EXCESS of the $200 minimum (up to a maximum of $3,450.00). Hold Harmless Agreement We recognize and acknowledge that ice hockey, as well as other sports, is a game in which there are risks of injury to the participants. Because of this, and desiring that the minor named below participate as a player with the Stevenson High School Hockey Association, Inc., (SHSHA) and in consideration of his/her enrollment and/or participation, we agree that we shall indemnify and hold SHSHA, it s officers, agents and coaches harmless from any and all liability for the damage because of injury of otherwise sustained by the minor named below; arising directly or indirectly out of or in connection with his/her enrollment and/or participation as a player with SHSHA during the 2011 2012 season Player Name (please print) Signature of Player Signature of Parent Hospital Release In the event of injury, I hereby give my permission to hospitalize and secure proper treatment for, and to order injection, anesthesia, or surgery for the above named minor. To: Stevenson Fall Sport Participants Subject: Conflict with fall sports Stevenson High School Hockey Association, Inc. (SHSHA) P.O. Box 542 Lincolnshire, IL 60069 Although hockey is a winter sport, practices will begin immediately after tryouts, and the first league games begin before some Stevenson fall sports are over. According to State High School rules, students may participate in only one interscholastic sport at a time. For example, a student may not play on school teams for both soccer and football in the fall. Participation in the SHSHA hockey program is not a violation of the State sport rule. However, individual Stevenson team coaches may establish team rules that restrict participation in other athletic activities. Restrictions, if any, vary by coach. As a fall sport participant, we ask that you observe the following: 1. All activities associated with your fall sport come before hockey. Also, follow any participation restrictions given by your coaches. Violating fall sport rules will not help you or your hockey team. 2. With the time and energy commitments required of a fall sport, it is important to insure adequate time and energy are devoted to academics. Please do not hesitate to restrict your hockey involvement during the fall sports season for academic reasons. 3. Given 1 and 2 above, please make every effort to attend tryouts and try to participate in as much of the fall hockey program as you can. Communication is the key!!! It is your responsibility to keep your hockey coach fully informed of any hockey activities you will miss. We understand that you may feel that you will fall behind by missing hockey practices. Remember, the general physical conditioning provided by your fall sport, combined with some extra effort on your part, should help you catch up quickly after your fall sport ends. Team Tryout Form - 2011-2012 Season **THIS FORM IS FOR UPPERCLASSMEN ONLY** 1. Upperclassmen are defined by SHSHA as a Senior or a Junior. 2. Upperclassmen who select Varsity only will be considered only for Varsity and not for JV. 3. Upperclassmen who select both Varsity and JV and who do not make Varsity will be considered for JV based on the following: a. Ability b. Leadership c. Team Needs SHS coaching staff will select players to the appropriate team. Please check the team you are trying out for. If you are trying out for Varsity, and do not make the team but you are willing to play for the JV team please check off the box for Varsity or JV. Players could be rostered on both the Varsity and JV team, and at the coach s discretion, could be told to participate at either level.
