United Gymstars & Cheer Week-long Day Program Monday, June 5 th Friday, August 4 th Our fantastic summer program features non-stop fun and excitement for your child. They will enjoy gymnastics, tumbling, soccer, basketball, volleyball, relays, fitness activities, organized games, trampoline, open gym time, outside water play, arts and crafts, movie time and much more. Each week has a theme with activities, crafts, and games! Mondays Fridays, 7:30 AM to 5:30 PM In order to guarantee your spot, payment must be received in advance. Summer Program is on a first come basis. We have LIMITED AVAILABILITY. Payment is to be received on the first day of drop off for each week if not paid in advance Each camper is required to bring a snack and a sack lunch daily An afternoon snack and drink will be provided Additional snacks and drinks available for purchase from the concession stand

I specifically understand that I am releasing, discharging, and waiving any claims or actions that I may have presently or in the future for negligent acts or other conduct by the owners, agents, officers, or employees of United Gymstars &Cheer, LLC. I understand and acknowledge that no medical insurance benefits will be provided to me during this activity and that I will be responsible for any medical bills that may result from my participating in gymnastics and/cheerleading. I, being the parent or guardian of the student, do hereby authorize United Gymstars & Cheer, LLC, its coaches, trainers, or any member of its staff, to obtain emergency medical treatment from any physician, hospital, or other qualified medical personnel or facility as needed in the event of accident or injury. I also agree to be responsible for all the costs of said emergency treatment. I, being the parent or guardian of the student, do hereby authorize my child s photograph to be used in advertisement for United Gymstars and Cheer, LLC.
