GSSC Academy Registration Instructions If you have new player you must email the uniform coordinator and the office administrator. Register team with playing League. o Primetime (this is what we budget off of, and we only pay for age appropriate group) o EAL & CFBAL (we will only pay the League fees, as long as they stay at the pricing of Primetime) o SDL (is the teams responsibility unless this is the ONLY League being played then we will pay what the team would have under Primetime pricing) o PAL (GSSC will only pay up to Primetime pricing for same league play) Any other Leagues needs approval first from DOC GSSC will only reimburse/pay for ONE approved league a season. Coach/Team Manager must email Treasurer copy of Comfirmation from League with confirmation number and amount charged. o Treasurer: rat6971@tx.rr.com New Players during season ( Aug. Dec and Jan. On North Texas Tournaments they will need to be completed in the office with the Office Admin (signature and NTSSA assigned number) Medical Releases, there are 2, we require the NTSSA & USCS. Parents Must Complete Parent Code of Conduct form in the Academy Registration Packet o http://www.gsscthunder.com/linkclick.aspx?fileticket=798gxlrrnve% 3d&tabid=182787&mid=206639 If you have less than 10 players per team you must have approval from the DOC of the Club before you register more than 1 team for League Play. o U6 (5v5) Team must have 8 min - 10 max o U7- U8 (7v7) Team must have 10 min - 12 max. o U9- U10 (9v9) Team must have 12 min 14 max o U9- U10 (11v11) Team must have 14 min 16 max Anything different from the above numbers needs approval from DOC (Barry Mitchell) and/or GSSC Committee. All Coaches/Team Managers are responsible for making sure all documentation is completed for registrations (League/Tournaments). Anything different than what is listed above for instructions must have prior approval from the GSSC Committee or DOC. When ALL of the above has been done, place in the large white envelope with your team name, the following: Academy Registration Packet for every player, Including payment form Volunteer sheet for ALL coaches and team managers Copies of ALL birth certificates, new and returning players (keep a copy for your records)

US CLUB SOCCER REGISTRATION INSTRUCTIONS Each team must enter all players and make sure Background checks are completed. http://www.usclubsoccer.org/ ***Member Login: Email Address: Coaches/Managers email address (all small letters) Password: Coaches/Managers last name (all small letters) ***Coaches, Assistant Coaches, & Team Managers must complete Risk Mgmt./Background Check. Click on the Coaches Corner Tab

***Then click on Risk Mgmt./Background Check on the left side of the screen ***You must then pick League team, then in the first drop down box pick PrimeTime Sports Academy League (TX), then in the next drop down box pick GSSC (TX) Club #6113

WHO MUST SUBMIT A STAFF BACKGROUND CHECK: All staff who are required to be registered and are coaching players U-20 and below must complete the application. This includes all coaches, assistant coaches and team managers assigned to a competitive team. If your team is registered as a recreational team or super rec team pursuant to US Club Soccer policies (i.e. teams that do not travel, or do not play in competitions outside the club, in-house league, or other similar programs in their area), minimally the head coach is required to be registered and complete a background check. (See Player Rule 105 and Policy 11.01). These staff members may not participate with a team until a background check is completed and a staff passcard has been issued. WHEN THE STAFF BACKGROUND CHECK MUST BE COMPLETED: The staff background check application must be submitted every two years - after July 1 in the year the passcard is to be issued or expires. Staff passcards will not be issued until the individual s application is reviewed and approved. Rec coaches must only complete an application in the first year they are registered via the rec roster. OTHER NOTES: 1. There is no charge for submitting the staff background check application. 2. You do not need a login and password to access the application. Once completed, a new staff passcard and background check is good for at least two years. 3. Be sure to designate the correct club, academy, or team with which you participate. If your club or team is a member of US Club Soccer through an affiliate league, select the league from the dropdown list, then your team. If your team is playing in a state cup and your organization is not otherwise a member, select the correct competition name and then your team from the dropdown. 4. You must complete this form yourself. Your registrar cannot do this for you. Complete the form in its entirety, read the language at the bottom, click I Agree, and submit. If you bypass any of these steps, the form will not be submitted. 5. Once your club or team requests your staff passcard, the background check will be processed. Your registrar will be able to see when you have submitted your application and when it is approved (but not the background check information). Nevertheless, you may want to tell them when you have completed the application so they can promptly request your staff passcard if they have not done so already. 6. We will contact you if we have any further questions or concerns. Please note that if we ask you for any more documentation, it will be kept confidential within our admin department and not shared with third parties. Otherwise, after the background check is reviewed and cleared, your registrar will be authorized to print your staff passcard. If you have more than one team (example 02 Boys & 02 Boys B), you need to go to the team management tab and make sure both teams are listed.

UPDATE A PLAYER: **Now you need to go to the players management tab and pick the team you are adding the player to. Then click the view ADD A PLAYER(S) ONE OR MORE AT A TIME: 7. To add a player(s), click Add Player button. 8. Choose a team from the Team Drop Down box [full teams are grayed out]. 9. Complete the information on the form for each player. [Note: parent name, e-mail, etc., is not required information for players] 10. Due to the speed of your computer and internet connection, if you are attaching POBs in this step, add up to the limit of 12 players. Click the Add Player button at top right of the form. ***If you entered the player last year/season then they will already be listed and they can be updated. If there is a player listed that needs to be removed it must be submitted to the Office Administrator info@gsscsoccer.com and Ronnie Qualls ronniequalls@gsscthunder.com. You will be responsible for collecting team dues for everyone listed.

11. To update a player, click the View button associated with the player's team. 12. Click the Details button for the player in question. 13. Amend the player's information. Click the Update button. ***Every player must have a copy of his or her Proof of Birth attached. Now you can go back to the team then play and click on the details tab for the player and then at the bottom of the page click on Choose File next to Upload POB. Uploading Proof of Birth It is expected that for all new player requests, the Proof of Birth (POB) will be attached to the player's profile as a.pdf file. Note this is not necessary if a player has been issued a card previously (i.e. a date appears in the Passcard Issued column). In fact, the Browse button on the Upload POB function at the bottom of the individual player s profile will be disabled for players with cards. INSTRUCTIONS: 14. Using a scanning device, scan the original or good quality copy of your player's proof of birth document as a.pdf file (birth certificate, driver's license or passport). Note that each player must be scanned and saved as a separate file. Size restrictions: Restrict the document's size to no larger than a standard sheet of paper (8 ½ x 11) Resolution: 72 to 150 dpi File overall size: Uploaded document should be no larger than 700kb (this will be plenty of space for your POB documentation). File types allowed:.pdf,.jpg,.jpeg and.gif 15. 16. Save this file as a.pdf and store it on your computer, using the player's name as the unique identifier (i.e. Smith- John.pdf). Please be sure to note which folder you have saved it in, in order to retrieve it later. 17. If you do not have the capability to scan documents, you can rely on the services of Kinko's, Office Depot, Office Max and other printing facilities that provide these types of services for a small fee. 18. Once you have saved the POB, you may attach it to the individual player's profile. Go to Player Management Click on the player's team. Click on the individual player. Browse and attach the appropriate document (the browse function is at the bottom of the profiles of all players who have not been issued a card previously). Update the player by clicking at the bottom (UPDATE PLAYER). 19. 20. When you are ready to request player cards, on the Passcards/Release screen, you can see which players have a POB attached (indicates Attached on the right hand side; or note the View POB to the right side of the birth date on the player s profile). The POB will be sent to us automatically for our review when you request the passcard. If the POB is accepted by us, Attached will change to Validated on the Passcards/Release screen, and the passcard can be issued. 21. Note that in case of an error, etc., you can change the player s POB up until the time the POB is reviewed and verified by our office. Thereafter the POB is locked into the player s profile until you electronically release the player. 22. We're hoping everyone will take advantage of this procedure, as it will cut down on mailing and significantly speed up card processing. If you wish to continue to send your POBs via postal mail, we may apply a $2/card surcharge at some time in the future for the additional processing.

HELPFUL HINTS FOR ENTERING DATA: 23. The state field must be the two-letter postal abbreviation in capital letters [ex: CA, SC, MD, etc.] 24. Date of Birth must be entered as mm/dd/yyyy. 25. E-mail address is NOT a required field for players and if you enter one make sure it is unique to all the others in your Web site. If you are having difficulty adding a player take out the e-mail address as if may have been entered for a sibling or parent. 26. Remember uploaded POBs must be sized accordingly [see link] and saved as a.pdf or.jpeg file type. 27. U.S. Citizen column should be Yes or No answers. 28. Gender column should be F or M answers. Phone numbers should be entered as: (843) 429-0006 or 843-429-0006. ***Now you need to print an Official Roster OBTAIN AN OFFICIAL ROSTER: 29. Click the View button contained within the Player Management tab. 30. Once the team has loaded on the page, click the Print Roster icon at the top of that team s page. The roster loads in Adobe format. Click below to obtain a free copy of Adobe Reader if you do not already have one. ***Next- Email the Office Administrator (info@gsscsoccer.org ) and Ronnie Qualls ronniequalls@gsscthunder.com and request Player and Team (coaches, assistants, team manager) Passcards. Please have the Team Roster, Player & Team Passcards at all games. This info can be requested at any time.
