Newcomers to TMHA not registered prior to Sept 7/2017 will pay rates as of July 1, 2017. ROOKIE PROGRAM: If this is the first time this player is in a Minor Hockey program, then TMHA waives registration fees, for Initiation, Novice & Atom levels ONLY. Rookie program fees only include insurance costs ($40); concession duty and all fundraisers are still required. WHAT TO BRING TO REGISTRATION - All players new to TMHA need to bring to registration: 1. Copy of child s birth certificate and Alberta Health Care #. 2. Proof of completion of online Respect in Sport Parent Program. 3. Full address information including mailing, physical (emergency) AND legal addresses. 4. AT least 4 cheques for fees, fundraising, concession and AGM deposits. Your completed registration form MUST include fees and postdated cheques, and your respect in sport must be completed before your child is considered registered for the 2017-2018 season. TMHA is under no obligation to release a player from its organization because the player does not meet the registration deadline(s). CONCESSION DUTY OPTIONS A: I wish to payout my concession duty: PDC dated October 15, 2017, for $225.00 for each registered player. Concession Deposit Signature: Date: This cheque will be cashed if you do not work your designated shifts. If you cannot work a shift switching with another parent or hiring an approved worker is acceptable. You will be required to pay worker Alberta Minimum Wage for each hour of the scheduled shift. This is to be paid directly to the worker if possible and a Shift Hire Form or Shift Exchange Form MUST be filled out every time and put into the shift form box. Concession Manager, VP Off Ice or any other executive member will not be filling shifts that you can no longer work. All Shifts are the responsibility of the player s parent to fill themselves. Exceptions may be made for emergency situations such as hospitalization, accidents, family death. *** PLEASE NOTE *** Concession shifts for the entire season to the best of executive members knowledge will be available for signup starting at early registration and will be first come first served. Sign up at this time is not mandatory however, be aware that choice of dates will be limited the longer you wait, and waiting will not reduce the number of shifts you will work this season. You will be required to work 16 hours per registered player to a Maximum of 32 hours per family. Note that shift times, dates or cancellation may be necessary throughout the season. ***PLEASE NOTE: Persons under the age of 15 are strictly prohibited from entering the concession.*** RESPECT IN SPORT (RIS) (needs only to be completed once and is valid for subsequent years) Before any player can register with TMHA, the Respect In Sport Parent Module must be completed. Proof of completion (actual certificate or completion code) is required at registration. The program cost to you is only $12.00 plus G.S.T. FUNDRAISING OPTIONS The following are the fundraising activities for the 2017/2018 season. If you choose not to payout the fundraiser you will be required to complete ALL of the following: **Please note- fundraising commitments are to a maximum of 2 players per family. 1. Raffle Tickets Each player will be required to sell 20-25 - $10 Raffle tickets. The raffle prize this year will be a Daily Cash Calendar for the entire month of February. Please see the prize payouts on the Parent Information sheets at the back of the package. 2. Volunteer Time- 5 options to choose from Must Choose One (first come first served) Ag-Society Events, Skate Sharpening or Tofield Men s Tournament - RyTof security Sept 2 2017, Bull Bash security- April 2018, Rodeo security-june 2018 Approx. 4 hours total per player at one of the above events. Skate Sharpening - throughout the entire hockey season (similar to last year). Only2-3 people per team will be considered. Tofield Men s Hockey Tournament- November 3-5, 2017 Bar shifts throughout the weekend (shifts to be announced) **Casino and Oilers 50/50 In the event that TMHA is selected to work the casino or Oilers game you may be required to volunteer even if you paid out your fundraising as per Policies & Procedures 18.2. If obligation for both of the fundraising events is not met your deposit cheque will be cashed at the discretion of the TMHA board. Please choose ONE of the following options: A) I do not wish to participate in the TMHA Fundraiser. I will provide a $275.00 cheque dated Sept 07, 2017, with my registration fee payment or first installment, for each registered participant. Fundraiser Payout Signature: B) I agree to participate in the TMHA FUNDRAISER. I will provide a $350.00 PDC for March 1, 2018 for each registered player. I understand that this cheque will be cashed if I do not meet my fundraising obligation. I understand that I am responsible for 100% of the fundraising product and its expected revenue and that I cannot return unsold product. All volunteer obligations must also be completed. TOFIELD MINOR HOCKEY ASSOCIATION ANNUAL GENERAL MEETING The AGM is set for 7:00 p.m. on May 16, 2018. This is where TMHA members have a voice and a vote. TMHA cannot function without the support of its members. It is expected that a representative from each family will attend. A $100 membership fee per family in the form of a PDC dated May 16, 2018 is required with your registration. When you attend and properly sign in and out of the AGM your cheque will be shredded unless otherwise requested. Should you not sign in and out of the AGM your deposit cheque will be cashed. Cancellation Policy (as per TMHA policy and procedures 16.2) Players withdrawing from TMHA will be paid on a pro rated basis. Written requests must be sent to the registrar. The treasurer and registrar will determine the fee to be refunded. There will be a $50 processing fee on all refunds.
