Preparing for All Stars Helpful tips for a smooth document collection process Read over these document thoroughly and be prepared. Do not put this process off until the last minute. This is an important process to get your teams eligible. All Star Binders Process Team All Star Binder for Each Division One of the main responsibilities you have as a league player agent is to assemble the All Star Teams Binders for each division of Softball and Baseball. It is strongly recommended that you divide this responsibility up. If you follow this recommended format your league will be prepared for the All Star competition. It is also your responsibility to communicate to the All Star Parents group exactly what is expected. Remind the parents this documentation is required as part of the Little League rules.

Team Affidavit Form This is a check form for District personnel to review all player documents. The online form is forth coming. This form should be placed in the front of your All Star binder. Make sure you use the correct sport specific affidavit for Softball or Baseball. Make sure this form has all accurate information on the players. You will also need to make sure the League President signs off on this form.

League Map You must include a league map that includes the following; Your league boundaries The addresses for all players on this specific divisional team All addresses must be plotted and shown on the map Any players outside of your boundaries must have waivers explaining their eligibility. The online form is forth coming.

Tournament Enrollment Form The Tournament Enrollment form can be accessed through LittleLeague.org Once this process is completed you will receive a receipt. Keep a copy of this receipt in each Binder

Charter Information Report This form can be found on the Little League Data Center. This Charter Information Report form verifies that your league charted at least one team in the division this player will be participating in. For example, if this binder is for a Softball Juniors team, this form will verify that your league charted a team for the season in that division.

Certificate of Insurance This form is your league insurance policy. This must be included in this packet to verify proof of insurance to verify that your league is up today with their insurance.

Waivers You must include all waivers that for each specific team that applies. Some examples would be: Approved Combined Team forms Waivers II(d) and IV(h) forms

All Star Packet Check List 1 per player 1 clear 8-1/2 x 11 sleeve Little League International Baseball and Softball Tournament Player Verification Player Birth Certificate Proof of residence, multiple proofs on one page is preferred Little League International Model Release Little League International Medical Release, back page face exterior of packet so it s visible

Confidentiality All documents collected should be treated as confidential information. Please make sure your Managers understand this. Binders contain private information and should be kept in a safe place. Content should be shared with no one other than the Tournament Site Director It is the manager s responsibility to take care of these binders and make sure they get signed off at each tournament and kept in a secure place. Once your team has concluded their tournament play, this information should be returned to their League Player Agent so this information can be stored if needed, or shredded if not needed.

Player Verification Form This form verifies the information is correct and should be completed by the parents for each new player. Remember to include the League s Name and ID number at the top of the form. Your league President needs to sign off on this form. The parents and league each need to keep a copy. If a player is returning from a previous All Star season, all that is needed is their approved packet of information.

Birth Certificate Little League requires that the ORIGINAL state produced birth certificate must be presented (along with copies) for the District Administrator to verify. Once the ORIGINAL Birth Certificate has been verified by the District Administrator it can be returned to the parents. Hospital certificates and California Abstracts are not accepted.

Proof of Residence Little League is very strict regarding their Proof of Residence Requirements Make sure you have the proper documents within the required time frame. Proof of Residence documents must be dates February 1 st of the current year, one year back to the previous year. Valid drivers license Car insurance Car registration

School Enrollment Form A player who attends school within the boundaries of an established local league is eligible to participate in that league. The player must be attending school during the traditional academic year on a regular basis in order to be eligible to participate in the established local league. Once established, the location of school/local league shall not be considered changed unless the child is enrolled and attends another school or is no longer enrolled in the previous school. School attendance shall be supported by a documentation dated prior to October 1, 2015 from ONE of the following categories to established school attendance. 1. Official/Certified School enrollment record 2. School issued report card or performance record 3. A Little League issued school attendance form completed by the principal, assistant principal or administrator

Player Medical Form A completed medical release form is required for each player In years past a medical forms had to be notarized. This is no longer a requirement.

Photo/Model Release Form Please have parent sign the photo release form.

Previous All Star Paperwork Approved Documentation All players that have been approved in past years can use the same documentation. All you will need to supplement previous year s documentation is the updated medical form and the signed Photo Model Release form.

A Certificate of Proof that your team manager and coaches have all taken the online Concussion and Sudden Cardiac Arrest training. League Score Books Make sure that at the end of the season you collect all scorebooks. This should include all teams that have all stars player representing your league. You must keep these scorebooks in a secure and accessible place in the event that one of your players game time eligibility is challenged. You can return these to your league coaches after the all star season.

All Star T-Shirt Form Forms will be distributed to all Leagues for names, sizes, etc.
