SOCCER LEAGUES & TOURNAMENTS Information Packet

General Information Checklist Please read through this packet and make sure that all required documents are completed and returned to Armonk Indoor by mail, email, or hand delivery prior to the start of your league/tournament. Team Roster The team roster must be completed by the coach and list all the players participating in the league/tournament. Only players listed on the team roster may participate. Playing other players will result in forfeit. The team roster can also be completed online through the link provided upon registration. Individual Waivers For insurance purposes, Armonk Indoor must have written consent of participation from each player on the team roster. The individual waivers can also be completed online through the link provided upon registration. Player Identification Each participating player must provide identification. Acceptable forms of ID include a WYSL, CJSA, or participating league pass; a birth certificate; or a passport. Remaining Balance Any remaining balance must be paid prior to your team s first game. Games will not be rescheduled unless approved by the league/tournament director and both teams. Armonk Indoor must be informed of cancellations at least 48 hours prior to the scheduled game time. Canceled games affect both the standings and the enjoyment of opposing teams, so please consider this when canceling a game.

Individual Waiver and Release of Liability Player Name: Birthdate: Address: City: State: Zip: Home Phone: Work Phone: Email: Team or Organization: In consideration of being allowed to participate in any way and/or enter upon, use and/or engage in sports activities at Armonk Indoor Sports Center, including participation in practices, events and/or other uses of the indoor facility at 205 Business Park Drive, Armonk, NY 10504, and their athletic/sports programs and related events and activities, the undersigned: Agree that prior to participating, he/she will inspect the facilities and equipment to be used, and if he/she believes anything is unsafe, he/she will immediately advise a representative of Armonk Indoor Sports Center of such condition(s) and refuse to participate; Acknowledge and fully understand that each participant will be engaging in activities that involve risk of serious injury, including permanent disability and death, and severe social and economic losses which might result not only from their own actions, inactions or negligence of others, the rules of play, or the condition of the premises or of any equipment used; further, that there may be other risks not known to us or not reasonably foreseeable at this time; Assume all the foregoing risks and accept personal responsibility for the damages following such injury, permanent disability or death; Understand that North Castle Sports Associates LLC does not provide medical insurance in such activities; Release, waive, discharge and covenant not to sue Armonk Indoor Sports Center, North Castle Sports Associates LLC, and The Town of North Castle or their affiliated clubs, their respective members, administrators, directors, coaches and other employees of said organizations, participants, sponsoring agencies, sponsors, advertisers, and, if applicable, owners and lessees of premises used to conduct the event, all of which are hereinafter referred to as releases, from demand, losses or damages on account of injury, including death or damage to property, caused or alleged to be caused in whole or in part by the negligence of the releases or otherwise; Shall defend, indemnify, and hold Armonk Indoor Sports Center, North Castle Sports Associates LLC, and The Town of North Castle, their officers, employees, and agents harmless from and against any and all liability, loss, expense, including reasonable attorneys fees, or claims for injury or damages arising out of the performance of this Agreement but only in proportion to and to the extent such liability, loss, expense, attorneys fees, or claims for injury or damages are caused by or result from the negligent or intentional acts or omissions of the individual or group renting space from the facility, its officers, agents or employees. The undersigned have read and acknowledge that he/she is entering into the above waiver and release, understanding that they have given up substantial rights by signing it and sign it voluntarily. WITHOUT LIMITING THE GENERALITY OF THE FOREGOING, IT IS MY INTENTION ON BEHALF OF MYSELF OR MY MINOR CHILD TO SPECIFICALLY RELEASE AND INDEMNIFY ARMONK INDOOR SPORTS CENTER, NORTH CASTLE SPORTS ASSOCIATES LLC AND THE TOWN OF NORTH CASTLE FROM ANY AND ALL CLAIMS ARISING FROM THEIR OWN NEGLIGENCE. Rules and Regulations Note: In the following rules, small-sided describes half- or third-field games, which are played with 6 or fewer players per side. 01) No cleats are allowed; players must wear sneakers, indoor soccer footwear, or turf shoes; shin guards are mandatory. 02) Size 4 futsal balls are used for small-sided games; size 5 soccer balls are used for full-field games. 03) Games are played with a running clock; extended injury time cannot be granted, so please help remove any injured player as quickly as possible. 04) Substitutions may be made on any stoppage of play after notifying the referee; substitutions must be made at the halfway line; the player exiting the field on a substitution must leave the field before the substituting player can enter the field. 05) There is no offside rule in small-sided games; offside is in play in full-field games. 06) The goalkeeper may not punt or drop kick the ball; an indirect free kick is awarded at the point of such an infringement. 07) Goal kicks and goalkeeper throws may not cross midfield without bouncing first or touching another player; an indirect free kick is awarded at midfield in case of such an infringement. 08) There are no goal kicks in small-sided games; goalkeeper restarts in place of goal kicks are: a) Distributing with hands, but not over halfway without bouncing. b) Putting the ball down, then dribbling or passing; be aware that opponents can steal the ball. 09) All sideline restarts are indirect free kicks in small-sided games; full-field games have normal throw-ins on sideline restarts. 10) All other restarts, except penalty kicks, are indirect free kicks; opposing players must give five yards on restarts. 11) Penalty kicks are taken at a spot one yard out from the goalkeeper s area; all players must be behind the ball until it has been kicked by the penalty kick taker. 12) Touch lines on the inside field markers are the nets. 13) Any ball hitting the bubble or a light above the field of play is considered in play unless ruled out of play by the referee, in which case an indirect free kick is awarded to the other team at the point of contact; any ball hitting the bubble by the end line is automatically ruled out of play. 14) Slide tackles are allowed, but the referee has discretion in regards to dangerous play. 15) Yellow and red cards are recorded throughout the league season/tournament; players/coaches receiving one red card or two yellow cards are ejected from the game and suspended for one game; players/coaches receiving three consecutive yellow cards are suspended for one game. 16) Armonk Indoor promotes positive coaching and sportsmanship and reserves the right to ensure that the program gets the support of all participants; as such, all referee calls must be respected as final. 17) No jewelry may be worn. 18) No spitting. 19) All other FIFA rules apply. 20) Players may only play for one team during a league season/tournament, except if both the league/tournament director and opposing coach agree to permit the player as a guest player on the day of the game. 21) Forfeited games result in a 3-0 loss and a 3-point deduction. 22) Teams are credited with a maximum goal differential of +6 per game; teams with a goal differential greater than +8 may be subject to a 2-point penalty. 23) In the event of a tiebreak scenario, differentiation is based on: 1) Goal differential 2) Goals scored 3) Head to head 4) Number of shutouts
