GENERAL INSTRUCTIONS: Every player participating in competitive/select soccer must be given full disclosure of certain facts about prospective soccer teams and soccer clubs (including information about the coach and other people involved, the team's plans for the year, the potential financial and time commitments, etc.). Every Select/Competitive soccer team must thoroughly complete this form and give it to prospective players before they are extended an offer to play with the team. Mandatory Uniform How many estimated local tournaments will the team play in? How many estimated out-of-town tournaments will the team play in? It is my responsibility to present a copy of the current fact sheet to each parent on my team. Please click continue and select leave page to submit your team club fact sheet.
