Carefully read and follow the instructions contained in this packet to ensure a timely response. Please contact 978-538-6338 concerning any questions. Carefully read the additional attached application instructions. All paperwork must be clearly and correctly filed to ensure processing of application. Completed applications must be deposited in the Firearms Licensing drop box located in the lobby of the police station, do not deposit payment in box as it will be collected at final interview. You will be directly contacted to schedule an appointment to complete the licensing process. Note: Any person who knowingly files an application containing false information shall be punished by a fine of not less than $500.00 nor more than $1,000 or by imprisonment for not less than 6 months nor more than 2 years in the house of correction, or by both such fine and imprisonment.

Additional Application Instructions and Information All first time applicants must complete a Basic Firearms Safety Course that is approved by the Massachusetts State Police, or shall have completed a Hunters Safety Course that is approved by the Division of Fisheries and Wildlife. No class is required for the issuance of an F.l.D. Card Class D. The Peabody Police Department will do ALL record checks. All applicants must provide two proofs of residency; Mass Drivers License, State Identification, Motor Vehicle Registration, Utility Bills, Voter Registration Card. All applicants are required to fill out the standardized Commonwealth of Massachusetts Firearms License Application. Payment accepted for Firearm Licenses shall be made by Money Order or Personal Check payable to the City of Peabody. Fees are as follows: L.T.C. Class A - $100.00 F.I.D. Class C- $100.00 {$25.00 under eighteen) F.l.D. Card Class D - $25.00 (Card Required for 15 18 Years of Age) Law Enforcement Officials (Including Retirees) $25.00 (Class A and C) Renewals for applicants over the age of 70 shall be at no charge (Class A and C)

Detailed Information Pertaining to License to Carry Firearms License to Carry Firearms: Fee is $100.00 for original issues and renewals (Note: There is no fee required for renewals for persons over the age of 70). This license is valid for up to, but not more than 6 years (see M.G.L. C140, s131(a)), it requires that all FIRST TIME applicants submit one of the following Firearm Safety Certificates with their completed application (see M.G.L. C140 131P). A. A certificate of completion of a basic Firearms Safety Course that is approved by the Mass State Police. B. A valid certificate of completion of a Hunter Safety Course given by the Massachusetts Division of Fisheries and Wildlife. This license permits the purchase, possession and carrying of Large Capacity firearms, rifles and shotguns. It also permits the possession of ammunition. This type of license allows a person to carry a loaded and concealed firearm, unless restrictions are implemented. Large Capacity refers to firearms feeding devices capable of carrying more than ten rounds of ammunition, or shotguns capable of carrying more than 5 rounds. Detailed Information Pertaining to Firearm Identification Cards Firearms Identification Card Class C: Fee is $100.00 for original issues and renewals (Note: There is no fee for renewals for persons over the age of 70). This license is valid for up to but not more than 6 years (see M.G.L. C140, s129b), it requires that all First Time applicants to submit one of the following Firearms Safety Certificates with their completed application (see M.G.L. C140 131P). A. A certificate of completion of a basic Firearms Safety Course that is approved by the Massachusetts State Police. B. A valid certificate of completion of a Hunter Safety Course given by the Massachusetts Division of Fisheries and Wildlife. Permit is valid for the purchase and possession of non-large capacity rifles, shotguns, ammunition. Non Large Capacity refers to firearm feeding devices capable of carrying 10 rounds or less of ammunition for firearms and rifles, 5 rounds or less for shotguns.

Firearms Identification Card Class D: Fee for the issuance is $25.00. A Firearms Safety Certificate is not required. AN F.I.D. CARD CLASS D IS NO LONGER REQUIRED TO POSSES SELF DEFENSE SPRAY FOR PERSONS EIGHTEEN YEARS OF AGE AND OVER. PERSONS FIFTEEN TO EIGHTEEN YEARS OF AGE MUST OBTAIN AN F.I.D. CLASS D TO LAWFULLY POSSES SELF DEFFENSE SPRAY. IF YOU ARE A STATUTORILY DISQUALIFIED PERSON AS DEFINED UNDER M.G.L. YOU ARE PROHIBITED FROM POSSESSING SELF DEFENSE SPRAY. Restrictions: Permit is valid for the purchase or possession, of chemical mace, pepper spray or similar other propelled liquid, gas or powder designed to temporarily incapacitate. It is not valid to purchase or posses, any firearm, rifle shotgun, ammunition or components thereof.
