PURPOSE To define the minimum requirements for controlling risks associated with persons entering or working in confined spaces. 1 SCOPE This procedure applies to all work or entry by any person into a confined space within areas controlled by the University of Canberra. The Confined Spaces Code of Practice 2016 defines a confined space as being an enclosed or partially enclosed space that: Is not designed or intended to be occupied by a person; and Is, or is designed or intended to be, at normal atmospheric pressure while any person is in the space; and Is, or is likely to be, a risk to health and safety from: o o o An atmosphere that does not have a safe oxygen level, or Contaminants, including airborne gas, vapours or dusts, that may cause injury from fire or explosion, or Harmful concentrations of any airborne contaminates, or engulfment Entry into a confined space is deemed to have occurred when a person s head (breathing zone) and/or upper body enters the space. Pre-entry atmospheric testing by inserting a hand or arm into the space while holding a test instrument or probe shall not constitute confined space entry. This procedure shall be read in conjunction with the Permit to Work System Guideline POL-1000, Isolation Lockout and Tagging PRO-1060, Barriers and Barricading PRO-1080 and Hot Work PRO- 1040 and any other relevant Permit to Work procedures. 2 PROCEDURE 2.1 Identification of Confined Spaces 2.1.1 Department Managers shall ensure all confined spaces within their area of responsibility are identified. Fig 2.1: Determining confined spaces, as outlined in Confined Spaces Code of Practice 2016, will assist in identification of confined spaces. Where access to a confined space is possible by multiple entrances, signage shall be posted at each entrance Secured to prevent unauthorized entry 2.1.3 Potential confined spaces do not constantly meet confined space criteria. However, specific tasks being performed within that space may alter conditions. A Risk Assessment shall determine whether the work area becomes a confined space for the duration of that task. 2.1.4 Some enclosed or partially enclosed spaces such as abrasive blasting or spray painting booths (as examples) have harmful atmospheric contaminates but are designed for human occupation and have systems such as gaseous fire extinguishing systems installed and so do not constitute a confined space and are not covered by this procedure. YES YES NO NO 2.2 Managing Confined Spaces 2.2.1 Each Department shall maintain a current Confined Space Register containing the following information: a. confined space number of all known confined spaces b. the location or plant area of the confined space c. a detailed description of the equipment and space d. inspection frequency and audits e. the underpinning Risk Assessment and reference to any related task analyses and emergency response procedures f. section for any comments relating to the confined space (reference to reports related to incidents/ accidents associated with the confined space) PRO-1020 V1.0 Confined Space Work.doc Page 4 of 28

2.2.2 The Confined Space Register shall be accessible to all relevant personnel, including contractors and emergency response services. 2.2.3 Contractor inductions shall include reference to confined spaces and the confined space register 2.2.4 Confined Space Permit to Work Process: a. As a high-risk, entry into a confined space shall be undertaken in compliance with POL-1000 Permit to Work System Guideline. b. FRM-1021 Permit to Work - Confined Space shall be authorized in conjunction with FRM 1001 Job Safety Analysis Form before work may commence within a confined space. Access shall not be permitted into a confined space unless justified under a risk management process. c. EFM Management shall appoint appropriately trained and competent personnel to authorize FRM-1021 Permit to Work-Confined Space. 2.2.5 Risk Assessment and hazard identification: a. As per POL-3203 UC Risk Management Plan 2016 and POL-1000 Permit to Work System Guideline, a formal Risk Assessment Shall be conducted before work in/entry into a confined Space may be authorized. Participants in the Risk Assessment shall have knowledge of the confined space and/or be competent in the tasks being performed. b. The risk assessment shall identify all known hazards associated with working in a confined space including, but not limited to: The atmosphere in the confined space, including whether testing or monitoring is to be undertaken The risk of engulfment All proposed work activities, particularly those that may cause a change to the conditions in the confined space The number of persons occupying the space The soundness and security of the overall structure and the need for lighting and visibility The identify and nature of substances last contained in the confined space Any risk control measures needed to bring the confined space to atmospheric pressure The number of persons required outside the space: i. To maintain equipment essential for the task being undertaken within the confined space ii. To provide continuous communication with the persons within the confined space iii. To properly initiate emergency response procedures Risks associated with other hazards, such as noise or electricity Arrangements for emergency response e.g.: first aid and resuscitation PRO-1020 V1.0 Confined Space Work.doc Page 5 of 28

The physiological and psychological demands of the task and the competency of persons involved in the tasks or emergency duties Adequate training of workers in any required procedure, particularly those that are unusual or non-typical, including the use and limitations of any personal protective equipment to be used Availability and adequacy of appropriate personal protective equipment and emergency equipment for all persons likely to enter a confined space The need for additional risk control measures, including: i. Prohibiting hot work in adjacent areas ii. Prohibiting smoking and naked flames within a confined space and adjacent areas iii. Avoiding contamination of breathing air from operations or sources outside the confined space, e.g.: from the exhaust of an internal combustion engine iv. Prohibiting movement of equipment in adjacent areas e.g.: forklifts v. Prohibiting spark-generating equipment, clothing and footwear Whether purging or cleaning in the confined space is necessary Requirement for a Permit to Work - Hot Work Conditions that could impede access and egress or the conduct of the tasks in the confined space, e.g.: plant layout, dimensions, manual handling and ergonomic aspects of the task c. The risk assessment shall be revised whenever the scope of the work changes. 2.3 Control Measures 2.3.1 Wherever practicable, identified risks arising from work in a confined space shall be eliminated. If not possible to eliminate, controls shall be implemented to reduce risks to an acceptable level. 2.3.2 An Emergency Rescue Plan shall be communicated to all personnel who may be required to enter a confined space and to emergency response personnel. This plan shall include, but not be limited to: Required first response protocol and equipment Communication/ notification process Evacuation protocol 2.3.3 All potentially hazardous services, including all process services normally connected to that space, shall be isolated in compliance with PRO-1060 Isolation and Tagging. 2.3.4 A trained and competent Safety Observer shall be appointed for the duration of the confined space task. They shall be familiarized with the Emergency Rescue Plan for the confined Space. 2.4 Pre-entry atmospheric testing 2.4.1 Atmospheric testing and monitoring of the confined space shall be conducted by a competent person using a suitable, correctly calibrated gas detector. 2.4.2 The type and frequency of atmospheric monitoring shall comply with specifications of the risk assessment and Permit to Work. 2.4.3 Atmospheric testing shall verify: Oxygen content (within safe O2 range of 19.5% - 23.5%) Airborne concentration of flammable contaminates Airborne concentration of potentially harmful contaminates 2.4.4 Initial testing shall be done from outside the confined space by inserting a sample probe and/or portable gas detection device at appropriately selected access holes, nozzles and openings. 2.4.5 Testing shall be carried out on a sufficient number of points to accurately reflect areas of the space likely to be accessed. 2.4.6 The Permit to Work Confined Space Entry Permit process shall be implemented if necessary to enter the space to test remote regions away from entrance or access holes. 2.4.7 Re-testing and continuous monitoring may be necessary if the risk assessment indicated that conditions may change due to work being done or the disturbance of hazardous material in the confined space. 2.4.8 Calibration and maintenance activities on confined space safety equipment shall comply with Appendix 5: Compliance of Gas Detectors 2.5 Completing FRM-1021 Permit to Work Confined Space and FRM-1001 Job Safety Analysis 2.5.1 FRM-1021 Confined Space and FRM-1001 Job Safety Analysis may be granted by a competent person (Permit to Work initiator), but shall only be approved by an appointed Confined Space Permit Authorizer. 2.5.2 Permit to Work Initiator shall complete relevant aspects of FRM-1021 Confined Space and FRM-1001 Job Safety Analysis, compile the required risk documents and supporting material and provide to the Confined Space Permit Authorizer for approval. 2.5.3 The Confined Space Entry Permit Authorizer shall review the risk documentation and control measures with appropriate personnel prior to authorizing FRM-1021 Confined Space. 2.5.4 In authorizing the task, the Permit to Work Authorizer shall pass all approved documentation to the Permit to Work Acceptor (Person in Control of Work) and ensure the process is fully understood Permit to Work Acceptor. 2.5.5 Personnel working under the permit shall be under the direction of the Permit to Work Accept[tor who shall ensure workers are fully apprised of all aspects of the task. 2.6 Personnel Protective Equipment (PPE) and Respiratory 2.7 Training 2.6.1 Underpinning Risk Assessments shall identify requisite PPE and RPE 2.6.2 Appropriate REP shall be worn in situations when it is not reasonably practicable to ensure the confined space contains a safe oxygen level, or safe levels of airborne contaminates. 2.6.3 Appropriate RPE shall be determined by the level and type of contaminates and the nature of the task as recommended by AS/NZS1715:2009 Selection, use and maintenance of respiratory protective devices. 2.7.1 All personnel required to perform activities defined in this procedure shall have the skills and knowledge to understand the hazards associated with working in a confined space, the contents of all confined space entry permits and the control measures implemented for their protection. 2.7.2 As a minimum, workers shall hold the following qualifications, or their equivalent: a. All persons performing role of Safety Observer DEFWHS012 Test a confined space DEFWHS013 - Control entry/ exit to a confined space b. All persons required to enter a confined space RIIWHS202D Enter and work in confined spaces c. Confined Space Permit to Work Authorizer RIIWHS202D Enter and work in confined spaces DEFWHS013 - Control entry/ exit to a confined space d. 2.7.3 Estate and Facilities Management shall maintain a current record of all personnel who are authorized to perform as a confined space permit authorizer. 2.8 Signage and Barricading 2.8.1 To prevent unauthorized access, barricading and signage similar to that below which complies with provisions outlined in AS12139-1994 Safety Signs for Occupational Environment and PRO-1080 Barriers and Barricades shall be erected at all points of entry to the confined space. 2.8.2 Signs and barricading shall be in place while a confined space is accessible, including when preparing to work in the space, during work in the space and when packing up on completion of the work. 2.9 Safety Observer 2.9.1 A Safety Observer shall be assigned to continuously monitor the wellbeing of those inside the confined space and, if practicable, observe the work being carried out. 2.9.2 The Safety Observer shall not perform other work or activity which may interfere with their primary role of monitoring the workers inside the space. 2.9.3 The Safety Observer shall have the authority to order workers to exit the space if any hazardous situation arises. 2.9.4 The Safety Observer shall be trained and deemed competent to perform the role and: a. Have available the evacuation procedures, emergency response plans, access to emergency response personnel, and be prepared to initiate the emergency notification procedures, if required. b. Retain the permit to work and ensure permit conditions are being complied with: c. Understand the nature of the hazards inside the confined space and be able to recognize signs and symptoms that workers in the confined space may exhibit or report. d. Be familiar with all external plant and equipment supporting work inside the space. e. Ensure access and egress points are clear of obstructions f. g. Monitor the general well-being of workers inside the confined space through the approved communication systems as identified in the risk assessment. h. Monitor the external systems that may be required for the confined space (e.g. atmospheric monitoring equipment, forced ventilation equipment, etc.) 2.9.5 The Safety Observer shall never: a. Enter the confined space whilst they are the designated Safety Observer, even in the case of an emergency. b. Leave their station unless another Safety Observer takes over their responsibilities. 2.10 Communication 2.10.1 The risk assessment and certificate documents shall identify an effective system to enable communication between people inside and outside the confined space and the process to follow to summon help in an emergency. 2.10.2 Conditions in the confined space shall determine the most effective method of communication. This may include, but not limited to, voice, radio and/or hand signals. 2.11 Entering/ Working in the Confined Space 2.11.1 The Permit to Work Acceptor shall ensure: a. All workers listed on the Permit to Work documentation and Safety Observer are advised of and understand all requirements of FRM-1021 Permit to Work Confined Space and FRM-1001 Job Safety Analysis. b. Signs and protective barriers are in place to prevent entry of persons not involved in the task. c. A competent person assesses atmospheric conditions and tests that the following are satisfied. Confined space contains oxygen level (between 19.5% and 23.5%) Atmospheric contaminants in the confined space are reduced to levels below the Safe Work Australia Workplace - Exposure Standards for Airborne Contaminants. The confined space is assessed and managed for temperature extremes Concentration of flammable contaminant is the atmosphere of the confined space is below 5% of its Lower Explosive Limit (LEL) d. Communication methods (both internal and external) specified in the risk assessment are effective and continuous. The communication device shall be located with the Safety Observer and regularly tested. e. That the following safety precautions are complied with: PRO-1020 V1.0 Confined Space Work.doc Page 10 of 28

Gas cylinders other than those used for RPE shall not be taken into the confined space Work such as, but not limited to, welding, cutting, grinding or the introduction/ removal of hazardous chemicals not be carried out unless a risk assessment has been conducted and the relevant Permit to Work documentation completed. Prohibited items including ignition sources and hazardous substances shall not be taken into the confined space unless specified controls listed in the PTW and JSA for this risk are implemented Electrical equipment without an RCD shall not be taken into a confined space All equipment taken into the confined space shall be fit for purpose Combustion-driven tools and equipment shall not be used inside or in proximity of the confined space where the exhaust produced could affect the atmosphere in the confined space Where there is a potential for flammable or explosive gas/vapour or dust mixtures, all sources of ignition shall be eliminated including any electrical device which is not explosion proof or intrinsically safe in accordance with AS/NZS 2381.1:2005 Electrical equipment for explosive gas atmospheres Selection, installation and maintenance General requirements. 2.11.2 Entry and exits shall be maintained by either of the below methods which shall be clearly communicated to all workers: Ensuring all personnel sign on and off FRM-1021 Permit to Work - Confined Space Work each time they enter/ exit the confined space; or Using a confined space Tag Board, which requires FRM-1021 Permit to Work Confined Space Work to be signed at the commencement of the task and then signed off when leaving the area. Interim entries / exits, can be monitored by adding or removing Personal Safety Tags from the tag Board. 2.11.3 Continuous atmospheric monitoring shall be conducted where there is potential for toxic, flammable or explosive gas/vapour or dust mixtures. Results shall be recorded on FRM-1021 Permit to Work Confined Space Work. 2.11.4 All persons shall be removed from the confined space if the concentration of flammable contaminant is the atmosphere increases to greater than 5% but not more than 10% of its LEL. The only exception shall be where continuous monitoring using a suitably calibrated flammable atmosphere detector is implemented within the confined space for the duration of the work continuously monitoring flammable contaminates. 2.11.5 All persons shall be removed from a confined space if the concentration of flammable contaminant is the atmosphere increases to 10% of its LEL or greater. 2.11.6 On initiation of any gas detector alarms, all personnel shall be removed from the confined space and current controls reassessed. The Permit to Work shall be PRO-1020 V1.0 Confined Space Work.doc Page 11 of 28

suspended until the conditions in the confined space can be reviewed, and conditions for entry have been met. 2.11.7 Whenever workers exit the confined space, atmospheric testing shall take place prior to re-entry. 2.11.8 Workers shall immediately leave the confined space if atmosphere or safe working conditions deteriorate, and notify the Safety Observer of the changes. The Permit to Work shall be reviewed to ensure conditions for entry have been met. 2.11.9 FRM-1021 Permit to Work Confined Space Work shall: a. Record current atmospheric testing readings of required monitoring b. Be reviewed when the person to whom the permit was issued changes c. Be reviewed when a break in the continuity of the task that may have changed the condition under which the written authority was issued. 2.12 Returning a Confined Space to Service 2.12.1 All personnel shall sign off the Permit to Work 2.12.2 The Permit to Work Acceptor shall notify the Permit to Work Authorizer when the work is complete or has stopped and the plant is ready to return to safe operational condition. The permit to Work Authorizer shall close FRM-1021 Permit to Work Confined Space after verifying that work is complete, the area has been cleared of personnel and equipment and is safe to return to service. 2.12.3 All Permit documentation shall be reviewed and recommendations escalated to the appropriate personnel before document are forwarded to Estate and Facilities Management for filing. 2.13 Reclassification of a Confined Space 2.13.1 A confined space may be reclassified as a non-confined space if it has undergone sufficient changes in structure or usage to eliminate (without need for risk control measures) all possible sources of inherent hazards that define a confined space. 2.13.2 Before reclassification, a risk assessment shall conclude that the area no longer meets the criteria for a confined space. 2.14 Consultation 2.14.1 In implementing the requirements of this procedure, consultation should take place with all stakeholders who may be required to work in the confined space or may be affected by same. 2.15 Records Management 2.15.1 All completed FRM-1021 Permits to Work Confined Space and accompanying documents shall be kept for a minimum of seven years. 2.15.2 All close FRM-1021 Permit to Work Confined Space documents that are relevant to an incident investigation or claim against University of Canberra shall be retained with the incident investigation documentation and a copy of this documentation remain within the Permit to Work filing system. 3 DEFINITIONS Airborne Contaminant Any contaminant present in the air that may be harmful to persons. Atmospheric Monitoring The continuous measurement of oxygen concentration or airborne contaminants over an uninterrupted period of time. Atmospheric Test Point-in-time measurement of oxygen concentration or airborne contaminants. Certificate Authorizer Person appointed in writing by Estate and Facilities Management using FRM-1003 Authority to Authorize a Permit to Work. The person shall have met the competency requirements listed in POL- 1000 Permit to Work System Guideline, Section 14 - Training and Competence. Competent Person A person who has acquired through training, qualification or experience (or combination of these) the knowledge and skills enabling that person to safely perform a specific task. Contaminant Any dust, fume, mist, vapour, biological matter, gas or other substance in liquid or solid form, the presence of which may be harmful to persons. Engulfment The immersion or envelopment of a person by a solid, liquid or other substance in powder or granular form (examples include: grain, sugar, flour, sand, coal and fertiser) that could be stored within a confined space. The risk of engulfment can occur in a stored free-flowing solid or a rising level of liquid and may cause suffocation or drowning. Entry (to a Confined Space) When a person s head or upper body is within the boundary of the confined space. Note: Inserting an arm for the purpose of atmospheric testing is not considered as entry into a confined space. Explosive Limit a. Lower Explosive Limit (LEL) The concentration of a flammable contaminant in air below which the propagation of a flame does not occur on contact with an ignition source b. Upper Explosive Limit (UEL) The concentration of a flammable contaminant in air above which the propagation of a flame does not occur on contact with an ignition source. Exposure Standard An airborne concentration of a particular substance in the person s breathing zone, exposure ot which, according to current knowledge, should not cause adverse health effects or undue discomfort to nearly all persons. The exposure standard can be of three forms: Time weighted average (TWA), short-term exposure limit (STEL) or peak exposure limit. The following terms are used in calculating concentrations of airborne contaminants: a. Time-weighted average (TWA). The average airborne concentration of a particular substance when calculated over the normal eight-hour working day, for a five day working week. b. Short-term exposure limit (STEL). A 15 minute TWA exposure that should not be exceeded at any time during a working day, even if the eight-hour TWA average is within the TWA exposure standard. c. Peak A maximum or peak airborne concentration of a particular substance determined over the shortest practicable period of time, not exceeding 15 minutes. NOTE: 1. Exposure at the STEL should not be longer than 15 minutes and should not be repeated more than four times a day. 2. For further information refer to the Safe Work Australia Hazardous Substances Information System (HSIS). FRM-1021 Permit to Work Confined Space and FRM-1001 Job Safety Analysis Documents that authorize entry into a confined space and the conduct of tasks associated with the confined space. These documents may only be authorized by an appointed Confined Space Permit Authorizer. Flammable Airborne Contaminant Any dust, fume, mist, vapour or gas present in air at concentrations that can propagate a flame on contact with an ignition source. Flammable Range The range of flammable airborne contaminate (percentage by volume)in air at which an explosion can occur upon ignition. Expressed as lower explosive limit (LEL) and upper explosive limit (UEL). Hot Work Welding, thermal or oxygen cutting, heating, including fire-producing or spark-producing operations that may increase the risk of fire or explosion. Operational Competencies Competencies required to perform tasks and defined within training requirements. Permit to Work Acceptor (Person in Control of Work) A competent person, sufficiently skilled, qualified and experienced responsible for completion of the work specified in FRM-1021 Permit to Work Confined Space and FRM-1001 Job Safety Analysis forms. Permit to Work Authorizer A person who has been approved in writing by the Estate and Facilities Management to authorize FRM-1021 Permit to Work Confined Space and FRM-1001 Job Safety Analysis. Permit to Work Initiator A sufficiently experienced person who is current with all relevant competencies and familiar with the specific work task to identify and control associated hazards through the risk assessment process. Role responsibilities include, but not limited to: a. Outlining how the work shall be undertaken (e.g. procedures, precautions, equipment, location, start time, duration) b. Participating in and/or facilitating the Task Analysis/ Risk Assessment c. Identifying specific PTW forms relevant to the task, entering requisite information and seeking authorization from an appointed PTW authorizer. Person in Control of Work Refer to Permit to Work Acceptor. Safe Oxygen Range A concentration of oxygen in the atmosphere having a minimum of 19.5% by volume and a maximum of 23.5% by volume, under normal atmospheric conditions. Shall Indicates that a statement is mandatory Safety Observer Any person sufficiently competent to and familiar with the expectations of the role to continuously monitor the wellbeing of those inside the confined space and, if practicable, observe the work being carried out. APPENDIX 5 Compliance of Gas Detectors 1 Purchasing Gas Detectors 1.1 Electrically operated gas detectors for use in flammable atmospheres shall be: If portable SAA certified EX IA IIC T6 If fixed SAA certified with minimum certification for use in an area in which an explosive gas atmosphere is likely to occur in normal operation. 1.2 Compliance certificates that accompany the gas detector are to be kept for reference. 1.3 Confined space gas detectors shall be capable of measuring the following gases: Methane Carbon monoxide Oxygen Sulphur dioxide Hydrogen sulphide 1.4 Any grab sampling equipment or tubing purchased with the detector is too maintained and stored correctly with the detector. This tubing shall also be capable of use for measuring the following gases: Ammonia Carbon dioxide Carbon disulphide Carbon monoxide Chlorine Hydrogen sulphide Nitrogen dioxide Nitrous fumes Sulphur dioxide 2 Maintaining Gas Detectors 2.1 Only qualified repairers (preferably NATA registered) shall be used to repair and calibrate electrically operated gas detectors. 2.2 Each electrically operated gas detector shall be entered into a routine maintenance and calibration schedule. 2.3 Inspection, maintenance and calibration shall be carried out at intervals advised by the manufacturer but not less than every six months. 2.4 Access for inspection, maintenance and calibration shall be provided for fixed detectors located in ducts or on structures. 2.5 The qualified repairer shall issue a calibration certificate showing conformance to the original specification. Testing Gas Detectors 4.1 Only competent persons shall challenge test electrically operated gas detectors. 4.2 Each electronically operated gas detector shall be routinely test for a valid zero and response to a challenge test gas before and after use. 4.3 The result of each zero check and challenge test shall be recorded on a gas detector challenge test sheet see example in Attachment 1. 4.4 The following information shall be recorded.
