1.0 SCOPE AND PERSONNEL RESPONSIBLE FOR THE PLAN This Fall Protection Plan (FPP) is specifically for the following project and is comprised of two sections: Section 7A. Fall Protection for work on the roof of the building. Section 7B. Fall Protection requirements for interior overhead work using elevated work platforms (scissor lifts) for removal/replacement of VAV boxes, ductwork, etc. Project Name: NS-Mayport Bldg. 1553 Renovation Location: Naval Station Mayport Date Plan Revised: 10/26/10 Plan Supervised By (Primary Competent Person): Carl Thomas-Bldg. 1553 Site Supervisor Other Competent Persons: Carl Thomas (WW Gay Site Supervisor & Competent Person) Kirk Barras (WW Gay Superintendent Naval Construction & Competent Person) Jon Meacham (WW Gay Safety Inspector/Trainer & Competent Person) The persons listed above are able to identify general and specific fall hazards on this project and have the authority to take immediate action to correct the hazards. They are also qualified in the safe use fall protection systems as required by OSHA standards and the EM 385. It is the duty of all competent persons to identify fall hazards and implement engineering, work practice or PPE controls to eliminate/reduce those hazards. Jon Meacham will conduct any fall protection training required on the project as well as scissor lift training. 2.0 STATEMENT OF COMPANY POLICY WW Gay Mechanical Contractor, Inc. is dedicated to the protection of its employees from on-the-job injuries. All employees of the Company have the responsibility to work safely on the job. The purpose of the FPP is to supplement our existing safety and health program and to ensure that every employee who works for the Company and our subcontractors takes the appropriate measures to address and protect against fall hazards. Fall protection on the project shall comply with 29 CFR 1926, Subpart M and USACE 385-1-1, Section 21. 3.0 IMPLEMENTATION OF FALL PROTECTION PLAN Fall protection will be implemented by the site supervisor or any competent person listed above. Employees are required to follow this FPP and are to inform the competent person of fall hazards that have not been addressed so that supervision can correct the situation with necessary safety controls. This plan is required to remain on the project during all phases of work. The WW Gay safety department will review the plan with site supervision as needed to ensure it is adequate to meet fall protection needs as the project moves forward. 4.0 ENFORCEMENT Constant awareness of and respect for fall hazards and compliance with these safety rules is a condition of employment. Employees who do not comply with the rules of this FPP will be reprimanded, which may include removal from the jobsite. Site supervisors and competent persons will inspect the jobsite frequently to ensure personnel comply with this plan. WW Gay personnel and affiliated subs who do not comply with this Fall Protection Plan will be removed from the jobsite immediately by supervision. 5.0 RESPONSIBILITIES Competent Persons listed in this plan are responsible to ensure that all employees who are exposed to falls are trained in fall protection, including the provisions of this plan. Competent persons will: Ensure employees are trained to inspect any fall protection equipment used and correct maintenance of the equipment. They will inspect the jobsite daily to ensure fall protection systems/equipment are safe for use. Contact the safety department when concerns or questions arise which would necessitate a change in this plan to resolve fall protection issues The WW Gay safety department will document any training done on fall protection. 4
6.0 TRAINING Employees will be fall protection trained before they are allowed to perform tasks that involve fall hazards. Training will be conducted by a competent person qualified in the items listed below. All training will have written certification by the trainer to identify the employee trained, the dates of training, and the signature of the trainer. The recognition of fall hazards in the area and control measures to eliminate/reduce them. Procedures for erecting, maintaining, disassembling and inspecting the fall protection systems used. The inspection and operation of fall protection systems, including but not limited to: personal fall arrest systems components, guardrails, secure anchorages, safety nets, fall restraint, etc. The correct procedures for the handling and storage of fall protection equipment. Employees who are assigned the competent person roll shall be trained with specific fall protection training for competent persons. 7.0 FALL PROTECTION SYSTEMS USED ON THE PROJECT All fall protection systems used on the project will be designed by a person who is qualified in fall protection. Competent persons will inspect them frequently to ensure they are maintained in safe condition and used properly. Activity Hazard Analyses (AHA s) have been developed for this project and WW Gay employees on site are to review these (also known as pre-task analyses) based on the task being performed each day. The WW Gay employees will also sign the AHA s after review of safety procedures for the tasks. The items below describe the equipment and methods used for fall protection that will be used for the different phases of work on the project. They describe how they are to be used and maintained. 7A. ROOF-WORK There are 6 air-handling units, 3 package DX units, 1 condenser unit and 29 exhaust fans which must be removed and/or replaced on the roof of the building whose leading edges are all unprotected. While most of these are located further than six feet from unprotected leading edges, fall protection must be in place where: 1. Units/fans are removed and the open hole presents a fall hazard until securely covered. 2. Crane operations require personnel flagging the crane or controlling loads with tag-lines approach within 6 feet of an unprotected leading edge. 3. Any employee is at risk of falling from an elevation 6 feet or more in height. 5
Fall protection systems used for roof-work will be either: 1. Mobile Fall Protection Systems (cut-sheet included) as a secure anchorage with a lanyard or self-retracting lifeline (SRL) connector attached to a full body harness. 2. Reusable roof-anchors with lanyard/srl attached to body harness. This system will be used only where the roof may be penetrated by screws/tapcons. 3. Finally, the least desirable protection which may be used is a warning line with a dedicated safety monitor in a body harness tied off at all times to an SRL attached to a secure anchorage (5000 lbs. at least) either structural steel or the mobile system in item #1. Fall protection system requirements are as follows: Personal Fall Arrest Systems These systems will be employed if no safer control exists to eliminate or reduce the fall hazard. The personal fall arrest system will consist of a secure anchorage, an inspected connector and an inspected fall harness (full body harness). These may include lifelines, inertial reels, lanyards, beam clamps/straps, rope grabs, concrete anchors, approved structural members, wire rope, etc., but all must do the following: Limit maximum arresting force on a falling employee to 1800 lbs. Be rigged so that an employee can free fall no farther than 6 ft. Have an anchorage that will support no less than 5000 lbs. per person attached to it. Any questions about correct anchorages will be addressed to the WW Gay safety dept. & Slone Associates safety. All components of a personal fall arrest system must be inspected prior to each use and personnel trained in the inspection and use of the equipment. All defective equipment will be removed from service immediately. Body belts will not be used EVER. Warning Line and Safety Monitor While this is the least desirable system for fall protection, the scope of work on the building s unprotected rooftop involving removal and replacement of air-handling units may require this type of control for personnel flagging the crane, etc. at leading edges. There are few secure (if any) secure anchorages on the roof for attachment of an inertial reel (preferred), or lanyard. Most personnel will not be within 6 ft. of a leading edge; however those who are will be protected from falls by either a PFAS (if a secure anchorage for connectors can be found) OR by a warning line with a trained safety monitor with no other assigned duties. When their use is permitted, warning line systems shall comply with the following: a. Warning lines shall be erected around all sides of the work area. (1) When mechanical equipment is not being used, the warning line shall be erected not less than 6 ft (1.8 m) from the roof edge. 6
(2) When mechanical equipment is being used, the warning line shall be erected not less than 6 ft (1.8 m) from the roof edge that is parallel to the direction of mechanical equipment operation and not less than 10 ft (3.0 m) from the roof edge that is perpendicular to the direction of mechanical equipment operation. b. Warning lines shall consist of ropes, wires, or chains, and supporting stanchions erected as follows: (1) The rope, wire, or chain shall be flagged at not more than 6 ft (1.8 m) intervals with high visibility material. (2) The rope, chain, or wire shall be rigged and supported in such a way that its lowest point (including sag) is no less than 34 in (86.4 cm) from the roof surface and its highest point no more than 39 in (99 cm) from the roof surface. (3) After being erected, with the rope, wire, or chain attached, stanchions shall be capable of resisting, without tipping over, a force of at least 16 lb (7.3 kg) applied horizontally against the stanchion 30 in (76.2 cm) above the walking/working surface, perpendicular to the warning line, and in the direction of the roof, floor, or platform edge. (4) The rope, wire, or chain shall have a minimum tensile strength of 500 lb (226.8 kg), and after being attached to the stanchions shall be capable of supporting, without breaking, the loads applied to the stanchions (as described in (3) above). (5) The lines shall be attached at each stanchion in such a way that pulling on one section of the line between stanchions will not result in slack being taken up in adjacent sections before the stanchion tips over. (6) No employee shall be allowed in the area between a roof edge and a warning line unless the employee is performing work in that area and is protected by a motion stopping safety (MSS) system, i.e. Self-retracting lifeline attached to a secure anchorage and body harness. During roofing work, materials and equipment shall not be stored within 6 feet (1.8 meters) of a roof edge. 7B. INTERIOR WORK USING ELEVATED WORK PLATFORMS: Given the extensive HVAC renovation work, scissor lifts will be widely used on this project. Lack of training, overhead obstructions, energized lines/equipment and unprotected floor penetrations present the greatest potential for accident/injury. The WW Gay safety program has specific safety and fall protection requirements concerning scissor lifts. They apply to all WW Gay employees & subs and are as follows: 7
1. No WW Gay employee or subcontractor employee will operate or be in a scissor lift unless they have been trained. The training will comply with 29 CFR 1926.452 and be documented. 2. During normal use, personnel WILL BE tied-off to a secure anchorage in the scissor lift using an inspected, properly worn body-harness and connector (lanyard or selfretracting lifeline). 3. No one will take one or both feet off of the scissor lift work platform for any reason. Standing on the equipment toe-board or mid-rail is FORBIDDEN. 4. No ladder or bucket may be used to gain extra height in a scissor lift.ever. 5. If the lift work platform is extended beyond the wheel base, all personnel on the lift will be tied-off while the platform is extended. 6. No person will operate a lift (go forward or backward) while it is raised. The lift will be lowered BEFORE travel. 7. Check the travel and work area for debris, unprotected floor penetrations and electrical hazards. Stay no less than 10 feet from an energized equipment or line. If that minimum safe approach distance from energized equipment or lines cannot be maintained, the equipment/line must be locked-out or insulated. 8. Inspect all equipment and body harnesses daily prior to use. Floor Penetration/Hole Covers Covers located in roadways/vehicular aisles must be able to support at least twice the maximum axle load of the largest vehicle to which the cover might be subjected. All other covers must be able to support at least twice the weight of employees, equipment, and materials that may be imposed on the cover at any one time. To prevent accidental displacement resulting from wind, equipment, or workers= activities, all covers must be secured. All covers shall be color coded or bear the markings "HOLE" or "COVER." 8.0 ELEVATED WORK PLATFORM TRAINING All WW Gay and affiliated subcontractor personnel will be trained in the safe operation of scissor lifts when used on this jobsite. Training will include instruction in hazard recognition (tipovers & falls), center of gravity, collision & crushing, terrain and slope hazards, overhead obstructions and power lines, vehicle inspections, emergency procedures and tie-off requirements. Trained WW Gay personnel will receive a certification card. 8
9.0 LADDERS All ladders will be inspected daily before use per WW Gay safety policy, OSHA and EM 385 requirements. Contact the competent person onsite for a checklist of items to be inspected. All leaning portable ladders will be tied-off to prevent displacement. All ladders will meet ANSI requirements. All WW Gay and affiliated subcontractor ladders will be non-conductive. 10.0 ACCIDENT INVESTIGATION All accidents and near miss events will be investigated by the WW Gay safety department and Slone Associates safety personnel (Bill Klima). The investigation will seek to determine what caused the event and what will be done to prevent reoccurrence. The investigation will be documented on an incident report form. Site supervision and the employees involved will be informed of the results of the investigation to educate personnel of the event and how to prevent reoccurrence. 11.0 ASSISTED RESCUE AND EVACUATION In the event that an employee falls, the guidelines below are to be followed to assist the person for rescue: A scissor lift or ladder may be used to rescue a person who has fallen inside the building. The competent person and site safety personnel will be notified immediately if a fall is observed. A person who has fallen outside the building will be rescued by the safest means available. 12.0 NOTIFYING EMERGENCY RESPONSE Call 911 on a cell phone. Inform them that you are at NS Mayport and require emergency response at Bldg. 1553. When reporting an emergency speak clearly: give location of event, what happened, stay on phone as needed for additional information. The Competent Person is to notify Slone Associates personnel immediately after calling 911. Send an escort (if possible) to front gate to guide the rescue team to the fall event. Assist the emergency team as needed by moving objects (construction materials, steel, etc.) with lull to all the ambulance to pass through. If a person falls onto a lower surface and is not in any further immediate danger do not move the victim as this may cause more injury. 9
If a person is suspended in a personal fall arrest system immediate rescue will be performed with an aerial lift if available or suspension trauma rescue steps if available. When rescuing the victim do not place yourself in danger of injury or falling. Position the aerial lift basket close to the fall victim and guiding them into the basket while remaining tied-off. 10
